PURPOSE AND SCOPE:
Works with the Facility Manager, facility staff and physician to coordinate the facility operations and patient procedures to ensure provision of quality patient care on a daily basis in accordance with policies, procedures and training. Provides nursing support to patients and staff. Assists with staffing, staff training, equipment, physician and patient relations, cost containment, supply management, medical records, patient billing, OSHA and all company, state and federal compliance.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
QUALITY:
Responsible for supporting and driving quality standards through meeting regulatory requirements and the practice of Continuous Quality Improvement (CQI), including use of CQI tools.
Assists with implementing quality goals and develops facility specific action plans to achieve quality standards.
Accountable for outstanding quality of patient care, as defined by the quality goals, by working with management to ensure that policies and procedures are followed.
Assists with implementing appropriate training according to policy; ensure ongoing compliance with all risk management initiatives.
Responsible for aggressively addressing and acting on adverse events and action thresholds.
Ensures all Quality policies and procedures are communicated to and implemented by the facility staff.
Maintains integrity of medical records and other administrative and operational records.
Complies with all data collections and auditing activities.
Maintains facility environmental integrity, including safety.
PATIENT RELATED:
Coordinates all aspects of patient care with the appropriate staff members, from admission through discharge.
May confer with patients prior to the procedure to obtain necessary medical history, discuss procedure and answer questions, as well as, performs nursing assessment on the day of the procedure.
Ensures patients and patient's families are educated regarding post procedure care of vascular access.
Acts as a resource for the staff to address patient concerns and questions.
Ensures patient's procedures are scheduled in a timely manner and the facility capacity is utilized efficiently.
As needed, assist with patient workflow, including providing direct patient care and monitoring pre, intra and post procedure.
Identifies and communicates patient related issues to the Facility Manager.
Oversees and ensures accurate and complete documentation of patient treatments from admission to discharge.
Confirms written orders, consents and lab work are completed and documented in accordance with facility policies. This includes appropriate preparation of lab requisitions, delivery to appropriate labs, reporting and forwarding of lab results to appropriate physician.
Assumes primary responsibility in an emergency situation and must maintain competency with all emergency operational procedures, and initiate CPR and emergency measures as needed.
Administers medications and IV conscious sedation to patients as directed per physician's orders, and in compliance with, company, federal, state and local regulations.
PHYSICIANS:
Ensures regular and effective communication with all physicians on an as needed basis.
Attends and participates in Governing Body, assisting with meeting agenda and gathering of data/material for meetings.
Attends and participates in CQI meetings with physicians, reporting on assigned topics.
Works with the physicians and staff to ensure possession of all necessary instruments and equipment.
MAINTENANCE/TECHNICAL
Ensures appropriate operation of facility equipment and technology, including but not limited, patient monitors, defibrillators, medical recording devices and computers.
Ensures the inspection of all facility equipment on a regular basis. Report any malfunctions or maintenance issues immediately.
Ensures the inspection of all facility operating systems, including temperature checks in appropriate areas and inspect the physical structure of the facility.
Maintains a clean, safe and sanitary environment in the entire vascular access facility at all times.
Ensures all blood spills are immediately addressed according to company Bloodborne Pathogen Control Policies.
ADMINISTRATIVE:
Participates in cost control initiatives. Responsible for efficient utilization of medication, laboratory, inventory, supplies and equipment to achieve supply cost goals following all guidelines established in the formularies.
Assists in the development and revision of any applicable policies and procedures for the facility as directed to ensure continuous process improvement.
Initiates, documents and completes ongoing Continuous Quality Improvement (CQI) activities including monthly reports and attend meetings as required.
Manages medications, including ordering, receiving, and inventory control and checking for expired medications, including appropriate recording of controlled substances as required by law.
Oversees maintenance of usage logs for all medications with appropriate documentation, as well as, records of drug disposal. Communicates with the pharmacist to assure medication accuracy, including appropriate recording of controlled substances as required by law.
Ensures medical records are accurate, complete and in compliance with policies and procedures prior to discharge of the patient. Performs chart audits on a regular basis.
Along with Facility Manager, coordinates the annual medical testing and competency testing for all facility staff.
Participates in all facility regulatory and accreditation surveys and action plans to address any deficiencies identified.
Verifies records for all implanted devices are completed accurately, timely and maintained according to records management guidelines.
Coordinates the collection and forwarding of all vendor invoices to the accounting department.
Acts as the Facility Manager in the absence of the manager or at the direction of management.
Ensure nurses' progress notes are accurate and complete.
Acts as the Patient Safety Officer and conducts quarterly patient safety meetings and fire drills.
Complies with HIPPA policies and standards regarding patient information and medical records.
Other duties as assigned.

Administrative Functions
* Direct the day-to-day functions of the nursing assistants in accordance with rules, regulations, and guidelines that govern the long-term care industry.
* Confirm that all nursing personnel assigned to you comply with the written policies and procedures established by this facility.
* Meet with your assigned nursing staff, as well as support personnel, in planning the shift's services, programs and activities.
* Verify that all nursing personnel comply with the procedures set forth the in the Nursing Policies and Procedures Manual.
* Make written and oral reports/recommendations concerning the activities of your shift, as required.
* Cooperate with other resident services when coordinating nursing services be certain that the resident's total regimen of care is maintained.
* Verify that all nursing service personnel are in compliance with their respective job descriptions.
* Participate in the development, maintenance, and implementation of the facility's quality assurance program for the nursing department.
* Participate in surveys (inspections) made by authorized government agencies.
* Periodically review the resident's written discharge plan. Participate in the updating of the resident's written discharge plan, as required.
* Assist in planning the nursing portion of the resident's discharge plan, as necessary.
* Admit, transfer, and discharge residents, as required.
* Complete accident/incident reports, as necessary.
Charting and Documentation
* Complete and file required forms/charts upon resident's admission, transfer, and/or discharge.
* Encourage attending physicians to review treatment plans, record and sign their orders, progress notes, etc., in accordance with established policies.
* Receive telephone orders from physicians and record on the Physicians' Order Form.
* Transcribe physician's orders to resident charts, cardex, medication cards, and treatment/care plans, as required.
* Chart nurses' notes in an informative and descriptive manner that reflects the care provided to the resident, as well as the resident's response to care.
* Record new/changed diet orders. Forward information to the Dietary Department.
* Report all discrepancies noted concerning physician's orders, diet change, charting error, etc., to the Nurse Supervisor.
* Fill out and complete transfer forms in accordance with established procedures.
* Enter appropriate documentation concerning unauthorized discharges in the resident's medical record in accordance with established procedures.
* Perform routine charting duties as required and in accordance with established charting and documentation policies and procedures.
* Sign and date all entries made in the resident's medical record.
* Write nurses' notes to reflect that the care plan is being followed when administering nursing care or treatment.
Medication Administrative Functions
* Prepare and administer medication as ordered by the physician.
* Responsible for IV medication/fluid administration.
* Verify the identity of the resident before administering the medication/treatment.
* Verify that prescribed medication for one resident is not administered to another.
* Verify that adequate supply of floor stock medications, supplies, and equipment is on hand to meet the nursing needs of the residents. Report needs to the Nurse Supervisor.
* Order prescribed medications, supplies, and equipment as necessary, and in accordance with established procedures.
* Verify that narcotic records are accurate for your shift.
* Notify the Nurse Supervisor of all drug and narcotic discrepancies noted on your shift.
* Review medication cards for completeness of information, accuracy in the transcription of the physician's order and adherence to stop order policies.
* Notify the attending physician of automatic stop orders prior to the last dosage being administered.
* Immediately notify the Nurse Supervisor and attending physician of any medication administration errors.
Nursing Care Functions
* Perform physical assessment through observation, inspection, palpitation, percussion and auscultation.
* Deliver and maintain optimum resident care and comfort by demonstrating knowledge and skills of current nursing practices.
* Promptly respond to call lights and other resident needs.
* Make rounds with physicians, as necessary.
* Consult with the resident's physician in providing the resident's care, treatment, rehabilitation, etc., as necessary.
* Review the resident's chart for specific treatments, medication orders, diets, etc., as necessary.
* Make periodic checks to confirm that prescribed treatments are being properly administered by CNAs and to evaluate the resident's physical and emotional status.
* Notify the resident's attending physician when the resident is involved in an accident or incident.
* Notify the resident's attending physician and responsible party when there is a change in the resident's condition.
* Administer professional services such as: catheterization, tube feedings, suction, applying and changing dressings/bandages, packs, colostomy, and drainage bags, taking blood, giving massages and range of motion exercises, care for the dead/dying, etc., as required.
* Monitor seriously ill residents, as necessary.
* Review care plans daily to confirm that appropriate care is being rendered.
* Confirm that CNAs are aware of the resident care plans. Verify that the CNAs refer to the resident's care plan prior to administering daily care to the resident.
Other Duties
* Attend in-service education programs in order to meet facility educational requirements.
* Be familiar with Standard Precautions, Exposure Control Plan, Fire Drill and Evacuation Procedures and know how to use the information.
* Maintain confidentiality of resident and facility records/information.
* Protect residents from neglect, mistreatment, and abuse.
* Protect the personal property of the residents of the facility.
* Others as directed by the supervisor or administrator.

Purpose Statement

About the role:

You will determine donor eligibility to donate plasma, management of donor adverse events, review of laboratory test results, and donor notification of unsuitable test results. The Medical Support Specialist (Plasma Center Nurse) works under the guidance of the Center Manager (or Assistant Manager as applicable) for operational guidance and under the management of the Center Physician for medical issues. The Medical Support Specialist will be familiar with regulations of the plasma collection industry or a manufacturing environment. The Medical Support Specialist follows guidance provided by BioLife Medical Affairs and provides center level support of environmental, health and safety (EHS).
How you will contribute
You will determine donor eligibility; to include, proper management of informed consent, AIDS education and confidential self-exclusion, medical history interview and acceptance requirements, physical examination, blood testing, and reviewing Serological Protein Electrophoresis (SPE).
You will evaluate donor reaction(s), which occurs at the facility as outlined in the SOPs. Follow applicable SOPs for medical emergencies including the development of Center Physician's standing orders and donor transport to emergency care facilities.
You will refer to the Center Physician or Medical Affairs (as applicable) when in need of providing unacceptable findings to donors, or guidance concerning medical or technical issues, including donor safety and eligibility.
You will support the Hepatitis B and Seasonal Flu vaccination programs for employees as applicable.
You will manage employee incidents and determine whether further evaluation is required by occupational health/ER. Refer to EHS guidance regarding employee incidents.
You can be a Pandemic Coordinator when authorized by EHS and support investigations associated with pandemic threats within the local community as indicated by EHS or Medical Affairs.

About the Role:
The purpose of the Registered Nurse Clinician is to provide medically prescribed infusion therapy to patients with acute and chronic conditions.
Schedule: PRN; Monday - Friday; 8:00 AM - 5:00 PM, Saturdays as needed.

Minimum Qualifications:
Experience/Education:
* Graduate from an accredited school of nursing.
* Active Registered Nurse license in the state(s) of practice.
* 1 year of nursing experience in a clinical specialty involving infusion therapy.
* BLS or obtain upon hire.
* Proficient at partner programs (i.e.: MS Outlook, Word, Excel).
* Ability to cope with ambiguous, changing environment while under pressure in a calm manner.
* Must be able to work as a team member and develop productive and cooperative working relationships with all members of the Palmetto Infusion team.
Essential Functions:
* Work collaboratively as a member of an interdisciplinary team in an ambulatory infusion clinic setting.
* Effectively utilize the nursing process to assess and care for patients.
* Provide comprehensive nursing care in a professional, ethical, knowledgeable, and compassionate manner.
* Provide disease and drug-specific education to patients and caregivers.
* Comply with HIPAA and ACHC regulations.
* Participates in on-call rotation as assigned for AIC nurses.
* Travel to outlying clinics as needed.
* Maintain knowledge of medical/nursing practices by participating in educational in-services.
* Accurately perform all nursing duties according to the state nurse practice act while adhering to company policies and procedures including, but not limited to:
* accessing and maintaining central lines and PIVS
* obtaining lab specimens
