Center Administrator - Adult Day Health Services
Salary: $87,000-$95,000 (Exempt) Schedule: Onsite, Monday-Friday, 8:00 AM-4:00 PM Reports To: Director of Adult Day Health Services
Preferred: Registered Nurse (RN)
Easterseals is seeking a mission-driven Center Administrator to lead daily operations of our Adult Day Health Services program. This role oversees staff, ensures regulatory compliance, supports clinical coordination, and promotes high-quality, person-centered services for older adults and adults with disabilities. An RN is strongly preferred due to clinical oversight responsibilities.
Key Responsibilities
Program Leadership & Supervision
Provide leadership and oversight to all program staff
Support participants with personal care (toileting, transfers, ambulation, feeding)
Recognize signs of illness and assist participants who become ill
Support clinical staff with vitals and HIPAA compliance
Regulatory & Operational Oversight
Primary contact for caregivers, case managers, and participants
Conduct annual case record reviews and lead Plan of Care (POC) meetings
Ensure complete and accurate documentation
Submit incident reports and maintain communication with support systems
Participate in agency compliance and required trainings
Financial & Program Management
Assist with operational and strategic planning
Maintain utilization targets and ensure services align with budget
Participate in committees and cross-functional initiatives
Program Activities & Support Services
Oversee therapeutic, educational, and health-supportive programming
Assist with meal service, transportation, and program documentation
Staff Recruitment & Development
Recruit, hire, and train Activity Assistants and other staff
Ensure staffing levels meet regulatory requirements
Provide coaching, feedback, and accountability
Support statewide program needs as required
Family & Community Engagement
Foster culturally respectful interactions
Ensure participant preferences are followed per their POC
Maintain a safe, clean, and welcoming environment
Quality Improvement
Identify opportunities for service enhancement and growth
Participate in ongoing compliance and training initiatives
Qualifications
RN preferred (strongly encouraged)
Minimum of 2 years of full-time supervisory experience in a social or healthcare setting (required)
Experience in adult day health, senior services, healthcare administration, or human services
Knowledge of state and federal regulations for Adult Day Health Services
Strong leadership, communication, and organizational skills
Ability to manage budgets and daily operations
Must work onsite Monday-Friday, 8:00 AM-4:00 PM

Elwyn seeks a Special Instructor to provide direct specialized instruction to preschool children with special needs in community and home based settings.
DUTIES AND RESPONSIBILITIES:
Provide special instruction as recommended in each child's Individual Education Plan (IEP) within the context of the natural environment
Complete required documentation including daily Service Verification Notes, Biannual Progress Monitoring, and other necessary reports

POSITION SUMMARY: The Group Home Behavior Technician provides direct support to people with developmental disabilities who exhibit acute challenging behaviors in a clinically-driven Applied Behavioral Analysis (ABA) residential program.

EDUCATION/EXPERIENCE/SKILLS REQUIREMENTS:

Job Description POSITION SUMMARY: As a member of the Interdisciplinary Care Team, the Nurse Case Manager is responsible for the coordination and implementation of health care services for each individual residing in Elwyn New Jersey's community living program sites and enhanced behavior homes.
The nurse provides clinical and consultative services and educates the team about consumer-specific needs.
The nurse also liaisons with external providers to facilitate care coordination and ensures that established standards of care are met.
The nurse practices within the scope of New Jersey Board of Nursing laws and in accordance with Elwyn New Jersey's policies and procedures and those of Elwyn New Jersey's regulatory agencies.
DUTIES AND RESPONSIBILITIES: Review ongoing medical, dental, and psychiatric treatment with the treatment team to ensure recommendations are addressed in a timely manner Assist in identifying health care needs and provide site staff with individual training that focuses on general health conditions, treatments, and interventions specific to each individual's health needs and consistent with standards of care Review all Medical Appointment Forms, lab data, and diagnostic test results with the Treatment Team Provide continuity of care through timely follow up with physicians and other health care providers to ensure that agency and licensure requirements are met Monitor individuals' weights and vital signs on a monthly basis and more frequently as needed; identify changes from baseline and report abnormal findings to the Primary Care Physician and Treatment Team.
Summarize Medical Appointment Forms, telephone contacts, and ongoing health care follow up in the nursing progress notes Monitor consumer immunization status and ensure that each consumer remains up to date and current (as per NJ State regulatory guidelines) Monitor consumers for potential safety risks and the need for adaptive equipment, monitor all adaptive equipment to ensure that it remains in good working order and facilitate repair/replacement when equipment issues are identified Complete a monthly review of medication boxes and Medications Administration Records for each site.

POSITION SUMMARY: The Program Specialist implements, oversees, and ensures that the services provided in the Community Living Program (CLP) program adhere to applicable regulations and that the services which are provided meet the needs of the individuals' receiving services, according to the Individual Support Plan (ISP). The Program Specialist develops, completes, and oversees the completion of required documentation; assesses individual needs; coordinates and secures services; trains staff; attends and leads meetings; communicates with all team members, including family members; and ensures the continuous development and monitoring of programming and services to people for whom they are responsible. The Program Specialist ensures that services provided are fully compliant, meeting the needs of individuals receiving services and are consistent with high quality care standards and best practices in service delivery.
DUTIES AND RESPONSIBILITIES:
Ensure that Program Coordinators (PCs) and Direct Support Professionals (DSPs) comply with implementing and/or overseeing processes and activities, including but not limited to a system of safe practices, inspection readiness always, and Health Insurance Portability and Accountability Act (HIPAA) compliance by conducting ongoing training and audits to support these compliance measures
Ensure sites operate in accordance with the organization's quality standards for safety, cleanliness, comfort, and state licensing and accreditation standards by, for example, leading staff meetings to train staff with the Program Coordinator and report any deficiencies back to the appropriate director of operations
Develop and maintain individual electronic and paper records to ensure accurate documentation
Conduct all admissions, transfers, and discharge activities including but not limited to completion of admission packet, coordination of initial ISP meeting, completion of transfer documentation and records, preparation of discharge letter, etc. to assist admissions/transfers, etc.
The Program Specialist provide details regarding the services available within the agency to the team and the Service Coordinator (SC) to ensure accurate documentation
Contribute to the development of outcomes specific to the services provided to ensure the outcomes are appropriate for each individual's needs
Assist with the establishment of units, start/end dates, etc., for identified services included in the ISP and confirming their accuracy within the ISP to ensure accurate billing
Report changes in individual service needs/preferences which may cause the need for changes to the ISP or effect the provision of services to ensure the individual is receiving all appropriate and accurate services
Review provided information related to the individual's receiving services including the Person-Centered Planning Tool (PCPT), previous ISP(s), available assessments, and evaluation data, as appropriate/relevant the individuals needs
Notify the support coordinator and requesting an Interdisciplinary Team (IDT) meeting be scheduled whenever there is a notable change in the individual's status regarding any medical, clinical, or operational needs in the residential and Day Habilitation Programs
Participate in the development and reviewing of the ISP, ISP annual update, and ISP revision and ensuring completeness, content accuracy, and that the ISP reflects the services and supports needed are included and have been provided
Attend all ISP meetings to ensure all pertinent information is conveyed and documented by support coordination
Report content discrepancy and changes related to the individual's needs to the Service Coordinator (SC), as applicable, and plan team members, by using the Department of Developmental Disabilities (DDD) issued worksheets to ensure the content is documented
Keep documentation of all communication with the SC of changes that must be made to the ISP, including changes to outcomes based on a need or revision to the assessment to ensure accurate information is documented
Assist in developing outcomes for individuals based on appropriate assessments provided by the clinical team and training all staff in the implementation of outcomes and strategies to ensure training is accurate
Ensure that all services/supports provided relative to ISP outcomes are reviewed with the individuals and that the ISP includes community involvements such as residential and day program outings, civic-minded opportunities, and membership in national or local organizations etc.
Coordinate and complete the training of DSP in the content of ISP needs relevant to everyone receiving services to ensure that accurate content is trained correctly.
Liaison with all team members, including individual's family, SCs, medical personnel, day program personnel, residential, by all means of communication to ensure all team members are updated with the most recent and accurate information
Aid in the resolution of problematic situations that may occur for all Elwyn New Jersey residential and day habilitation individuals
Promptly report any infractions of failure to follow established Elwyn NJ policies and procedures with DDD, HIPAA, and applicable state/local/federal regulations at all times to designated and authorized person outlined in each applicable policy or regulation
Maintain current state-mandated training and certifications
Follow all current policies and procedures of Elwyn New Jersey
Cooperate with Elwyn New Jersey and the State of New Jersey Department of Human Services, or any other authorized investigating body during an inspection or investigation
Perform cardiopulmonary resuscitation (CPR), safe crisis management, and medication administration using agency-trained protocols
Provide on-call coverage
Perform other related duties as assigned
EDUCATION/EXPERIENCE/SKILLS REQUIREMENTS:
Possess at least a bachelor's degree in a human services field and at least two (2) years experience working directly with persons with intellectual and development disabilities (IDD), OR sixty (60) college credits in human services field with two (2) years case management experience and additional two (2) years of direct support with an IDD population, OR one (1) year IDD case management experience with five (5) years of direct support with an IDD population
Non-profit human service experience highly preferred
Demonstrated ability to coordinate teams and individual staff
Demonstrated ability to work effectively as part of a team
Knowledge of applicable regulations including but not limited to Community Care Program and Supports Program Manual, and applicable New Jersey State regulations

Responsibilities
Elwyn Seeks a Charge Nurse to provide clinical leadership, nursing education, and direct support to the Adult Behavioral Health Programs and Assertive Community Treatment (ACT) teams across Delaware. This role ensures high-quality nursing practice, promotes clinical excellence, supports regulatory compliance, and reinforces a therapeutic, recovery-oriented environment for the individuals we serve. The Charge Nurse serves as a skilled resource and mentor for program nurses, provides LLAM (Limited Lay Administration of Medication) training to Direct Support Professionals (DSPs), and may provide on-site nursing coverage in program homes when needed.
Essential Duties & Responsibilities
Clinical Leadership & Support
Serve as the primary clinical nursing lead for all Delaware sites, offering guidance and expertise in behavioral health and psychiatric nursing care.
Support program nurses with complex clinical situations, medication management, assessments, and care coordination.
Provide direct nursing coverage in a program home during call-outs, vacations, or identified staffing needs.
Participate in multidisciplinary team meetings to promote integrated, person-centered care.
Assist in ensuring compliance with state regulations, agency policies, and best-practice standards.
Education & Training
Provide LLAM training and competency assessments for DSPs across program homes.
Offer ongoing nursing education, skills coaching, and professional development for nursing staff.

Program & Site Support
Travel regularly to all six Delaware sites to maintain a visible and supportive presence.
