Essential Duties and Responsibilities Conduct daily well-being checks and support medical intake processes for new clients under clinical supervision. Assist with scheduling and coordinating medical and behavioral health appointments, transportation, diagnostics, and follow-up visits. Support clients in medication self-administration and monitor adherence to care plans. Document all health-related interactions, medication logs, and care coordination in the EHR (Athena); ensure accuracy and timeliness. Provide basic health education to clients, reinforcing medication adherence, hygiene, and preventive care practices. Communicate changes in client condition or care plan adherence to the Medical Coordinator or clinical supervisor promptly. Participate in daily huddles and weekly interdisciplinary team (IDT) meetings to support integrated client care. Maintain a safe and clean environment; assist with facility upkeep and infection control. Collaborate with case managers, behavioral health staff, and housing teams to support holistic client recovery. Ensure compliance with HIPAA, OSHA, and WPCC policies and procedures. Support tracking of program metrics and client health outcomes as directed by the Medical Coordinator or Director.
