We're seeking compassionate caregivers in the Toledo, OH area who are available for night and weekend shifts, including 12-hour rotations (7:00 a.m.-7:00 p.m. or 7:00 p.m.-7:00 a.m.) and evening shifts (9:00 p.m.-5:00 a.m.). This role provides 1:1 hands-on home care for clients who require consistent support, including some 24-hour care cases.

What You'll Do:
Provide personal care and companionship to clients in their homes
Assist with bathing, housekeeping, meal preparation, and other daily living needs

What you'll do on an Average Day:
Ensures that all activities performed align with the vision of Elara Caring's board of directors, executive team, and the leadership of the Hospice team.
Conduct initial and ongoing spiritual assessments of patients and families to identify their spiritual needs and preferences.
Develop and implement individualized spiritual care plans as part of the interdisciplinary team.
Provide spiritual guidance, counseling, and emotional support to patients and families, respecting cultural and religious diversity
Facilitate discussions about meaning, faith, forgiveness, and other end-of-life concerns.
Participate in interdisciplinary team meetings to ensure spiritual care is integrated into the overall hospice care plan.
Maintain accurate and timely documentation of all spiritual care services provided, ensuring compliance.
Provide bereavement support to families before and after the patient's passing.
Educate patients, families, and staff about the role of spiritual care in hospice
Advocate for patients' spiritual needs within the care plan and ensure their beliefs are honored.
Promotes Elara Caring's philosophy, mission statement and administrative policies to ensure quality of care.
Maintains patient and staff privacy and confidentiality pursuant to HIPAA Privacy Final Rule.
Performs other duties/projects as assigned.
What is Required?
Bachelor's or Master's degree in divinity, Theology, Pastoral Care, or a related field.
Certification or ordination as a clergy member, chaplain, or spiritual counselor
2 years of experience working at a hospice, counseling, or ministry environment
50% travel required
Sit, stand, bend, lift and move intermittently and be able to lift 50 lbs.
Valid driver's license and insurance and reliable transportation to perform job tasks

As an Account Executive, your primary responsibility is generating admissions and growing patient census in a designated territory. This customer-facing (outside sales) role involves the sales, growth, and marketing of clinical programs and outcomes to be a top provider of care in our communities. As a Registered Nurse Behavioral Health, you'll contribute to our success in the following ways:
Assesses assigned case load of behavioral health care patients and families to identify the physical, psychosocial, and environmental needs of patients as evidenced by documentation, clinical records, Interdisciplinary Team reports, after hours reports, and on-site evaluations.
Assumes primary responsibility for caseload, including assessing, planning, coordinating, implementing, and evaluating the plan of treatment.

Teaches behavioral health philosophy and pain and symptom management to staff in alternate care settings (nursing homes, hospital, assisted-living facilities, etc.).

What You Bring
Graduate of an accredited PTA program recognized by the American Physical Therapy Association.
Active, unrestricted PTA license for the state of Michigan.
At least six months of PTA experience (home health experience a plus).
Reliable transportation with valid driver's license and current auto insurance.
Ability to travel within the service area (up to 50%).
Physical capacity to lift 50-100 lbs and perform frequent movement throughout the day.

Reviews and clears HCHB coordination notes and processes administrative task workflows in an accurate and timely manner.

Counsels and instructs the patient and family in meeting therapy and related needs.

What We're Looking For
High School Diploma or GED
1+ year of experience as a Nursing Assistant or Hospice Aide
Completion of a state-approved Hospice Aide training program with competency evaluation (per 42 CFR ยง484.36)
Hospice or home health experience preferred
Reliable transportation, valid driver's license, and auto insurance (50% travel)
Preferred to live in the service area
Ability to lift 50-100 lbs and perform hands-on caregiving duties

What You'll Do
Provide skilled, compassionate hospice care to patients and families during after-hours and on-call shifts.
Assess and respond to changes in patient condition, ensuring comfort and dignity.

As a Licensed Vocational / Practical Nurse you'll contribute to our success in the following ways:
Verifies the Plan of Care prior to each visit and provides care according to delegation by the RN, physician's orders, assessment data, and established standards and guidelines.
Incorporates patient care goals established in the Plan of Care, into nursing visits, as evidenced by documentation on skilled nursing notes.
Communicates significant findings, problems and changes in condition or environment to the RN Case Manager, the physician, clinical supervisor, and/or other personnel involved with patient care.
Reports unsafe conditions to the appropriate RN Case Manager, clinical supervisor, and physician, as appropriate.
Implements the plan for patient safety, using the patient, family, and community resources.
Assists the patient in learning appropriate self-care techniques while delivering quality patient care.
Suggests revisions to the plan of care in response to identified patient care issues and notifies the RN Case Manager.
Provides those services required substantial and specialized nursing skills under the direction of a Registered Nurse or physician.
Collaborate with the RN Case Manager during normal business hours for advice, assistance, and instructions.
Documents medication regimen and updates medication profile according to established procedures.
Treats patients and caregivers in the highest and most effective manner.
Implement all available actions to prevent avoidable hospitalizations and ER visits.
Provides positive, supportive communication to physicians, patients, families, visitors, and agency personnel.
What is Required?
Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
Current State License as a Licensed Vocational / Practical Nurse
1 year of experience in a clinical care setting
Experience in a hospice or home health environment is preferred
50% travel required
Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.
Valid driver's license and insurance and reliable transportation to perform job tasks
You will report to the Clinical Team Manager.
