As an Account Executive, your primary responsibility is generating admissions and growing patient census in a designated territory. As a Physical Therapist, you'll contribute to our success in the following ways:
Promotes Elara Caring's philosophy, mission statement and administrative policies to ensure quality of care
Provides direct patient care, evaluates functional needs and outcomes, and consults with other specialists. Reviews patients' functional needs and adjusts care plans to ensure optimal patient outcomes.
Designs and implements a plan of care for patients based on a thorough assessment of potential development and incorporates participation by significant others whenever it seems appropriate.
Conducts therapy practice within defined standards of care.
Directs the assistance of support staff to achieve patients' functional goals.
Documents evaluations, treatment goals, and plans. Regularly updates documentation and maintains prescriptions and signatures.
Provides direction and leadership to staff and acts as a resource to promote physical therapy in the community

What is Required?
Graduate of an American Physical Therapy Association (APTA) approved physical therapy program
Current, unrestricted PT license valid for the state of work
Minimum one (1) year of experience in a healthcare facility
Able and willing to travel within branch/office coverage area.
Must have a dependable vehicle, valid driver's license, and current auto insurance in accordance with state laws.
Must be able and willing to travel 50%

Ensures that all activities performed align with the vision of Elara Caring's board of directors, executive team, and the leadership of the Hospice team.
Assesses hospice care patients and families to identify the physical, psychosocial, and environmental needs of patients as evidenced by documentation, clinical records, Interdisciplinary Team reports, after hours reports, and on-site evaluations.
Assesses patient needs and obtains data on physical, psychological, social, and spiritual factors that may influence patient/family/caregiver health status.
Communicates significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the Clinical Supervisor, physician, facility, and/or other personnel involved with patient care.
Counsels, instructs, and includes the patient, facility, and family in following the Interdisciplinary Plan of Care.
Teaches hospice philosophy and pain and symptom management to staff in alternate care settings (nursing homes, hospital, assisted-living facilities, etc.).
Provides appropriate support at time of death and perform bereavement assessment.

As a Hospice C.N.A /H.H.A., you'll contribute to our success in the following ways:
Verifies the Plan of Care prior to each visit and provides care according to delegation by the RN, physician's orders, assessment data, and established standards and guidelines.
Incorporates patient care goals established in the Plan of Care, into nursing visits, as evidenced by documentation on skilled nursing notes.
