Essential Duties and Responsibilities Conduct daily well-being checks and support medical intake processes for new clients under clinical supervision. Assist with scheduling and coordinating medical and behavioral health appointments, transportation, diagnostics, and follow-up visits. Support clients in medication self-administration and monitor adherence to care plans. Document all health-related interactions, medication logs, and care coordination in the EHR (Athena); ensure accuracy and timeliness. Provide basic health education to clients, reinforcing medication adherence, hygiene, and preventive care practices. Communicate changes in client condition or care plan adherence to the Medical Coordinator or clinical supervisor promptly.
