Social Worker

This position is responsible for providing direct high quality psychotherapy services to patients in assessments, diagnoses, treatment planning, group treatment, individual treatment, family treatment, and discharge planning. The Social Worker assists patients and their families in understanding and adjusting to diagnoses and potential treatments. Social Workers can also help identify resources during treatment and after discharge. The Social Worker effectively participates as a member of the multidisciplinary team and the unit community relations team.
Responsibilities:

Designs and Implements a treatment plan of care for patients based on a thorough assessment of the patient's symptom presentation, professional/referral input and all other available data

Actively involved in assessing patient progress and communicating results and trends to supervisor and treatment team
As directed, conducts individual psychotherapy sessions in accordance with state/regulatory standards, displays sufficient theoretical knowledge of psychopathology and effective treatment techniques
As directed, conducts family therapy sessions in accordance with state/regulatory standards, displaying sufficient theoretical knowledge of family therapy principles/psychopathology and effective treatment techniques
As directed, conducts group therapy sessions, displaying good theoretical knowledge of group therapy principles/psychopathology and effective group therapy techniques

Provides group and individual therapies on weekends as directed by supervisor
Conducts therapies in accordance with patient diagnosis and needs
Provides ongoing education to patients and families as needed.
Provides appropriate resources and support for patients before, during, and after treatment
Engages in timely, efficient discharge planning. Works in close conjunction with patient family, nursing staff and attending physician. Displays thorough knowledge of referral/placement process and available community resources, and makes effective contact with disposition sources
Completes progress notes and assessments according to program procedures
Participates as an active member of the interdisciplinary and treatment teams
Submits timely and accurate CQI+ outcome data on a weekly basis
Actively involved in measuring patient performance; communicates results and trends to supervisor
Provides feedback to supervisor on an ongoing basis in regards to concerns, improvements, changes, etc.
Attends Unit and Hospital meetings as directed
Identifies areas needing improvement and utilizes the facility performance improvement process
Actively participates in department processes as requested
Ensures facility is safe and reports deficiencies to the Program Director
Ensures confidentiality of all information encountered

Discharge Planner- Behavioral Health- PRN
Horizon Health is seeking a per diem discharge planner to work on our behavioral health unit at Great Plains Hospital in Elk City, OK. The Case Manager will be responsible for facilitating the continuum of care. Identifies discharge needs on admission. Engages in timely, efficient discharge planning. Works in close conjunction with the patient, family, physicians, unit staff and community resources to ensure safe discharge in the appropriate level of care with the necessary services. Arranges for after care and maintains follow up contact with patient, family or facility. As a member of the interdisciplinary team, works with the team to identify discharge needs and services, and is responsible to identify discharge needs in treatment planning. Works closely with physicians to secure prescription, physician orders and completion of physician documentation required for discharge. Develops and maintains patient discharge information for aftercare tracking. Develops and maintains relationships with community service agencies and residential facilities.
The hours will very between 8-20 hours per week based on census with an average of 16 hours per week. This will be Monday-Friday, no weekends.
Responsibilities:
Assesses and reassesses the needs of the patient and accurately documents assessments and discharge needs
Monitors on a daily basis the status of patients and communicates status to team members
Completes and coordinates discharge with the treatment team and physician within the designated time frame
Completes discharge planning forms and patient discharge forms by date of discharge
Documents on the patient chart the specifics of problems associated with implementing the discharge plan and actions being taken to solve problem(s) including the overall effectiveness of the action plan.
Communicates daily with physician to ensure complete documentation of primary/etiologic diagnoses, all appropriate comorbidities and complications
Reviews and discusses discharge plan with patient and/or family receiving input from them as well as providing explanation, information and support for acceptance and implementation of the discharge plan and obtains signature for involvement in treatment planning
Communicates with referral sources including, family members, legal system, psychiatrists, primary care physicians, and other treatment facilities
Ensures that patient is discharged to a safe environment with necessary follow up care prior to discharge
Ensures after care medical and psychiatric appointments are made prior to discharge
Gathers follow up data on all patients at designated times
Utilizes feedback regarding performance and uses supervision to improve performance
Maintains schedule for filing court paperwork and hearings. Works closely with court personnel to maintain good relationships
Addresses issues in a timely manner including crisis and higher level of care referrals
Maintains appropriate ethical and clinical boundaries with client, family members and other staff members
Demonstrates knowledge of and adheres to LPS and Mental Health policies and procedures
Maintains compliance with state and federal confidentiality laws as well as upholding our Horizon's confidentiality policies and procedures

DeAngelo Contracting Services (DCS) is seeking a Railroad Supervisor to join our growing team. The Railroad Supervisor oversees the performance of assigned Technicians and is responsible for ensuring the completion of various tasks, such as spraying and/or clearing railroads and crossings, in accordance with contract specifications.
Duties and Responsibilities:
Lead and manage a team of technicians, providing direction, support, and performance feedback. Ensure crew members adhere to all safety guidelines and operational procedures.
Oversee the execution of daily tasks, such as spraying railroad crossings, clearing brush, and maintaining railroad right-of-way, ensuring all activities meet contract specifications and safety standards.
Management: Ensure proper use and maintenance of company vehicles, tools, and equipment, including trucks, sprayers, weed eaters, and chainsaws.
Monitor safety practices on site, conduct safety briefings, and ensure that all crew members are properly trained in safety protocols. Enforce adherence to safety regulations and OSHA standards.
Coordinate daily operations, including assigning tasks and ensuring that work is completed in a timely manner. Maintain accurate records of work completed, equipment used, and any incidents.
Serve as the main point of contact between the team and management. Maintain clear communication with project managers, MTM (Maintenance and Training Management), and other relevant departments.
Address any issues that arise during operations, whether related to personnel, equipment, or project specifications. Make decisions to ensure smooth and efficient operations.
Provide training to new employees, ensure ongoing development of the team, and maintain a high level of job competency.
Qualifications
A Commercial Pesticide Applicators License in the relevant state categories for Vegetation Management is preferred but not required.
Previous supervisory experience is preferred.
Must be able to pass a pre-employment drug test, DOT physical, and a background check to work on railroad property.
A valid driver's license is required with 5 years clean MVR.
Willingness to travel for at least 4 weeks at a time with a week off between assignments.
Basic knowledge of weed, grass, and brush types.
In-depth understanding of chemicals used, their control functions, and all safety protocols.
Familiarity with application methods and equipment.
Basic knowledge of calibration and spray patterns.

Essential Duties and Responsibilities:
To perform this job successfully, an individual must be able to perform each essential job duty satisfactorily. The essential job duties for this position are listed but not limited to the following:
* Responsible for ensuring all shutdowns, controls, transmitters, recorder, chromatographs, analyzers, and measurement facilities are calibrated and functioning properly.
* Assist in servicing the compressors, expander, plant compressors and associated equipment.
* Respond to alarms and problems within the gathering system, compressor stations, and plants and takes corrective measures.
* Maintain and adhere to required trainings, policies, and procedures.
* Maintain a valid Driver's license in state of residency
* Live within 60 - 65 miles of reporting location
* Actively participate in safety programs and initiatives.
* Communicate with partnership control centers, business groups, and other operating groups as necessary.
* Comply with company safety policies and procedures.
* Work assigned shift and be available for overtime if needed.
* Perform related duties and responsibilities as required and other duties as assigned.
* Comply with all internal, state and federal regulations.
* Comply with company policies.
* Maintain honest, respectful, and effective internal and external working relationships.
* Complete all training as directed by management.
Required Education:
* High school diploma or equivalent
* 2 year I/E degree or 5 years of experience in I/E field of work.
Required Qualifications:
* Must maintain valid driver's license in state of residency.
* Must live within 60 miles of reporting location to accommodate call outs.
* Excellent organizational skills.
* Excellent written and verbal communication skills with the ability to work effectively in a group environment.
* Demonstrated self- starter with the ability to work independently and exercise good judgement.
* Ability to solve problems and troubleshoot situations.
* Ability to work in a 24/7 operational environment, shift rotations, and perform call outs.
* Must be able to react to high stress/emergency situations in a deliberate, thoughtful, and timely manner.
Preferred Qualifications:
* Minimum of 3-5 years in Instrument and Electrical Controls.
* related experience preferred.
* Mechanical aptitude with ability to learn and perform operation and maintenance tasks associated with various types of facilities and equipment.
* Proficiency with computers including MS Office applications such as Excel and Outlook as well as other computer-based work training, time reporting, expense reporting, and on-line work applications and documentation.
* Working knowledge of P &ID's Electrical Diagrams.
* Current NFPA70E certification preferred.
* Working knowledge of GE and Allen Bradley PLCs i.e. ladder logic and hardware.
* Working knowledge of Liquid and Gas Chromatographs i.e. ABB, AIC, Daniels.
* Working knowledge of gas analytical equipment i.e. AMI H2s and O2 analyzers.
* Working knowledge of specialty electrical tools i.e. Voltmeter, Hart Communicator, Process meter
* 120V, 240V, & 480V Electrical experience preferred.

* Frequent exposure to heat, cold, and other adverse weather conditions.
* Working conditions may include confined spaces.
* Repetitively standing, walking, bending, stretching, reaching over shoulder height, and climbing ladders up to 25 feet. Occasionally lifting up to 50 lbs. with or without assistance.
* Work in and around industrial and construction settings, which may include hazardous products, processes, materials and chemicals, as well as extreme temperature conditions and loud machinery, and may require appropriate personal protective equipment.
