The Center for Recovery and Wellness takes a holistic, strengths-based approach to recovery and wellness that responds to the distinct needs of each individual. Programs include residential and outpatient addiction treatment, recovery services, prevention education, and community programming. CRW offers culturally competent, evidence-based programs and services that strengthen individuals and communities, enhance resiliency, and foster lasting recovery.
Reporting to the Residential Manager and delivering milieu services in the trauma-informed modified therapeutic community, the Motivational Enhancement Counselors will monitor the residential systems and deliver behavioral interventions to the residents for the 100 bed, OASAS licensed residential treatment facility. They will work in an integrated fashion with other clinical program staff to provide effective individualized services and interventions that meet the treatment plan goals and objectives for residents. They will provide a safe haven in which our resident patients may confront their behavior, learn how to utilize their strengths to deal with the many problems related to chemical dependency, and develop the skills necessary to allow them to function as valuable members of the community without the use of drugs or alcohol.
Responsibilities
Direct Services with Credential
Individual and group behavioral interventions in line with the modified trauma-informed modified therapeutic community.
Monitor and coordinate residents' attendance at medical, dental, and psychiatric appointments.
Monitors resident's participation in all residential services with appropriate communication and interventions applied for noncompliance with the program schedule.
Participate in and lead crisis intervention for behavioral episodes or relapse episodes on-site, following facility procedures and documenting accordingly.
Conduct regular and scheduled safety runs, safety searches, and reporting back observations, intervening with residents appropriately, and making recommendations for a follow-up to the team.
Conducts department meetings and committee meetings with residents in line with the modified Therapeutic Community model.
Follows procedures for investigating safety issues, and managing onsite incidents with appropriate written incident reporting and verbal communication.
Counseling
Conduct group counseling sessions with residents.
Acts as a liaison to a clinical counselor and conducts caseload check-in rounds on shift with assigned caseload
Utilize strengths-based counseling and MI spirit as well as employ strengths-based behavioral modification strategies.
Treatment
Act as the SUD “behavioral expert” for participation in treatment planning led by the clinical counselor with the support multidisciplinary team
Document behavioral interventions in the clinical record as per 820 and CARF standards
Communicate any outcomes of individual or group interventions to the clinical team members involved in the resident's treatment plan.
Administrative
Supervise and assist residents with in-house job functions, cooking, clean-up chores, and other daily living activities.
Regular review of resident signs out book and cross-referencing with resident's headcount
Provide recreational activities, including trips.
Maintain patient records.
Dispense metro cards and lunch money to residents as needed.
Collect HRA finger images and other required reporting elements.
Participate in house meetings.
Communicate pertinent information in Staff on duty log, Shift Exchange Meetings and Shift plans
Perform other duties as assigned by supervisor.
Qualifications
A High School diploma or equivalent is required with valid CASAC. 2 years of experience in SUD treatment setting and a Master's degree in behavioral science; social work, psychology, sociology may be substituted for CASAC if the applicant applies for CASAC-T within 90 days of hire.
One (1) year of related work experience in a Residential Therapeutic Community setting is strongly preferred.
Knowledge of strengths-based counseling, cognitive-behavioral treatment, and behavior modification in practice
A degree in a related field (social work/psychology) is a definite plus.
Current adult CPR and First Aid certifications a plus
Ability to communicate effectively both verbally and in writing with a broad variety of people, including other team members and management.
Ability to maintain appropriate professional boundaries and confidentiality.
Competency with computer-based programs such as Microsoft Word, Outlook email, Relias, and Electronic Health records

We are seeking a qualified healthcare professional to be our Clinical Counselor Outpatient Services who will counsel and aid individuals who have substance use disorders and/or their families/loved ones in a New York State OASAS-certified outpatient treatment program. Utilizing a person-centered and trauma-informed lens, individualized assessment and counseling (telehealth or virtual) services are delivered both in-person and virtually at Project Contact Outpatient at the Center for Recovery and Wellness.
Schedule: (3) Weekdays - 10am to 6pm, (2) Weekdays - 12pm to 8pm, and a rotating weekend once a month.
Responsibilities
Provide caseloads with weekly individual case management and counseling sessions consistent with the individualized treatment plan for individuals in the Substance Use Treatment and/or Significant Other Services treatment tracks.
Continuously assess the appropriateness of the current level of treatment, corresponding with LOCADTR and utilization review instruments (Substance Use treatment track).
Coordinate integrated services with medical, mental health, vocational, recovery services, and other social service providers.
Complete detailed progress notes, comprehensive treatment plans, intake/admission assessments, psychosocial assessments, discharge plans, and other required documentation in accordance with OASAS and CARF standards.
Maintain accurate and organized clinical charts, engaging in pre-audits of charts to ensure compliance with OASAS, managed care, and CARF standards.
Send weekly, monthly, and quarterly reports to HRA through the Substance Abuse Tracking and Reporting System.
Specimen collection for clients and monitors for treatment-related issues in order to update the treatment plan (Substance Use treatment track)
Check the toxicology reporting portal daily to ensure any positive toxicology reports are communicated to supervisory staff, documented, and addressed with the client (Substance Use treatment track)
Coordinate integrated services with medical, mental health, housing, vocational, recovery services, and other social service providers.
Facilitation of individual and group-level interventions during other clinician vacations or leave
Ensure that clients' annual medical examinations are completed, with associated referrals as needed.
Contact referent and/or family contacts upon admission of new clients in a timely manner, and provide written or oral clinical updates as needed.
Effective advocacy and collateral contacts to empower clients
Outreach to referral sources to expand the network of clients.
Attend staff meetings and trainings as required, as well as individual and group supervision.

Qualifications
Master's Degree in a mental health discipline (MSW, MHC, MFT, etc), and either licensed in the state of New York, or working under a limited permit and eligible for licensure. OR CASAC credentialing (Credentialed Alcoholism and Substance Abuse Counselor) OR CASAC-T with a bachelor's degree in a human services-related field and a minimum of 2 years' experience working in a substance use program.
Possess a demonstrated ability to deliver patient treatment, including communication skills, successful engagement in specialized human services training, understanding of human interaction and personality development, and the ability to write and record case documentation.
Familiar with Regulatory Agency/OASAS Regulations regarding the operation and practices of patient care.
An understanding of group dynamics is essential.
Must be computer-literate with knowledge of Microsoft Office applications.
Competent in electronic health records that support treatment plans and case notes such as AURA Relias, Netsmart, and AWARDS (willing to learn new systems).

Job Summary: Working from a strength-based; trauma-informed approach, the Youth LifeSet Specialist is responsible for mentoring and providing services to address the needs of young people who are in care and/or transitioning out of care ages 17 to 21. Job Summary: The College Transition and Success Counselor works with students from GSS programs who are both transitioning to and attending designated two and four year CUNY colleges; collaborates with college personnel and GSS program staff to assure a smooth transition to college; mobilizes the supports to sustain college success; participates in strategizing and research to document best practices and successful structures/resources/collaboration and interventions; participates actively in this research initiative; provides outreach to students and staff at referring sites to recruit appropriate candidates.
Major Duties:
Establish and maintain collaborative relationships and communication with CUNY counselors and staff of key college departments to support and track students.
Establish and maintain collaborative relationships with college candidates counselors at GSS Transfer schools, YABCs and day school programs to identify the cohort of students to undertake the application process and transition to college.
Provide individual support and advocacy to students in the cohort.
Develop and implement workshops on transitioning to college.
Arrange site visits to colleges to orient students to place, space and college departments and contacts.
Review and monitor students financial arrangements and financial aid to assure maximum aid is obtained and managed and student can budget appropriately.
Assist students to plan and prepare for CUNY Assessment Test.
Develop academic monitoring and early warning system to address risks to retention.
Arrange study groups/tutoring for students as needed.
Assist in arranging activities and celebrations to advance cohort cohesion and peer support.
Maintain and provide demographic and case notes for data keeping and research. Maintain all required statistical and narrative documentation.
Participate in staff trainings and meetings including partnership meetings/conferences/presentations and professional development.
Perform other duties as assigned.
Qualifications:
Bachelor s degree in related field and at least two years of prior experience working with young adults, required.
Experience with college access including the application process or in college retention, necessary.
Must possess strong teamwork, communication (verbal and written), and organizational skills.
Knowledge of Microsoft Office applications.
Strong ability to engage students and parents in activities that support college completion.
Must have a commitment to working with youth from a strength based and/or youth development perspective.

Major Duties:
Direct Work with Families
Provide home-based and on-site individual, family, and group counseling, as well as short-term crisis intervention, including advocacy and outreach for designated caseload of eight (8) families.
Engage families immediately upon referral, at times participating in Administration for Children s Services (ACS) Child Safety Conferences.
Implement and integrate the principles and procedures of Solution-Based Casework (SBC): assessment, family and individual level outcomes, action plans, and documentation and celebration.
Handle case-related crises with professionalism, urgency, and care, including incidents involving reports to the State Central Registry.
Conduct screenings and assessments for substance use, mental illness, and child safety and well-being, including assessing caregiver capacity, home environment and conditions, discipline methods, etc.
Integrate the practices of SBC into every counseling session.
Ensure that every child has been seen and properly assessed every month.
Meet appropriate case work contacts for each phase of treatment: Initial, Baseline, and Stabilization.
Represent participant needs, concerns, and perspectives at all required ACS and GSS meetings and trainings.
Administrative and Advocacy Role
Collaborate closely with ACS, alcohol and other drug treatment providers, schools, medical and mental health clinics, and other resources serving family members.
Provide consultation to and collaborate with Family Workers as needed to ensure high quality services to families.
Collaborate with ACS Family Services Unit (FSU) staff, Family Court Legal Services (FCLS) staff and other service providers in making safety and treatment decisions.
Attend and participate in Family Court hearings for families involved with ACS FSU.
Facilitate referrals to alcohol and substance abuse treatment facilities, mental health treatment providers, and other service providers, including escorts to appointments as needed.
Actively participate in Clinical Diagnostic Team meetings (CDTs) to provide case information, updates, and assessments in order to obtain recommendations for continued case practice or case termination.
Participate in case consultation, continued SBC training, and the SBC certification process.
Prepare and maintain FASP documentation, monthly statistical reports, and case work notes in adherence with agency policy.
Provide Court reports as needed to inform as to families progress or lack of progress in achieving outcomes.
Maintain appropriate utilization of state and city databases including PROMIS and Connections, as well as other agency databases as needed.
Qualifications:
Bachelor s degree required, (Social Work, Human Services, Psychology, Sociology, Counseling or other humanities degrees will be considered.
