The Customer Service Representative for the Call Center Department serves as a primary source of contact for patients at El Centro del Barrio clinics and as such is responsible for welcoming patients into the system and ensuring they have positive experiences. The CSR is responsible for scheduling, rescheduling, and cancelling appointments for all patients/families requesting health care services. The CSR will answer general questions asked by callers, or transfer callers to other parties if appropriate. The CSR will also make appointment reminder phone calls to patients as required. DUTIES AND RESPONSIBILITIES: Demonstrate ability to provide excellent customer service to all callers including, whenever possible, “first call resolution.” Complete all calls as efficiently as possible, while still providing excellent customer service, to maximize the number of calls the Contact Center can handle and to provide a high service level to all callers. Demonstrate proficiency in looking up patient accounts and encounters in the practice management system, using a variety of fields including but not limited to name, birth date, social security number, and account number. Demonstrate proficiency in scheduling, rescheduling, and cancelling appointments in the practice management system based on patient or clinic needs. Demonstrate a strong ability to schedule patient appointments according to established protocols by service line (medical, obstetrics, dental, and behavioral health) and by provider. Answer telephones using script, identifying self and name of clinic. Demonstrate a basic understanding of insurance (especially Medicaid and CHIP) and eligibility programs. Make appointment reminder calls to patients (if an automated system is not making the calls), and follow up on calls where the automated system is unable to contact the patient. Maintain excellent personal attire to include uniform and nametag. Demonstrate excellent attendance and punctuality. Demonstrate ability to meet department metrics. Abide by policies and procedures dictated in the Employee Safety Handbook. Participate in the agency's Quality Improvement Program. Monday Thru Friday 8am to 5pm Monday Thru Thursday 8:00am to 2:00pm Other duties as assigned by the immediate supervisor.     Full-time Description The Care Coordinator I is responsible for providing care coordination services for clients in their assigned group. The Care Coordinator I will assess clients with all care management needs and address any potential barriers to care, to ensure timely diagnosis and treatment. Under direction of the supervisor, the Care Coordinator I will maintain a clinical tracking system to ensure appropriate follow-up, reporting and billing. DUTIES AND RESPONSIBILITIES Demonstrate proficiency and efficiency in processing referrals (specialty care and ancillary services), verifying insurances, and acquiring authorization for services if needed. ·Demonstrate proficiency in following up on referrals submitted and retrieving progress notes from specialist in order to close out referral loop and assisting with obtaining second referral if necessary. ·Demonstrate proficiency in documenting activities performed on clients in EHR and C3 database, as well as in looking up patient accounts and encounters in the EHR, EPM, and C3 database, using a variety of fields including but not limited to name, birth date, social security number, and account number. ·Documents and updates PHI log in EHR when referrals are sent/processed. · Assist in updating to maintain patient demographics in EPM, EHR and C3 database. · Assists providers with follow-up on referral as well as no-shows as directed by providers or contract staff. Assists with obtaining second referral if necessary · Actively responds to phone calls from contractors and/or patients who may be in need of referrals or medical records for patient's continuum of care. · Demonstrate a strong ability to scheduling, rescheduling, and cancelling patient appointments according to established protocols by service line (medical, obstetrics, dental, and behavioral health) and by provider. Maintains faxes received in-patient EHR and acquires provider's signatures for faxes needing provider approval for services provided. Flags charts for alerting staff regarding non-compliance services or needing updates to demographics. Assist clinical teams in patient assessment via receipt of reports or review of charts that show abnormal labs/findings and notify clinicians in the event of an emergency. Demonstrate proficiency in using great customer services skills/ proper phone etiquette when communicating with patients by phone, text messaging system, or by patient portal · Completes daily tasks assigned by Director/Supervisor. · Completes and runs rescreen report to bring clients back into service. · Attend weekly meetings with department. Assists in maintaining adequate supply of inventory and follows protocol regarding ordering of supplies. Flexibility to travel independently to other locations as needed.     Full-time Description The Accountant I is responsible for assisting the VP/Chief Financial Officer in planning and managing all phases of financial activities to include auditing, budgeting, cost reporting, billing, collections, accounts payable, payroll, cash receipts, pension plan accounting, grant management, contractual compliance and monitoring, financial reporting and presentations. Counsel by telephone on breastfeeding problems and make follow-up phone calls. Distribute breast pumps to those WIC participants who demonstrate a need. As a Medical Assistant you will assist the physician with physical examinations and carry out nursing treatments and procedures according to Physician's documented orders. The Medical Assistant must be well-versed in departmental policies and standards in order to efficiently perform assignments. The Medical Assistant must be able to maintain good public and working relationships and be familiar with community health and social services. DUTIES AND RESPONSIBILITIES: Intake - takes patient history, vital signs, symptoms and conditions and documents such in the medical record. Administers patient immunizations following clinical protocols and provider's instructions. Administers medications and treatments ordered by the provider. Checks the treatment room for properly functioning equipment and stocks supplies. Maintains lab duties to include, but not limited to: daily quality checks and documentation in log books, specimen processing including related result documentation and Provider notification of abnormals, daily cleaning and disinfection of lab and equipment in accordance with infection control guidelines, submits QC and patient testing logs monthly. Travels to other Centro Del Barrio sites in carrying out duties. Checks the treatment room for properly functioning equipment and stocks supplies Assists the provider in specialized examinations and treatments/procedures as directed. Keeps and maintains the inventory for the supply room and assists the LVN Nurse Supervisor in other duties. Performs venipuncture as ordered by provider. Abide by infection control policies and procedures as dictated in the Centro del Barrio Infection Control Manual. Abide by policies and procedures on hazardous materials as dictated in the Employee Safety Manual. Abide by all safety policies as dictated in the Employee Safety Manual. Demonstrate competence with all patients. Participate in the agency's Quality Improvement Program. Performs other duties as required. Requirements Education: High School diploma or equivalent and completion of Medical Assisting Program. Experience: 1 year experience in a clinic or hospital setting preferred. PHYSICAL REQUIREMENTS: Must be able to perform the essential functions of the position. Must be able to meet the attendance requirements of the position. Must not pose a threat to the health or safety of other individuals in the workplace. Physical ability to lift up to 20 pounds unassisted.
