This position also assists with scheduling of LPNs and administrative duties in office. We are requesting LPNs with the greatest flexibility to be available on an as needed basis Requirements Varied medical coordination with the Services Division Coordinator and Physicians to ensure the timely and accurate treatment and optimal health care is rendered to the persons Nia supports. Maintain Medical charts and usual nursing documentation. Conduct telephonic and in-person coordination with Doctors, Nurses, Services Coordinators.The LPN also is responsible, in consort with the Agency Registered Nurse or Designee, in the distribution of medication, conducting blood glucose checks, blood pressures and other nursing requirements for individuals supported, per doctors' orders. The LPN shall adhere to Nurse License Statutes as well as Nia Association medical policies, to effect proficient and accurate medical care to the person(s) assigned. Appointments Along with the PC, maintain a monthly calendar of medical appointments for the assigned individual(s) in the agency. Review Physician Order Encounter Forms to ensure Prescription Protocols have been followed. Assist PC with coordinating appointments and write out all notifications of appointments. Ensure all patient paperwork (lab work, etc.), is returned and placed in client's files. Ensure client attends follow-up appointments (if necessary). May coordinate annual, semi-annual and/or quarterly exams. MAR Update MARs and Prescriptions, if necessary, prior to the RN's check. Along with the Agency RN, it is the LPN's responsibility to review MAR's before filing in main chart. Medical Profiles must be updated on a daily basis if any changes occur. Responsible for applying correct physician orders and distributing monthly report to the RN or designee. Deliver monthly MARS before the end of the month to PC and/or HD for distribution to home. Prescriptions Any newly prescribed psychotropic medication or psychotropic medication changes shall be immediately changed on all documentation (MAR, charts, etc.). All psychotropic changes or additions must go through the Incident Review Committee. all individual prescriptions and refill requests are properly documented. Vitals Obtain vital signs, heights and weights for individuals each month or as required in the ISP. For those unable to be weighed, a designated place will be determined and info shared with Program Coordinator, Agency Nurse and filed IAW Nia Policy. Professional Contacts / Training A prerequisite for this position is to maintain a professional and positive working relationship with various medical facilities, state monitors, conservators, advocates and pharmacies. LPN will partner with the Agency to occasionally teach subject areas that may further aid in the delivery of quality services. Subject areas may vary depending upon the health status of the individuals being served such as, but not limited to, seizure, diabetes, skin assessments, bed baths, etc. These trainings may require that they be taught to staff in the individual's homes. LPN will conduct both new hire training and staff refresher training on basic first aid situations, patient advocacy (how to assist client at routine doctor's appointment, etc.), and signs and symptoms (respiratory infections, UTIs, stroke, etc.) to watch for in clients. Committee Participation LPN may be on the Incident Management Team and will provide input concerning possible interventions. LPN communicate with the Incident Review Committee Liaison to provide needed medical input and explanation to any issues brought forth. Controlled Substances / Disposal of Medication The controlled substances maintained in the home or main office shall be counted at distribution. The check should be initialed by the Agency RN or designee for accountability. EmPATH will provide these services to individuals who present to the program or are transferred from area EDs. Initial admission will be on a voluntary status to adults (18 years and older) who are presenting with a mental illness and/or co-occurring disorder and experiencing a behavioral health crisis. This position hires, trains, and supervises nursing staff for EmPATH. Oversees scheduling, monitors training needs, and functions in a leadership role in the multi-disciplinary EmPATH treatment team. This position will be responsible for ordering supplies for the unit and monitoring all nursing duties. Completes chart reviews to ensure agency compliance. Provides direct care to clients as needed and assists with crisis interventions and medical emergencies, Serves as a liaison with other agencies as indicated.
