The Advanced Practice Clinician in the Jump Start Assessment (JSA) program is a physician-led and advanced practice clinician-driven program that is intended to care for and support Alignment Healthcare patients with complex and chronic care needs. Works within an interdisciplinary team environment that includes physicians, nurse practitioners, physician assistants, case managers, and other health care team members to ensure proper delivery of clinical and home-based patient care. Assess', develops, and coordinates options and services promoting quality care in order to achieve optimal health care outcomes while also ensuring cost-effective care complying with Alignment policy and all state and federal regulations. Our members are typically aged 65 and over and home and virtual visits will be between 45 and 60 minutes depending on whether it is an annual or initial visit.

Responsibilities:
1. Conduct in-home assessments 3 days a week (with some outpatient clinical work) on health plan members and work from home 2 days a week.
2. In-Home Assessments will include obtaining comprehensive history, physical exam, medication review, and appropriate cognitive/depression/safety screenings.
3. Comfort with End-of-life care discussions imperative.
4. Laboratory specimen collection in the home setting when appropriate.
5. Identify diagnoses to be used in active medical management and care management of patients with focus on chronic disease management.
6. Communicate findings of the patient assessment to inform the PCP of potential gaps in care.
7. Provide patient/family/caretaker education, with an emphasis on close monitoring and follow up of patient needs.
8. Emphasis on knowledge of appropriate community resources for referral.
9. Comply with all HIPAA regulations and maintain security of protected health information (PHI).

What You'll Do:
Provide frontline care to patients experiencing homelessness, behavioral health challenges, serious mental illness, substance use disorders, and complex chronic conditions, including patients transitioning from incarceration.
Manage diverse patient panels by delivering acute, chronic and preventative care directly in the field,
Conduct patient assessment, gather histories, perform physical exams, order and interpret diagnostics, and prescribe treatment under physician supervision.
Report to the supervising physician, working closely to address atypical and complex patient cases, while exercising independent judgement within state regulations and scope of practice.
Design and implement care plans that support patients as they work toward stability and improved health outcomes.
Collaborate daily with multidisciplinary team - physician, nurses, social workers, outreach staff to prioritize and coordinate care.
Document all encounters to maintain accurate, up-to-date records for continuity of care.

Clinicians are a critical part of our care team at Ophelia. As an Advanced Practice Practitioner, you will provide clinical care to patients receiving medication for opioid use disorder and a narrow suite of mental health conditions (depression, anxiety, insomnia, etc…).
You'll work closely alongside other clinicians, triage nurses, and administrative care coordinators to deliver compassionate, evidence-based treatment to patients with OUD.
Our Clinicians offer compassionate, non-stigmatizing support for patients, act as a liaison and advocate for them, and ensure patients receive appropriate care as their needs evolve. Depending on role type, clinicians conduct Intake visits and/or follow-up visits, and work within a team-based approach model of care to deliver high quality care to our patients. We have multiple different clinical role types depending on your experience with MOUD, and through our team-based model of care we provide a strong support system to ensure that you and our patients feel comfortable and supported.
Our patient care hours are Monday - Friday 9-7pm and Saturday and Sunday 9-5pm, with our weekend coverage provided only by our most experienced providers! We will work with you to determine your ideal schedule within these hours.

In this role you will be responsible for:

Adena Health is an independent, not-for-profit and locally governed health organization that has been “called to serve our communities” for more than 125 years. With hospitals in Chillicothe, Greenfield, Washington Court House, and Waverly, Adena serves more than 400,000 residents in south central and southern Ohio through its network of more than 40 locations, composed of 4,500 employees - including more than 200 physician partners and 150 advanced practice provider partners - regional health centers, emergency and urgent care, and primary and specialty care practices.
