Provides direct client care to a diversified population, across a continuum of care Collaborates with staff in assessing client needs and creating and implementing a nursing plan of care Aids Utilization Review in communicating pertinent nursing information Designs and teaches programs for clients/families and aids staff in program implementation Works closely with physicians to provide education and counseling for clients and other organizational health care providers on optimal use of medicines (e.g., proper use, avoidance of over medication) Provides nursing care adapted to the individual client's needs based on the nursing process, which includes assessment, planning, implementation, and evaluation Reports observed changes in mental status and/or physical condition Documents and reports incidents, includingmedication errors Oversees the medication management system that includes but is not limited to; medication planning, storage, ordering, transcribing, administering, monitoring, and evaluating.
