Provide education to clients to promote improved functioning and skills development. Coordinate transportation for clients to appointments which connect them to community resources and services. Collaborate with other members of the care team involved in the client's case. Collaborate with community providers, including law enforcement and hospital staff to establish appropriate community response to identified individuals. Educate clients and/or families and significant others on resources to assist with identified needs. Maintain HIPPA compliance and client confidentiality. Complete initial and/or annual and other mandatory trainings as assigned related to crisis situations.
