* Respond in-person to all MCR requests within 90 minutes after receiving the referral from the CARES line.
* Complete a screening and assessment using the Illinois Medicaid Crisis Assessment Tool (IM-CAT).
* Responsible for immediate and short-term follow-up, transition, and linkage activities.
* Coordinate with the assigned Care Coordinator and participate on a short-term basis on the CFT for Pathways clients.
* Complete all required MCR Intake packet paperwork at the point of crisis.
* Ensure all required crisis intervention documentation is complete and accurate.
* Provide and arrange for reunification, family generated placement or psychiatric hospitalization at time of crisis, with a short-term crisis plan.
* Provide Linkage and Referral services specific to client and family needs during time of crisis.
* Adhere to MCR schedule, by ensuring that answering service can contact worker immediately via cell phone and home phone as a secondary means of contact.
* Provide required documentation (i.e. mileage, time sheets, etc.) as indicated by agency protocols.
* Participate in individual, group supervision, and program staff meetings as scheduled to address client issues and revised documentation, placement options, professional development, and crisis schedules.
* Assist in coordinating and facilitating regularly scheduled community outreach meetings.

ESSENTIAL DUTIES AND RESPONSIBILITIES
Serve as a responder for Designated Service Area (DSA) #25.
Must be immediately available during scheduled shift to prevent, respond to, de-escalate, and mitigate crisis situations. Includes responding to crisis referrals for youth under the SASS Program, Medicaid adults, MCO members, and Pathways clients.
Responsible for receiving all crisis referrals from the CARES line on a no-decline basis within 30 minutes of the referral.
Respond in-person to all MCR requests within 90 minutes after receiving the referral from the CARES line.
Complete a screening and assessment using the Illinois Medicaid Crisis Assessment Tool (IM-CAT).
Responsible for immediate and short-term follow-up, transition, and linkage activities.
Coordinate with the assigned Care Coordinator and participate on a short-term basis on the CFT for Pathways clients.
Complete all required MCR Intake packet paperwork at the point of crisis.
Ensure all required crisis intervention documentation is complete and accurate.
Provide and arrange for reunification, family generated placement or psychiatric hospitalization at time of crisis, with a short-term crisis plan.
Provide Linkage and Referral services specific to client and family needs during time of crisis.
Adhere to MCR schedule, by ensuring that answering service can contact worker immediately via cell phone and home phone as a secondary means of contact

Provide required documentation (i.e. mileage, time sheets, etc.) as indicated by agency protocols.
Participate in individual, group supervision, and program staff meetings as scheduled to address client issues and revised documentation, placement options, professional development, and crisis schedules.
Assist in coordinating and facilitating regularly scheduled community outreach meetings.
Comply with the State of Illinois-mandated reporting law.
Complete all required Federal, HFS, and Agency Core Curriculum trainings, meetings, and workshops as required.
Provide services to program clients during on-call shift as deemed necessary by the supervisor.
