Eliot is seeking dedicated Care Managers to join our Behavioral Health Community Partner (BH CP) program. This innovative Care Management program serves members with complex medical, behavioral health, and social needs. As a Care Manager, you will work with a multidisciplinary team to conduct comprehensive assessments and develop treatment plans for clients. In this role, you will build relationships with clients, medical and health providers, family and other service providers. This position requires highly motivated and organized individuals capable of managing a caseload of clients.
Responsibilities:
Promote hope and empowerment through support and meetings with clients
Empower clients to exercise autonomy in all aspects of their lives
Help clients identify choices and develop self-advocacy skills
Provide interventions focused on problem-solving, skills training, behavior modeling, optimism, and encouragement
Support the team and clients in the ongoing assessment of symptoms and responses to interventions
Ensure that all required assessments are completed in a timely manner in compliance with MassHealth and contract guidelines
Contribute to the development of assessments and treatment plans for each client.
Ensure treatment goals are culturally competent, strength-based, realistic, attainable, and focused on recovery and clients' wellness goals
Engage clients in the treatment planning process, ensuring their preferences are embraced and collaborated with the enrollee's PCP and Care Team
Ensure that enrollees are treated with dignity and respect in accordance with ECHS' policy
Qualifications:
Requires a Bachelor's degree in Psychology, Social Work or related field
Experience working with adults with mental illness/substance abuse
Valid Massachusetts drivers license
Schedule: Monday-Friday, 9am-5pm

Eliot is seeking a strong, experienced, licensed RN, Nurse Case Manager, to join our Program for Assertive Community Treatment (PACT) team. PACT is an evidence-based model that provides comprehensive treatment, rehabilitation, and support to adults with severe and persistent mental illness.
The ideal candidate would possess knowledge of psychiatric medications, psychiatric and medical treatment. This person would be comfortable with outreach, case management and providing support in individuals homes.
Responsibilities:
Act as a primary case manager to 3-6 consumers
Provide nursing interventions to individuals
Facilitate and coordinate medical care to include communication with health care providers
Provide health education to individuals
Assist in the management of medications and the overall functioning of the nursing department
Formulate and regularly update Comprehensive Assessments and IAP's for clients on your caseload

Qualifications:

Overview The Community Crisis Stabilization Program (CCS) is a short-term behavioral health stabilization program. The NurseManager is responsible for working closely with the Program Director to oversee the nursing services for the Youth CCS unit (ages 13-18) and Adult CCS unit (ages 18+). The Community Crisis Stabilization Program (YCCS) NurseManager, with the Program Director, has twenty-four hours a day/seven a week oversight of the medical and clinical aspects of the YCCS. The YCCS serves as a medically necessary, less restrictive, and voluntary alternative to inpatient psychiatric hospitalization. The Program NurseManager ensures proper standards and regulations are adhered to as set forth by the Massachusetts Nursing Practice ACT and MAP requirements, funding source regulations, and Vinfen company policies.

Arranges and performs intake procedures and conducts initial holistic assessment to

The essential job duties/responsibilities of the position include but are not limited to the information listed below:
• Promote the engagement, housing permanency, community tenure, achievement of greater self-sufficiency and successful completion of tenure for all PS living at the site.
• Oversee management of medical conditions of residents to proactively address the complex medical conditions of the PS, including diabetes; kidney, pulmonary, mobility, physical safety and end of life issues; medical consequences of HIV/AIDs virus, and incontinence, etc.
