Job Description:
Maintains and follows a schedule of due dates for all MDS.
Coordinates the completion of the MDS by all disciplines.
Reviews the MDS data for accuracy and meets with appropriate staff as needed to assure accuracy.
Completes the MDS and inputs into the computer.
Verifies that assessments have been transmitted and approved by the State in a timely manner.
Corrects any rejected records and prepares them for re-submission.
Completes CAAs according to state guidelines.
Formulates Resident Care Plan (RCP) along with interdisciplinary care plan team for all residents in accordance with their needs and within the required time frame.
Schedules and conducts RCP meetings on a regular and timely basis.
Communicates the RCP to appropriate staff members and notifies supervisor if noncompliance by staff is noted.
Works with all in house and ancillary departments to assure understanding and compliance with the RCP.
Performs other related duties as directed by the DON and -or Administrator.
Job Requirements:
RegisteredNurse, graduated from an accredited School of Nursing and have a current, valid RN license from the North Carolina Board of Nursing and submit proof of license renewal every other year.
Ability to read, know, and follow personnel, department and facility policies and procedures and adhere to local state and federal requirements.
Experience with MDS-RAP and Care Planning functions.
Prefer experience with RUG-IV.

WORK ENVIRONMENT
Nurse Corps Officers may serve at any one of more than 250 Navy and medical facilities around the globe, from Hawaii to Japan, Germany to Guam and Washington, D.C., to Washington state. One year of clinical experience as a registerednurse (RN). If less than 1 year clinical experience as a RN, candidate must be approved by VP clinical.*
Current RN license, specific to the state(s) you are assigned to work.
Minimum six months home health experience.
Current CPR certification.
Valid driver's license, reliable transportation and liability insurance.
Able to assess patient status and identify requirements relative to age specific needs.
Must be familiar with general use and functions of the computer, such as, user names and password concepts; internet; e-mail; navigation of computer desktop, including starting programs, using files and windows, effectively use navigation buttons and tool bars; ability to self-manage online HR services and online training programs.
Note - If less than 6 months clinical experience as a RN, candidate must participate in RN Intern program.

* Coordinates and oversees all aspects of patients' home care.
Collaborates with patient care teams to develop and implement care plans.
Educates families on patient condition, needs and care.
Performs head to toe physical assessments.
Checks vital signs and manages medications.
Gives injections, IV therapy or tube feedings.
Administers and maintains catheters.
Completes documentation timely and accurately.
Supervises LPNs and HHAs.
Regularly communicates patient progress to the clinical manager, physician and care team.
Other duties as assigned.
