Great Life work Balance position with excellent benefits! We are seeking a Physician who is passionate about providing quality care to the geriatric population. The Physician participates as a member of the PACE Interdisciplinary Team in the care of high risk older adults living in the community with a strong focus on the participants goals as well as excellent geriatric care management. The Physician is the leader of the Clinical Team whose primary mission is to assist frail older adults to remain living in the community for as long as it is safe. Care meets the objectives, standards and policies of the PACE model. Incumbent demonstrates proficiency in collaborating with an interdisciplinary team and treating participants with multiple chronic and acute illnesses from diverse backgrounds. This is a full-time position, Monday- Friday 8-4 position; no nights, weekends or holidays!
Responsibilities:
Completes initial, periodic and annual physical examinations of the participants.
Evaluates participants for episodic visits, for acute illnesses and for follow-ups of hospitalizations in the clinic, at home or at a nursing home.
Makes nursing home visits in accordance with the state nursing home requirements and as needed.
Use telehealth as needed and where appropriate
Participates in the PACE hospital rounding system and the on-call schedule.
Evaluates the needs of the participants and develops treatment plans in concert with the other members of the Interdisciplinary Team.
Perform other duties as required.
Qualifications:
Bilingual/Bicultural desirable.
Demonstrated ability to work within urban communities, particularly with racially, ethnically, and culturally diverse, low income, and special needs populations.
Board Certified in Internal Medicine or Family Practice, or eligibility for Board Certification (for recent graduates).
Board Certified in Geriatric Medicine, preferred.
Current License to practice medicine in the Commonwealth of Massachusetts.
Current Massachusetts and DEA Prescription Privileges. Suboxone Waiver preferred.
Maintains appropriate hospital and long term care facility credentials and privileges.
Current CPR Certification.
1 year geriatrics experience preferred
Ability to be able to relate well to participants, to anticipate their needs and to encourage their independence.
Preferred: At least one (1) year of experience working with low income and diverse cultural groups.
Ability to convey information clearly and succinctly with team members.
Ability to be able to work well with others and to be a dedicated team member.
Ability to pay attention to detail, to be a self-starter, and to complete assignments on time with minimal direction.
Ability to be able to quickly recognize situations/problems before they become acute, and to be able to implement effective solutions and understand consequences.
Must be comfortable with palliative and end of life care.
Must be comfortable doing home care visits

The Department of Dermatology at the University of Massachusetts is currently recruiting several BC/BE General Dermatologists to meet the needs of our expanding program within the UMass Memorial Health system with locations in Worcester, Leominster, Clinton, Sturbridge, Westborough, the Milford area, and soon Marlborough. We are recruiting for both academic and community practice positions.
Our dermatology practices provide care to the local community and additionally serve as a safety net as well as tertiary referral center for the diverse patients of central Massachusetts. Our physicians see the full spectrum of skin conditions, including patients with complex diseases referred by both community and other UMass physicians. In addition to practicing clinical dermatology, our faculty actively participate in resident and medical student education, lecture at local, national, and international conferences, have opportunities to participate in clinical research, and take on leadership roles. The Position
Job Title: Rheumatologist
Reports to: Medical Director / or Designee
Work Schedule: Normal business hours are 8:00 am to 5:00 pm; Monday Friday (Work schedule negotiable). Other hours and / or days as needed for program activities.
Overview of the Position:
Under the direct supervision of the Medical Director, the rheumatologist will be responsible for the diagnoses and treats arthritis and other immune-related diseases and conditions.
Education and Experience:
Medical Degrees appropriate to their specialty from an accredited medical college or university
Completion of an approved Residency Program
Must be Board Eligible or Board Certified
Required Skills / Abilities:
Ability to work cooperatively with patients, health care staff members and administration in the performance of assigned duties
Ability to exchange information on factual matters communicate in a variety of manners with patients, staff, and explain policies of the Centers.
Ability to apply common sense understanding to carry out instructions furnished in written, oral or diagram form.

Essential Job Functions: These functions will be performed . and will be the responsibility of the incumbent working with the provider, clinical staff and non-clinical staff to ensure excellence in service for the patient.
Prescribing medications to treat conditions such as arthritis or Lupus
Conducting physical examinations, including taking x-rays and MRIs as needed to diagnose conditions
Performing biopsies to remove tissue samples for analysis by a pathologist or other specialist
Providing consultations to other physicians regarding difficult cases that they are unable to diagnose or treat
Diagnosing patients with rheumatic diseases, such as rheumatoid arthritis, systemic lupus erythematosus, Sjogren s syndrome, scleroderma, and osteoarthritis
Performing clinical research to advance the understanding of rheumatic diseases
Referring patients to other specialists when appropriate to help them manage their conditions
Prescribing medications to treat rheumatic diseases and other conditions that cause inflammation in the body
Teaching patients how to manage their condition, work with their physician on a treatment plan, and prevent flare-ups of their disease from occurring

This is a mobile practice: The physicians visit facilities in a mile radius of practice location.
Our physicians visit 2-3 facilities per day treating approximately 20-30 patients per day.
Clinical documentation is at point of care, in real-time during clinical rounds, using our proprietary wound specific EMR.
Paid Training is provided during onboarding + 6 months wound care fellowship
