The position will assist individuals in navigating access to whole-person care (e.g. behavioral and physical), facilitates screening and assessment tools, provides referral and coordination for internal and external health needs, and helps to engage individuals in recommended care. The Care Coordinator works directly with individuals experiencing behavioral health challenges, as well as with individuals' supports, internal ELC team members and external community agency partners. This position reports directly to the QI Director. MAJOR JOB RESPONSIBILITIES: Welcomes individuals to ELC through exceptional customer service, to help, provide information about ELC, the services we offer and community services responsive to their needs Assists individuals in smooth transitions to different types/levels of care Assists individuals in completion of required documentation Assists in completion of client health measures (e.g. blood pressure, height, weight) Responsible for data entry and monitoring through excel spreadsheets and Electronic Health Record Monitors assigned CCBHC (Certified Community Behavioral Health Center) outcomes and coordinates with internal and external providers to increase frequency of desired outcomes Coordinates care among providers and provides necessary follow-up Documents activities in individuals' electronic health records Maintains confidentiality of all employee and client information Completes other duties as directed Completes all required background checks as assigned Complies with all ELC policies and procedures EDUCATION and/or EXPERIENCE: Graduation from High School or the equivalent, some college preferred Minimum 3 years' clerical or general office experience/training required, medical office experience a plus Must possess a compassionate attitude and present a professional demeanor Familiarity with and/or lived experience with behavioral health challenges is preferred Must be able to successfully complete all required background checks KNOWLEDGE, ABILITIES, AND SKILLS: Ability to communicate effectively orally, digitally, and in writing Ability to perform tasks accurately, effectively, and in a timely manner Ability to perform basic mathematics calculations accurately Ability to understand and follow oral and written instructions Ability to successfully complete required trainings Ability to establish and maintain effective working relationships with service recipients, other employees, officials, and the public Has a demeanor that is pleasant, friendly and helpful to staff and visitors Willingness and ability to support a diverse population of clients Ability to safely perform essential job functions with or without reasonable accommodations Possesses and supports ELC values of integrity, respect, quality, professionalism, and innovation Join us in our commitment to equality and diversity as an Equal Opportunity Employer (EOE). Complete timely and accurate documentation of all crisis and after-hours work. Travel within Franklin and Miami Counties as needed to provide crisis response. Maintain a valid driver's license, reliable transportation, and proof of automobile insurance. Outpatient Responsibilities Maintain a small outpatient caseload (approximately 10 hours/week of non-crisis therapy). Meet or exceed minimum billable service expectations. Complete initial assessments and treatment plans. Provide individual, family, and group therapy as appropriate. Collaborate with ELC staff and external agencies to support coordinated care. Prepare reports and maintain clinical documentation in accordance with agency, contractual, and regulatory standards. Comply with all ELC policies, procedures, and clinical expectations. MAJOR JOB RESPONSIBILITIES: Assists in developing and providing structured activities to include attendant care, psychosocial group activities and individual community support Maintains a safe and productive environment during time spent in attendance of consumers Provides transportation to and from various activities or appointments (must maintain a good driving record) Provides medication observation in the community and in supported housing environment Maintains appropriate records for reporting, evaluation, accounting, and clinical purposes Meets deadlines for daily paperwork and reports Provides quality documentation Completes other duties as deemed appropriate by immediate supervisor Complies with ELC policies and procedures EDUCATION and/or EXPERIENCE: High school diploma or G.E.D. Minimum of 40 hours of training in providing equivalent supportive services to child and/or adult consumers OR be able to attend 40 hours of on-site training.
