Job Summary:

Essential Functions:
Utilizes principles of education and adult learning to deliver all perinatal education according to CPSP manual guidelines. Assures all patients receive and records appropriate documentation in the patient's record, including combined assessment, first trimester visit, second trimester visit, third trimester visit and post partum visit. Assures care plans are complete and triggers or abnormalities are addressed in a timely manner.
Acts as a liaison between the patient and the team to promote effective communication. Ensures communication between team members and encourages care conferences to evaluate the patient's progress and quality of care. Oversees the patient's chart for completeness and documentation of care. Addresses all fall out and incomplete charts. Assures completeness and adequate documentation.
Interacts with patients, families, guests, co-workers and/or the public to positively promote the hospital and its services. Utilizes knowledge of age-related development and psychosocial needs in assessing and providing treatment.

Staff work directly with students to explore their educational and career goals, guiding them in creating academic- and community-oriented professional roadmaps for success.
The Career Education & Community Engagement (CECE) Coordinator independently develops, implements, and assesses comprehensive career and community engagement programs that prepare students and alumni for professional success and lifelong learning. Working under the general direction of the Director, the incumbent exercises professional judgment to provide advanced career advising, program planning, and strategic partnership development with employers, faculty, and community organizations. The CECE Coordinator independently coordinates major initiatives, provides lead direction to professional and student staff, designs career readiness curricula, and analyzes student engagement data to inform program improvement and reporting. REQUIRED QUALIFICATIONS:
EDUCATION & EXPERIENCE - Equivalent to graduation from a four-year college or university in a related field, including or supplemented by upper division or graduate course work in counseling techniques, interviewing, and conflict resolution where such are job-related and three (3) years of progressively responsible professional student services work. A master's degree in Counseling, Clinical Psychology, Social Work, or a directly related field may be substituted for one year of experience. A doctorate degree and the appropriate internship or clinical training in counseling or guidance may be substituted for the three years of experience for positions with a major responsibility for professional career or personal counseling.

Working Conditions
TYPICAL WORKING CONDITIONS: The Teacher, State Hospitals (Adult Education) used by the Department of Developmental Services, Porterville Developmental Center (PDC) is responsible for maintaining order and supervising the conduct of students; maintaining the safety of persons and property; and delivering classroom instruction to ensure the achievement of adult students using evidence-based practices and recognized instructional strategies to create a learning environment for students to achieve their learning goals. The Teacher, State Hospitals (Adult Education) incorporates activities and instruction designed to build appropriate social emotional skills; and support the students as they develop academic proficiency. Potential exposure to communicable diseases, blood-borne pathogens, and other conditions common to a clinical nursing environment. Ensures the safety of public, staff, and forensic clients, which may include, but not limited to: escorting forensic clients in the facility; distributing and accounting for all training equipment and supplies to prevent their use as weapons; inspecting facilities to identify security breaches that could lead to the escape of a forensic client; and observing and intervening in forensic client behaviors that may injure people, damage property, or signal impending escape attempts. We're looking for someone to join our team as a Nutrition Educator I who:
The Nutrition Educator is a member of the nutrition education team. The Nutrition Educator conducts nutrition education services in the context of a program to reduce the risk associated with nutritional deficiencies in the population they serve. The Nutrition Educator works under the leadership of the Dietitian Supervisor or Nutritionist Supervisor.
Essential Functions:
1. Provides quality customer service to clients and team members with an attitude of helpfulness, dignity and respect.
2. Greets clients and provides general information about WIC and Clinica Sierra Vista services.
3. Obtains nutrition and diet histories of low-risk participants according to established procedures.
4. Weighs and measures participants according to established protocols.
5. Provides education and support of breastfeeding and promote a breastfeeding friendly environment.
6. Identifies patient needs and provide necessary referrals.
7. Schedules appointments and issues WIC food benefits.
8. Screens clients for WIC eligibility, counsel and refer participants as needed.
9. Documents pertinent information in the WIC Program's Integrated Statewide Information System. WIC Wise system.
10. Participates in case reviews, staff meetings, and program planning as needed.
11. For non-WIC encounters, documents and submits the required billing documents for reimbursement and screens referrals in Electric Health Records.
12. Participate in community outreach and promotional activities.
13. Complies with established Clinica Sierra Vista and WIC policies.
14. Maintains confidentiality and security at all times.
15. Upholds the mission of Clinica Sierra Vista and the WIC Program.
16. Performs other work duties as required.
You'll be successful with the following qualifications:
1. High School Diploma or completion of GED.
2. Certified as a Lactation Educator, desirable.
3. Previous nutrient counseling experience desirable.
4. Basic computer skills.
5. Valid California driver's license and current automobile insurance.
6. Precise and conscientious about details.
7. Excellent Communication Skills.
8. Ability to handle multiple tasks well.
9. Ability to work well independently and with others.
10. Bilingual ability (English/Spanish) required.
11. Experience or desire to work with low-income families.
