Eliot's Homeless Services is seeking a creative/strengths-based Housing First Coordinator to work within our innovative Enhanced Navigation and Stabilization Team which provides services to support homeless individuals in securing and maintaining housing. Eliot is committed to a Housing First Model that prioritizes permanent housing as the initial step in assisting individuals experiencing homelessness.
In this role you will identify and address barriers to obtaining and maintaining long term sustainable housing utilizing a housing first approach within this cutting-edge multidisciplinary team model (e.g. specialized treatment modalities to address post-traumatic stress, Recovery Coaching, in the field Psychiatry/MAT). Working knowledge of area resources/providers is a plus.
Responsibilities:
* Engage people served within a client centered/harm reduction/Housing First framework. make collateral contacts, and consult with other providers (i.e., mental health, primary care, etc.). Ensure that support services and interventions are person centered/strength based/recovery oriented
* Collaboratively develop and implement viable pathways to short/long term sustainable housing with people served.
* Forge and maintain positive and productive working relationships with community-based stakeholders and coalitions
* Provide ongoing assessment of client needs for clinical, supportive services, and safety issues. Develop, modify and implement individual client service plans to address ongoing clinical and/or systemic needs - including risk factors.
* Recognize and respond in real time to sudden changes in client behavior, physical health and/or related medical issues, or other related areas of concern.
* Develop and implement crisis response planning as needed.
Qualifications:
* Requires a Bachelor's degree in a human services field; social work, psychology, human services, or sociology preferred or 2 years case management or comparable experience with the homeless population.
* Valid Massachusetts Drivers license

Eliot is seeking a Vocational Coordinator to work with our Clubhouse program. The ideal candidate will have familiarity with concepts and implementation of Individual Placement and Support (IPS) as well as Clubhouse-model Transitional and Supported employment.
Grounded in the principles of psychosocial rehabilitation and recovery, the clubhouse operates as a member-driven program where individuals actively participate in all aspects of service delivery and are empowered to shape, evaluate, and improve program operations according to their needs and preferences.
Responsibilities:
* Work in conjunction with program members, supervisor, staff and community employers / resources to meet or exceed Dept. of Mental Health (DMH) program employment goals (minimum 26% of program membership employed in community-based jobs).
* Meet with all members interested in employment individually to gather employment history, job preferences, strengths and challenges, current entitlements, transportation resources, preferred supports, identify intermediary steps towards employment goal, and resume creation / updating.
* Provide assistance with member job training / coaching as needed that is in alignment with employer specifications. Provide supervision to other clubhouse staff who are engaged in member job training to insure consistency.
* Facilitate weekly "Workers Night" employment support meeting and dinner (Mondays 4 - 6pm).
* Build and maintain mutually beneficial relationships with member employers by demonstrating understanding of their business needs, prompt response to issues or problems with members on the job and recognition of the positive role they play in member's success / recovery.
* Provide ongoing / as needed support / check-in with working members to ensure their success. Note and report any member job changes such as pay rate, hours worked, job title for DMH reporting purposes with Assistant Director.
Qualifications:
* Requires a Bachelor's degree in Psychology or related field
* Experience working in a residential setting with adults experiencing severe mental illness
* Experience with vocational counseling and vocational development
* Valid Massachusetts drivers license

How You Will Be Making a Difference
Under the general direction of the AVP for Transitional Aged Youth Services, plan, direct, oversee and manage the operations and staff within the program(s). Collaborate externally with referral sources to ensure a steady flow of eligible clients to their program(s). Establish, maintain and nurture working relationships with community agencies, and federal, state, and local agencies to ensure a steady flow of eligible clients to program(s).
Establish, review and implement program(s) policies and procedures. In consultation with Senior Director, continuously consider new processes and initiatives to improve and/or expand services as appropriate.
Maintain effective working relationships with agency staff. Leverage the breadth and depth of agency resources available to ensure the successful management of the program(s). Responsible for compliance with policies of the agency at the Program(s) level.
Participate in the development and implementation of the annual operating budget for the program(s). Monitor performance against approved budget allocations, investigate variances, and implement corrective actions as necessary. Communicate with Senior Director on issues related to budget or budget requests.
Oversee the intake and discharge process and ensure all appropriate paperwork and all data entry is completed and delivered in a timely, efficient manner. Maintain program(s) census and ensure program(s) is following relevant agency guidelines.
Oversee risk management and participate in crisis resolution. Review, approve and process critical incident reports to Quality Management and appropriate staff.

About the Program
The Cambridge/Somerville Community Service Agency (CSAs) serves youth with Serious Emotional Disturbance (SED) and this program is part of the Massachusetts Children's Behavioral Health Initiative (CBHI).
Our primary goal is to help families strengthen their skills and knowledge in order to move towards their envisioned future.
Under the direction of Program Director and Senior Intensive Care Coordinator; facilitates the development and implementation of an Individual Care Plan and risk management/safety plan for MassHealth youth with serious emotional disturbance.
How You Will Be Making a Difference
* Acts as a liaison with collateral agencies and programs; establish and maintain relationships/services that support children and family growth. Act as an advocate for families; educate parents on advocating for themselves and their children.
* Identifies and actively assist youth and families to obtain and monitor the delivery of available services including medical, educational, social, therapeutic, or other necessary services including formal and natural supports and community-based agencies, services, and organizations
* Perform initial assessments and evaluations of new clients and families. Develops, implements, and monitors Individual Care Plans and Transition Plans.
* Maintains working relationship with local providers of all services in order to facilitate referrals from these providers, including ESP/Mobile Crisis Intervention providers, to ensure care is properly coordinated

About the Role
Under the direction of the Program Director and Senior Fit Clinician, the FIT paraprofessional provides support to youth under the age of 21 with Serious Emotional Disturbance (SED) SUD, ASD, IDD and their families under the CBHI framework. The FIT paraprofessional uses a trauma-informed, culturally responsive, and strength-based approach to implement the individualized treatment plan authored by the FIT master's level clinician. The focus is to prompt behavioral support to help improve overall emotional regulation, social skills and strength family structure.
How You Will Be Making a Difference
* Provide caregivers with tools to build advocacy and navigate community resources and larger child-serving systems.
* Support caregivers with advocating for the needs of their youth
* Coach families on how to implement effective strategies to address emotional, behavioral, mental health needs
* Provide education around child development, parenting strategies, mental health and trauma
* Connect families to resources in their community to address the identified needs
* Participate in monthly meetings facilitated by the FIT clinician
* Complete progress notes in the electronic health record system
* Maintain regular communication with FIT team regarding status updates on progress and challenges
* Collaborate with CBHI service, emergency provider, families and other relevant supports identified by the family
* Provide coaching in support of decision-making in both crisis and non-crisis situations
* Work with assigned youth and family 3-5 times weekly managing a caseload of 5-6 families
* Participates in individual, dyad, and group supervision, weekly supervision, and regular staff meetings and training as required.

The LTSS Supervisor provides team oversight of care coordination and clinical care management for MassHealth Members with complex medical and behavioral health needs who are enrolled in an Accountable Care Organization (ACO) or Managed Care Organization (MCO) plan. The Supervisor collaborates with the Community Partner team and the clinical staff of each Enrollee's ACO/MCO's plan to minimize duplicative efforts, promote integrated care, ensure quality and continuity of care, and support the values of person-centered planning. The Supervisor is at the helm of organizing and coordinating resources and services in response to the Enrollees' healthcare needs across multiple settings, and inclusive of both LTSS and SDH needs. The Home in Walpole is a residential treatment placement for adolescent youth who need specialized treatment as they heal from the traumatic events that have shaped their young lives. Youth who live on the campus are part of a greater community which includes Clifford Academy, a Massachusetts Chapter 766 Special Education School. The campus is located on 166 scenic acres in Walpole and offers recreational activities such as a state of the art gym, and walking trails for youth to explore. Our strength-based clinical services help youth learn to manage their behaviors while preparing them for life in the community.
The clinical nurse specialist (CNS) is the clinical leader for a program or area of nursing practice within the organization. The advanced knowledge and skills required for this role include clinical expertise in a focus area, evidence-based practice, collaboration, consultation, education, mentoring, and change leadership.
The CNS and the Nurse Manager are partners in leading the nursing clinical practice area. The CNS coordinates and guides clinical activities/projects of nurses within a practice area and is responsible for maintaining current professional knowledge and competencies and contributing to the advancement of the practice of nursing at the unit/system, local, state and/or national and international level.
How You'll Be Making a Difference
Direct Patient Care
* Serves as a reliable source of information on the latest evidence supporting cost-effective, safe nursing practices
* Collaborates with the multidisciplinary team using the nursing process to integrate the nursing perspective into a comprehensive plan of care for the patient/family
* Identifies and prioritizes nursing care needs for patients/families
* Conducts comprehensive, wellness and illness assessments using established or innovative evidence-based techniques, tools, and methods
* Initiates and plans care conferences or programs for individual patients or populations of patients
* Designs and evaluates innovative educational programs for patients, families, and groups
Nursing/Nursing Practice
* Collaborates with others to resolve issues related to patient care, communication, policies, and resources
* Creates and revises nursing policies, protocols and procedures using evidence-based information to achieve outcomes for indicators that are nurse-sensitive
* Identifies facilitators and addresses barriers that affect patient outcomes
* Leads clinical practice and quality improvement initiatives for a unit or a program
* Collaborates with nurses to develop practice environments that support shared decision-making
* Assists the staff in developing critical thinking and clinical judgment
* Creates a nursing care environment that stimulates continuous self-learning, reflective practice
* Leads in the conduct and utilization of nursing research
Organization/System Leadership
* Consults with other units and health care professionals to improve care
* Leads/assists institutional groups to enhance the clinical practice of nurses and improve patient outcomes
* Develops, pilots, evaluates, and incorporates innovative models of practice across the continuum of care
* Designs and evaluates programs and initiatives that are congruent with the organization's strategic plans, regulatory agency requirements, and nursing standards
* Participates in need identification, selection and evaluation of products and equipment
* Advances nursing practice through participation in professional organizations, publications, and presentations
Qualifications
* Master's degree in Nursing with a clinical focus. Graduate preparation must have included clinical application of the CNS role component.
Desired qualifications
* CNS Advanced Practice Certification, Certification in area of clinical specialty Clinical nursing experience within the last 5 years
* Minimum 3 years + clinical nursing experience required
* Clinical knowledge in specialty area. If hired in area with a substantially different clinical knowledge set, the candidate will be expected to demonstrate clinical expertise in the practice area within 6 months to a 1-year time frame.
* Demonstrated skills at the level of an advanced practice nurse in leadership, communication, collaboration, mentoring, and change as evidenced by portfolio, recommendation letters, references, interview, and demonstration
* Demonstrated ability at the level of an advanced practice nurse to utilize evidence to develop, teach, guide, and implement practice standards and policies

Actual compensation will be based on a variety of factors, including a candidate's relevant experience, education, skills, and internal equity relative to other employees in similar roles. Candidates with less directly related experience may be placed toward the lower end of the range, while those with extensive or specialized experience may be placed toward the higher end. Market conditions and budget considerations may also influence final pay decisions.

Requirements:
Minimum working towards a Master's degree in social work or counseling and entering your 2nd year.

Eliot's Homeless Services Division is seeking a knowledgeable Tenancy Support Manager to work in our Permanent Housing and Low Threshold Housing Programs across the State.
The ideal candidate will bring housing and mental health experience. The responsibilities include site visits, engagement with tenants and landlords, acting as the primary point of contact, along with solid knowledge of policies and procedures and the ability to support a team of case managers.
Responsibilities:
Coordinate lease-up process; meet new tenants on-site at units, complete necessary documents, and review Eliot's leasing policies and procedures.
Supervise Housing Coordinators
Coordinate with landlords and tenants to complete on-site unit inspections for each rental unit at new tenant lease-up, tenant exit, annually, and as needed.
Serve as primary contact with landlords; triage to other staff as needed.
Serve as the main point of contact for assigned tenants to address any housing related needs.
Foster relationship and coordinate with case management staff at partner service provider agencies to discuss tenant issues and successes related to housing.
Attend monthly service provider/landlord meetings.
Triage and respond to unit transfer requests and reasonable accommodations and ensure all required documentation is complete and submitted.
Develop expertise in contract and legal requirements for leasing programs including tenant/landlord law; eviction process.
Qualifications:
Bachelor's degree and 2 years of related experience.
Weekly travel across the Metro Boston and Northeast region required. Mileage reimbursed at the federal rate.
Understanding of homeless services and nonprofit organizations preferred.
Understanding of housing first practices, including low threshold and trauma informed approaches preferred.
Bilingual candidates fluent in a needed language may be eligible for a 6% salary increase
Schedule: Monday-Friday, 9am-5pm

Eliot is seeking a Vocational Coordinator to work with our Clubhouse program. The ideal candidate will have familiarity with concepts and implementation of Individual Placement and Support (IPS) as well as Clubhouse-model Transitional and Supported employment.
Grounded in the principles of psychosocial rehabilitation and recovery, the clubhouse operates as a member-driven program where individuals actively participate in all aspects of service delivery and are empowered to shape, evaluate, and improve program operations according to their needs and preferences.
Responsibilities:
Work in conjunction with program members, supervisor, staff and community employers / resources to meet or exceed Dept. of Mental Health (DMH) program employment goals (minimum 26% of program membership employed in community-based jobs).
Meet with all members interested in employment individually to gather employment history, job preferences, strengths and challenges, current entitlements, transportation resources, preferred supports, identify intermediary steps towards employment goal, and resume creation / updating.
Provide assistance with member job training / coaching as needed that is in alignment with employer specifications. Provide supervision to other clubhouse staff who are engaged in member job training to insure consistency.
Facilitate weekly “Workers Night” employment support meeting and dinner (Mondays 4 - 6pm).
Build and maintain mutually beneficial relationships with member employers by demonstrating understanding of their business needs, prompt response to issues or problems with members on the job and recognition of the positive role they play in member's success / recovery.
Provide ongoing / as needed support / check-in with working members to ensure their success. Note and report any member job changes such as pay rate, hours worked, job title for DMH reporting purposes with Assistant Director.
Qualifications:
Requires a Bachelor's degree in Psychology or related field
Experience working in a residential setting with adults experiencing severe mental illness
Experience with vocational counseling and vocational development
Valid Massachusetts drivers license
Schedule:
Monday-Friday, 8am-4pm

$45,000 - $50,000 USD
