Description
Northwestern Medicine has an exciting opportunity for either a Nurse Practitioner or a PhysicianAssistant to work in our Interventional cardiology - structural heart/valve department. This position is a combination of 75% outpatient, 25% inpatient consults (not primary management of inpatients). The APP will be responsible for one-month and one-year clinic follow-ups for all structural heart/valve procedures and management of inpatient consults for valve. The program is evolving to support our APPs to expand their scope to evaluation of new outpatients for valve disease, primarily aortic valve for TAVR, and increased independence with inpatient consults.
Schedule: M-F, 5x8-hour shifts, potential for 4x10-hour shifts based on department needs
no nights/weekends/holidays/on-call
Essential Functions:
Demonstrates clinical expertise and expanded knowledge through patient care activities which may include diagnosis and management of a wide range of health problems.
Responsible for health outcome of an assigned patient population.
Confers with collaborating physician and other health professionals in area of clinical expertise.
Develops appropriate educational systems for patients, families and staff in areas of expertise.
Mentors graduate students in aspects of advanced practice and patient care.
Actively participates in quality improvement and/or research pertinent to area of expertise.
Responsibilities:
Conducts comprehensive appraisals of patients' health status by taking health histories and performing physical examinations in clinical area.
Provides direct clinical care to patients.
Prescribes/orders medications, therapies; orders/interprets laboratory tests, x-rays and procedures according to clinical privileges.
Evaluates patients to determine their need for further health care.
Provides comprehensive teaching to patients and families.
Documents in the medical record all care and education provided and/or procedures performed for which the Licensed Advanced Practice Provider is responsible.
Refers to other disciplines as appropriate.
Consultant to other disciplines in areas of expertise.
Integrates research based knowledge and clinical expertise in decision making.
Exemplifies a professional image in appearance, manner and presentation.
Advocate for patients, families, coworkers, and the community.
Maintains certifications and continuing education as required for licensure and privileging.
Serves as a patient advocate and role model for "Patients First".
Participates in unit/departmental/divisional or hospital committees/task forces as appropriate.
Provides comprehensive teaching strategies for patients and their families.

Qualifications
Required:
Master?s degree from an accredited school/college of nursing OR master's degree from an accredited PhysicianAssistant Program
Acute care certification required for APRNs (inpatient experience as an NP will be considered in lieu of acute care certification)
Current license as a Registered Nurse in the State of Illinois. Current license as an Advanced Practice Registered Nurse in the State of Illinois. Certification as an Advanced Practice Registered Nurse by a national certifying body as approved by the Illinois Department of Regulation APRN Board. Once licensed may apply for a controlled substance license and DEA number OR Master?s degree from an accredited PhysicianAssistant Program/Current license as a PhysicianAssistant in the State of Illinois. Privileged and credentialed through the Medical Staff Office (MSO)
Cardio-Pulmonary Resuscitation ? BLS, issued by the American Heart Association
Minimum of two (2) years of experience as an APP in Cardiology is required

Nurse Practitioners practice in our outpatient practices on a collaborative care team composed of a physician, nurse practitioner or physicianassistant, medical assistants, a dedicated medical scribe, and support from registered nurses and care managers. We partner with a network of elite specialists and hospitals for specialty and acute care. As such, our healthcare providers can focus on care within the clinic: primary care, care coordination, and population health.
Core Responsibilities:
Provision of exceptional primary care.
Conduct office visits for routine and acute issues.
Administer Annual wellness visits and health risk assessments, which require a holistic view of health and a focus on thoughtful, accurate, and specific documentation.
Care coordination with other providers, specialists, testing facilities, and agencies.
Population health leadership, in coordination with the Care Team (e.g., making sure all eligible females get their evidence-based breast cancer screening every 2 years)

Qualifications:

Order and interpret labs, diagnostics, and screenings
Hospital and Nursing Home rounding
Develop and manage treatment plans in collaboration with physicians
Prescribe medications within scope
Provide chronic disease management and preventive care
Joint injections, botox, skin biopsies and other minor procedures for internal medicine office
Participate in quality improvement initiatives
