As a Hospice C.N.A /H.H.A., you'll contribute to our success in the following ways:
Verifies the Plan of Care prior to each visit and provides care according to delegation by the RN, physician's orders, assessment data, and established standards and guidelines.
Incorporates patient care goals established in the Plan of Care, into nursing visits, as evidenced by documentation on skilled nursing notes.
Communicates significant findings, problems and changes in condition or environment to the RN Case Manager, the physician, clinical supervisor, and/or other personnel involved with patient care.
Reports unsafe conditions to the appropriate RN Case Manager, clinical supervisor, and physician, as appropriate.
Implements the plan for patient safety, using the patient, family, and community resources.
Assists the patient in learning appropriate self-care techniques while delivering quality patient care.
Suggests revisions to the plan of care in response to identified patient care issues and notifies the RN Case Manager.
Provides those services required substantial and specialized nursing skills under the direction of a Registered Nurse or physician.
Collaborate with the RN Case Manager during normal business hours for advice, assistance, and instructions.
Documents medication regimen and updates medication profile according to established procedures.
Treats patients and caregivers in the highest and most effective manner.
Implement all available actions to prevent avoidable hospitalizations and ER visits.
Provides positive, supportive communication to physicians, patients, families, visitors, and agency personnel.
What is Required?
High School Diploma or GED equivalent required
1 year of experience as a Hospice Aide or Nursing Assistant is required
State-established or other Hospice Aide training program that meets the requirements of 42 CFR §484.36 and a competency evaluation program
Experience in a home health environment is preferred
50% travel required
Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.
Valid driver's license and insurance and reliable transportation to perform job tasks

The Pharmacy Graduate will participate in a 12-month post-PharmD training program with a primary focus on Physician Engagement, including but not limited to; the use of digital applications, data, and insights to enable informed provider decisions. The Physician Engagement Pharmacy Resident will gain managed care proficiency through in-depth longitudinal rotations and as well as enterprise-wide exploratory rotations, while delivering clinical support to the Provider Engagement, Data and Analytics, and Physician Innovation teams.

**RESPONSIBILITIES**
+ Recommend medications, including initiation, continuation, discontinuation, and alternative therapies based upon established protocols.
+ Utilize Express Scripts' multiple data analytics programs and tools to monitor and analyze trends in providers prescribing patterns. Collaborates with client, client's Express Scripts' account team and others to optimize opportunities to inform and influence providers. The goal is to improve clinical and financial outcomes and support trend management including appropriate promotion of generic and formulary prescribing toward client and Express Scripts' strategic goals.
+ Provide clinical support for Physician Innovation teams' research and development efforts, predictive capabilities, and reporting and outcomes processes.
+ Activities and discussions to gain knowledge of Pharmacy Benefit Management (PBM) services, including claims processing, coverage reviews, pharmacy networks, formulary management, and client management.
+ Effectively participate in and contribute to staff meetings, committees and departmental work groups to share best practices, improve processes and/or other items relating to provider engagement.
+ Provide patient and health care professional education and medication information.
+ Design and present education and/or training activities via newsletter publications, clinical journal clubs, and department in-service opportunities.

Summary: National EMR consistently seeks talented and hard-working individuals who are eager to assist impacted communities and first responders on an “as needed basis” as reservist employees. Reservists (YOU) are part of an elite, all-hazards response cadre who are available to work intermittently, commit to working when needed, and travel/deploy within 24-48 hours. The cadre plays a key role in keeping National EMR ready to deploy the right people in the right jobs quickly in an emergency management situation. These situations could include hurricane responses, mass shooting(s), wildfire responses, and more. As a company we have responded to every major US incident that has occurred in the last 25 years. This is NOT a full-time job. Reservists may be deployed to fulfill any role specifically related to the incident, and deployment roles will be based on reservists' skills, performance, and experience. National EMR does not guarantee length or frequency of deployments. YOU will be paid for the actual number of hours worked. Pay scales are based on the job classification as determined by National EMR.
National EMR was built on the premise of filling gaps and serving those with the greatest needs in a disaster. National EMR has spent most of our time in the emergency support functions (ESF) of mass care, emergency assistance, housing, human services, public health, and medical services as our core mission areas. Reservists are responsible for maintaining personal readiness and your “active/eligible” status. Reservists will be entering into a fast-paced, ever-shifting, high-pressure environment. YOU are expected to perform well, make informed decisions, and be prepared to work nights, weekends, and holidays.
To successfully perform the essential functions of a reservist, the employee may be expected to perform the following, with or without reasonable accommodations, in accordance with the Americans with Disabilities Act of 1990.
Responsibilities and Duties:
1. Understand and adhere to the National EMR Policies and Procedures Manual, Uniform Handbook, and Operations Manual.
2. Foster a culture that encourages team members to work safely, identify potential hazards, and report safety concerns immediately to their direct chain of command.
3. Maintain active status within the agency's platforms, including learning management systems to complete training that may be directed toward self-improvement, mission-specifics, or Just-In-Time (JIT) training.
4. Responsible for vital signs on patients.
5. Responsible for assisting patients with activities of daily living to include but not limited to grooming and bathing, helping patients with eating, assisting with mobility, etc.
6. Ensure complete and appropriate documentation on medical records.
7. Collaborates with nurses (LVNs and RNs) for duties or tasks to be completed by end of shift.
8. Familiarity with signs and symptoms of improved or declined medical status.
9. Reports changes in patient's conditions quickly and efficiently to LVN or RN.
10. Ensures patient's comfort and well cared for while under the care of Doctors and Nurses.
11. Ensures patient's room are stocked with adequate supplies as deemed necessary.
12. Know and recognize the signs of abuse, neglect, exploitation, and sexually provocative behavior when observing clients and implement professional response, reporting, and documenting procedures.
13. Maintain first aid kits per licensing standards.
14. Assist in medical intake process in collaboration with other medical staff.
15. Maintain confidentiality of health care records and information in keeping with HIPAA, Protected Health Information (PHI) requirements and National EMR confidentiality protocol and procedures.
16. Assist with distributing daily medical list to all designated staff.
17. Participate in transport of clients to and from the medical clinic for medical appointments.
18. Implement National EMR safety protocols including evacuating with clients and other staff in case of an emergency.
19. Other duties as assigned.
Requirements:
1. Maintain current certification for Certified Nursing Assistant.
2. Must be willing to obtain additional licenses/certifications as needed/required.
3. Adhere to acceptable standards of professional integrity and accountability in the workplace, and comply with all federal, state, and local laws, rules, and regulations in all duties.
4. Provide proof of valid driver's license and driving record in good standing with no restrictions (subject to review annually).
5. Demonstrate the ability to:
a. Respond sensitively and competently to the service population's cultural and socio-economic characteristics.
b. Work collaboratively with other staff members, service providers, professionals, and clients.
c. Maintain professional people skills and non-verbal communication skills.

• Assumes responsibility for varied resident care duties as assigned by charge nurse to include admitting, discharging, transferring and caring for the resident's personal belongings and assisting the professional nurse in performing resident treatments;
• Bathes the resident in bed, tub or shower, combs hair, cleans and cuts fingernails and gives shampoos;
• Makes both occupied and unoccupied beds;
• Assists with self-medications;
• Assists with treatments and vitals, as necessary;
• Verifies that all residents have wrist bands, medication, picture, and door tags;
• Accompanies residents when transported outside facility for appointments, as needed;
• Assists the charge nurse during the resident admission assessment;
• Participates in the formulation and follow through of the resident's nursing care plan;
• Takes and records weights, blood pressures, temperatures, pulse and respiration;
• Assists all residents with their meals and provides in between meal nourishment, as well as providing fresh ice and water to the resident's bedside, as needed;
• Answers resident's call light or bell, delivers messages, administers bedpans and urinals, cleans areas of spillage or accidents caused by the resident;
• Participates in the orientation of new nursing assistants;
• Reports any/all changes in resident's condition, any family concerns and resident's complaints to charge nurse and/or supervisor;
• Collects and bags soiled linen and delivers to dirty linen area; if assigned, washes and dries items in domestic washer/drier;
• Obtains clean linens and supplies from supply areas as directed, cleans utility rooms, kitchen areas and all other nursing areas, as assigned;
• Cleans and sanitizes resident equipment; must have a good understanding of basic medical asepsis, sterile techniques and universal precautions to avoid infection of residents and self-contamination of equipment and supplies.
• Receives and escorts visitors, collects, charts, records and reports;
• Transfers resident's belongings and equipment as directed, and assists with postmortem care;
• Accept assignments as staffing needs require and performs duties in accordance with well-established methods and techniques in conformance with recognized healthcare standards and practices;
• Report any occupational exposures to blood, body fluids, infectious materials, and or hazardous chemicals in accordance with facility policy.
• Report work related injuries and illnesses immediately to your supervisor.

As a Director of HUD CNA/PCNA Services Division, you will be the lead technical resource and main contact for clients and sales teams. You will oversee the management of both in-house, full and part-time personnel, as well as per-project contracted individuals. The Director of HUD Assessment Services oversees the entire project cycle, from engagement to deliverable, and ensures that compliance with applicable standards, technical quality of reports, and client scope of work are met or exceeded. This position requires effective communication skills, both written and verbal. The success of this position is dependent on the ability of the Managing Director to articulate highly technical concepts and recommendations to a largely non-technical audience.
Role and Responsibilities
· Provide bids for services to clients and sales teams as needed. Ensure engagement letters are properly prepared and projects are properly entered into the BBG project tracking system;
· Effectively communicate project expectations (i.e. Client-specific SOW, templates) to team members (Project Managers and contract assessors) in a timely and clear fashion throughout the course of the project. Delegate tasks and responsibilities to appropriate team members as needed. Oversee all site visit setups and client/site contact communications;
· Collaborate with engineers, architects, and other professionals to gather necessary information for HUD assessments;
· Provide reasonable recommendations and solutions to clients, while understanding the deliverable's potential impact on the transaction process;
· Identify and resolve issues with team members and clients;
· Deliver appropriate progress reports (verbally or e-mail), recommendations to the client and sales teams;
· Perform a final reviews, to ensure quality and client readiness of all portions of reports, including text and appendices;
· Manage client follow-up, questions, and concerns;
· Provide feedback on reports to Sr. Project Managers, Project Managers, and contract assessors as needed.
· Interface with client/borrowers/HUD reviewers to address issues and particular project parameters.

Position Summary
Under the direct supervision of the Central Admissions Nursing Supervisor, the Central Admissions Medical Assistant will perform clinical and administrative duties and delegated functions as a member of the patient care team to provide a variety of multi-skilled services.
Essential Functions
• Prioritizes work responsibilities daily, decreasing waiting time. Collaborates with providers, clinical and clerical support staff to facilitate appropriate patient flow. Effectively works as a member of the health care team assisting other team members as needed or requested.
• Prepares patients to be seen by provider.
• Accurately performs and documents patient vital signs and medical histories.
• Independently and accurately performs appropriate CLIA waived tests, blood alcohol concentration tests, and EKG using proper technique and procedure
• Performs blood draws and sends samples to lab for processing
• Maintains a clean, orderly, safe working environment. Oversees maintenance of supplies and documents in medical logs as directed.
• Setup and stock exam rooms and clean rooms at end of each day worked.
• Assist prospective clients with admission to the Medication Assisted Treatment Program services by gathering data, screening clients, coordinating treatment services, obtaining records, collaboration with other agencies, and providing referrals to appropriate community resources when indicated.
• Answering telephone in a polite and timely manner, screening phone calls to ensure caller is transferred to the appropriate dept./person, retrieves messages, distributes voice mail message and faxes to appropriate staff.
• Orient prospective clients on program services and expectations.
• Obtain new and concurrent insurance authorizations.
• Collects fees and records payments; coordinates accounts receivable to include the billing and insurance authorization processes; photographs clients and prepares client ID cards; electronically prepares medical records for new admissions and accompanying information; processes client refunds; typing for clinic staff members including correspondence to outside agencies; interoffice memorandums, staff meeting minutes, etc.
• Data collection and reports as requested by the Nursing Supervisor.
• Adheres to all infection control standards, follows Standard Precautions and safety policies and procedures of the Agency.
Minimum Qualification Requirements
High school graduate required, plus appropriate certification preferred. Prior experience with phlebotomy required, prior medical office/health aide experience working with physicians and nurses preferred.
