JOB SUMMARY
Under the general direction of the Charge Nurse and CNA Supervisor, the CertifiedNurse Aide will assume direct care for residents in the Care Center in accordance with TJC, federal, state, and local guidelines, organizational and departmental policies, and procedures. Communicates with medical staff, other departments, and outside agencies while maintaining confidentiality. The position requires self-motivation, creativity, and capabilities to function in a semi-autonomous role within a fast-paced and dynamic environment.
STANDARDS OF PERFORMANCE
Assumes direct resident care responsibilities as assigned by the Charge Nurse, to include, but not be limited to AM, PM care, backrubs, assisting with ambulation, assisting with dietary needs, dressing and undressing residents, and changing bed linen.
Carries out Special Orders or treatments as directed by the Charge Nurse, to include, but not be limited to special skincare, collecting specimens, intake and output measurements and documentation, vital signs, compresses, and enemas.
Ensures adherence to proper infection control, OSHA and safety standards.
Provides encouragement to residents and family members.
Observes the resident's condition and report immediately any untoward signs, symptoms, or unusual occurrences.
Maintains current documentation of resident status on Activity of Daily Living Sheet and gives report to Charge Nurse.
Assists in admission, transfer, and discharge of residents.
Answers resident call-lights quickly, whether that of assigned resident or not.
Escorts resident to other hospital services and/or departments as needed.
Accepts and completes Special Assignments in a timely manner.
Assists other staff in the completion of their duties.
Maintains certification in accordance with hospital, state, federal, and TJC regulations and guidelines, including 12 hours of resident-related Inservice Education yearly to maintain state certification.
Maintains constant vigilance over residents, taking necessary safety precautions (rails up, call lights within reach, bed in lower position).
Maintains residents' room in a neat, orderly manner with clear walk areas.
Assists residents to a car or other transport vehicle.
Transports residents, supplies, specimens, etc., as needed.
Relieves in other areas as necessary.
Requirements:
Minimum Level of Education: Education level equivalent to completion of high school, and or completion of CertifiedNurse Aide Training course of study.
Formal Training: None
Licensure, Certification, Registration: Must have and maintain a valid unencumbered Georgia CertifiedNurse Aid Certification, and current Basic Life Support CPR certification.
Work Experience: One-year of experience in healthcare as a CNA is preferred.

Position Purpose
The ALC & DSH CNA/Transportation Professional supports Villagers during medical appointments by advocating for their needs, assisting with the appointment process, and empowering them to make informed decisions about their care. This role ensures Villagers receive the necessary support while respecting their autonomy in healthcare choices.
Required Qualifications
CertifiedNursingAssistant (CNA) in good standing, meeting all relevant Georgia and federal requirements
High school diploma or GED
Proficiency in English (read, write, follow instructions)
Valid Class C Georgia Driver's License, proof of insurance, and acceptable driving record
Successful completion of background check, drug screen, and TB screening
CPR/First Aid certification
Physical capacity to lift up to 25 lbs independently and assist up to 75 lbs with help
Ability to assist Villagers into and out of vehicles safely
Key Responsibilities
Transportation & Medical Appointment Support (Primary Focus)
Provide safe, timely, and compassionate non-emergency medical transportation (NEMT) for Villagers to off-campus doctor visits, therapies, labs, hospital outpatient appointments, pharmacy runs, etc.
Use accessible practices (ramps, lifts, securement) as needed, ensuring that each Villager's mobility needs are addressed and accommodated
Offer emotional support, reassurance, and assistance to Villagers before, during, and after appointments to reduce stress and promote positive experiences
Advocate on behalf of Villagers-help them communicate needs or concerns to medical staff, assist with check-ins, accompany them as needed
Maintain detailed trip logs, mileage records, and required documentation of each outing
Monitor vehicle condition: ensure cleanliness, appropriate fuel levels, safety checks, and report any mechanical or safety concerns
Plan routes to optimize time and safety (considering traffic, Villager comfort breaks, accessibility)
Coordinate pick-up and drop-off times with the care team to minimize waiting and overlap
Follow all vehicle, traffic, and safety regulations; secure mobility devices, seat belts, restraints appropriately
Support Role in Care (Secondary, When Off-Transport Duty)
When not transporting, assist as needed with personal care tasks such as grooming, toileting, dressing, positioning-always in a supportive role and in a way that honors Villager choice
Be available to help fill in gaps in care, offer respite support, or respond to situations during non-transport periods
Communication & Documentation
Observe Villagers' physical, emotional, or behavioral changes during transport or in supportive moments; report concerns promptly to nursing or administration
Document transportation interactions, appointments attended, outcomes, challenges, and any follow-up needs clearly and promptly
Communicate effectively with the care team (ALC and DSH) so everyone knows appointment outcomes, delays, or Villager feedback
Safety, Infection Control & Environment
Adhere strictly to Universal Precautions, infection control protocols, and safety procedures, especially when assisting Villagers in vehicles
Use proper body mechanics when assisting with transfers into/out of vehicles
Maintain hygienic and orderly Villager spaces and vehicles; report repair or cleaning needs
Ensure vehicles are safe and properly clean; monitor maintenance, schedule repairs as needed
Villager Rights, Dignity & Relationship Building
Treat every Villager with kindness, respect, and patience; remain attuned to their emotions and preferences
Protect Villager privacy and choices; never rush or force
Promote Villager autonomy-listen to preferences about timing, seating, reinforcement, and comfort

Job Summary:
The Nuclear Medicine technologist assists with providing safe, age appropriate care to the patient by performing all exams provided by the department per department procedures. The technologist has the knowledge and ability to properly and safely obtain a quality diagnostic study. He/she is responsible for performing a wide variety of technical procedures requiring independent judgment, ingenuity and initiative in the utilization of the equipment for the diagnosis and/or treatment of diseases. The technologist can independently perform job functions. He/she sets up the equipment to provide imaging service and positions patient for the procedure.
Core Responsibilities and Essential Functions:
Patient Care * Verifies proper clinician order for procedure. * Obtains complete clinical history, gathers examination data and assesses for contraindicating conditions. * Follows established procedures for the administration of radiopharmaceuticals so that no patient condition deteriorates, and no patient suffers undue pain, due to error or poor technique. * Ensures that all studies have proper patient identification and right and left marker are utilized appropriately. * Reviews images for clinically acceptable results and releases patients. * Responds to emergency situations and provides for the restocking of the crash cart and maintenance of life support equipment. * Assists other technologists in all modalities when needed. * Observes patient condition and properly evaluates situations when physician intervention is necessary so that no patient condition deteriorates. * Verifies correct identity of patient using 2 unique identifiers prior to performing a procedure per hospital policy and procedure. * Ensures patient's privacy and comfort is accommodated. Quality Control * Consistently produces acceptable images, as evidenced by radiologist feedback and quality audits. * Ensures that equipment is fully operational prior to each use, by checking for proper calibration and operating deficiencies. * Sends all studies to PACS immediately after completion, prints CDs as needed. * Studies that have been transmitted after hours to teleradiography are printed and scanned in PACS, if applicable. * Completes all studies in RIS before completing in PACS. * Prints completed log sheet from teleradiography and submits to Radiologist, if applicable. Job Summary:
The Medical Assistant functions under the direction of the Physician, Clinical Lead, Clinical Nurse Supervisor, Nurse Supervisor and/or Office Manager. Diagnostic Imaging Duties and Expectations
* Perform radiographic and other diagnostic imaging procedures to assist clinical team and physicians in treatment of the patients.
* Prepare and position the patients for imaging procedures ensuring all proper techniques including radiation protection.
* Obtain optimum views as ordered by the physician
* Process images and review for proper identification and quality control.
* Cross train other team members
* Complete and maintain logs, reports, and documentation
Laboratory Duties and Expectations
* Properly start lab equipment daily and maintain daily logs and documentation requested
* Perform laboratory tests according to orders and instructions
* Draw blood samples from patients and perform Point of Care testing
* Order and stock supplies weekly
* Perform daily quality control and cleaning procedures according to guidelines

Patient Liaison

* Always partners with the patient and significant others and/or family (as appropriate) using such appropriate method for setting and purpose (e.g.,Huddles; interdisciplinary rounds; just in time and explanation of physicians instructions ); keeping the patient at ease in the physicians office, and making the patients goals the focus of the plan of care.
* Practices using current clinical practice standards.

* Coordinates the delivery and documentation of safe quality patient care that promotes the professional care delivery model. Job DescriptionDescription:
Under the general supervision of the Director of DON, the Patient Care Technician (PCT) is responsible for assisting the Medical surgical department in providing patient care and meeting the personal care needs of patients as directed. The PCT delivers routine, non-licensed technical bedside care upon the request of licensed staff members and plays an essential role in the Nursing Team. This assistance is provided in accordance with The Joint Commission (TJC) standards, as well as federal, state, and local regulations, while adhering to organizational and departmental policies and procedures.
STANDARDS OF PERFORMANCE
1. Provides basic age-appropriate patient care under the supervision of an RN/LPN.
a) Basic hygiene is provided by offering complete or partial bath as needed (i.e. mouth care, hair care, shaving and catheter care) as assistant to the RN/LPN.
b) Vital signs are accurate and documented correctly according to unit specific policies and guidelines and gathered when requested by the RN/LPN.
c) Provides a secondary visualization of patients for pressure areas and promptly reports to RN/LPN.
d) I & O's and weights are accurate and documented according to unit guidelines when requested by the RN.
e) Patient arm bands are checked upon admission, each shift and as needed.
f) Emptying and recording output from foley catheters and/or colostomies.
2. Laboratory policy/procedures are followed.
a) Two patient identifiers are verified prior to specimen collection.
b) Verify ID band with the specimen label.
c) Other specimens: sputum, stools, urine, etc. are promptly delivered or sent to the laboratory as appropriate.
d) Patient armbands are scanned for bedside glucose testing.
1. Collaborates with nursing and respiratory to provide appropriate respiratory care.
a) Encourages turning, coughing and deep breathing for appropriate patients.
b) Removes and reapplies oxygen when instructed by RN/LPN.
2. Demonstrates patient safety awareness.
a) Ensures identification band is on each patient at beginning of shift.
b) Communicates fall risk and activates exit alarm as indicated.
c) Uses proper body mechanics.
d) Turns and positions patients every 2 hours as assistant with RN/LPN.
e) Demonstrates patient safety awareness.
3. Performs tasks or clinical skills as delegated.
a) Maintains awareness of patient's nutritional status and assists with meals as needed.
b) Performs urinary catheter care every shift and as needed.
c) Maintains infection control measures per policy.
d) Assists patients with readiness for discharge ensuring that all patient belongings are gathered. Upon D/C, prepares room for environmental services.
e) Ensures patient rooms are kept tidy and organized.
f) Restock supplies as directed by RN/LPN, to include linen, lab supplies, IV supplies.
4. Maintains customer service as a priority.
a) Customers and co-workers are treated in a courteous manner.
b) New patients are promptly welcomed with patient care items and oriented to unit.
c) Call lights are answered promptly with message communicated to the appropriate person.
d) Listens to shift report.
e) Discusses patients plan for the day at the beginning of shift.
5. Demonstrates interpersonal behaviors congruent with the basic principles of working well with others.
a) Focus on the situation, issue or behavior, not on the person.
b) Maintains self-confidence and self-esteem of others.

Requirements:
JOB QUALIFICATIONS:
Minimum Level of Education: High School or equivalent; or Patient Care Tech Certification or completion of (1) semester in a clinical nursing program.
Professional Requirements: Graduate of a PCT, NursingAssistant, EMT, CMA, MOA, or U.S. Military Medic Program or Enrolled in a school of Nursing and has completed one (1) full semester of clinical rotations. Patient Care Tech certification preferred.
Formal Training: Additional training regarding glucose testing. (in-house training provided)
Licensure, Certification, Registration: PCT, CMA, MOA, EMT, or NursingAssistantCertificate; also, BLS/CPR.
Work Experience: Preferred 6 months experience in a hospital/outpatient care setting.

Calculator, personal computer, telephone, facsimile machine, paper shredder, copier, printer, standard office equipment, dynamap, telemetry.
