Summary:

The Role
The Complex Care Navigator LPN plays a key role in delivering coordinated, compassionate care for members with serious illnesses. Working closely with nurse practitioners and registered nurses, the LPN administers clinical assessments and screenings throughout the care journey. They prioritize member needs based on assessment results and collaborate with the interdisciplinary team to develop and execute individualized care plans. The LPN ensures seamless communication, participates in care rounds, and leverages Tuesday Health's electronic tools to optimize the member experience. Trust-building, empathy, and consistent member engagement are essential to the role, enabling a truly person-centered approach to care.
Key responsibilities of this role may include:
Administer multiple assessments and screening tools within the clinical model at different times throughout the care timeline with the support of nurse practitioners and registered nurses
Prioritize needs based on assessment results and task interdisciplinary care team
Be accountable for care plan development within the multidisciplinary care team
Participate in internal and joint rounds to ensure the member experience is optimal and coordinated
Learn the Tuesday Health electronic systems, tools, technology, to deliver a coordinated approach to ensure the members have a unique experience
Build a strong, trusting relationship with every member, where listening and empathy are the foundation of every interaction
Serve as the quarterback of the multidisciplinary team transforming care for members with serious illness
A strong candidate will demonstrate the following:
Active and unrestricted licensed practical nurse license in the State of Ohio without any board action
Experience in clinical/medical setting preferred
Experience in a multi-disciplinary care model working across network providers, health plan care management support, and employed clinicians
Strong communication and organization skills
Ability to multitask and work under pressure without sacrificing a positive member experience
Ability to know when to escalate and potentially act professionally in an emergent situation

Responsibilities
The Registered Nurse Home Health provides intermittent skilled nursing services in a face-to-face capacity to include patient residence and qualifying facilities. The Home Health Visiting Nurse is responsible to communicate the patient's progress with other disciplines and directs, supervises and instructs non-professional home health aide staff in the provision of personal care to the patient.
Registered Nurse (RN)/Home Health Nurse/ Clinical Manager: Essential Duties & Responsibilities:
Under the physician's order, admits patients eligible for home care services within 24-48 hours
Assess and evaluates patient needs/problems, identifies mutually agreed upon goals with patients
Reports patient status and need for other disciplines to agency Clinical Supervisor and referring physician
Reports to assigned follow-up Clinician as indicated
Develops patient care plan that specifically addresses identified patient problems, patient problems and goals
Updates care plans on an ongoing basis; revises and resolves patient problems and goals as changes occur and/or at recertification
Completed admission paperwork and patient care plan submitted to Clinical Supervisor per agency policy following the admission including completed and signed admission checklist
Provides intermittent Skilled Nursing services including assessment, evaluation, procedures, teaching and training activities as outlined in the patient Plan of Care
Provides Skilled Nursing visits according to visit schedule and notifies agency of need to alter schedule in any way
Reports significant findings to patient's physician and Clinical Supervisor as they occur
Submits completed skilled nursing visit notes; communication notes and home health aide supervisory notes per agency policy on designated days as requested by Clinical Supervisor
Submits change orders per agency policy
Performs all OASIS time point assessment per Medicare Criteria and submits recertification paperwork per agency policy and procedure
Maintains open lines of communications to all members of the continuum of care team
Supervises Home Health Aide and license and documents per Medicare criteria and per agency policy and procedure
Acts as a preceptor in the orientation of new nursing staff as requested
Attends staff meetings, team conferences and educational in-services per agency requirements
Participates in Process Improvement (PI) program by assisting with collection of data and serves on PI team upon request
Participates in discharge planning process Medicare Criteria and agency policy and procedure
Follows agency policies and procedures
Performs these and all other duties as assigned by the Administrator

POSITION SUMMARY. The Registered Nurse is responsible for administering and supervising medication administration and participation in the plan of care for clients of the Psychiatric Residential Treatment Facility (PRTF) & MBA program. Provide physical health and nurse assessments of clients, and document significant findings in the client's record.

Participates and assists in nursing activities specific to client admissions and ongoing treatment including but not limited to medical case management, physical health screens and nursing assessments
Teach clients and staff about health care topics and medications.
Utilizes the time log system to document billable hours of direct care.
Orient new staff and trains them on policies and procedures.

Assist and train Monarch PRTF staff to make appropriate decisions pertaining to the health care needs of clients.

Return and credit medications when indicated.
Call for lab results when indicated and follows up on outside medical appointments. Contact the physician with abnormal lab results and with emergencies.
Assists with daily operations of the ASD, PRTF & MBA units.
Participates in Quality Improvement as indicated.
Contribute to the development and maintenance of the clinical record through the timely completion of assigned documentation in accordance with applicable licensing and accreditation regulations and standards.

Attend scheduled staff meetings, supervision, and on-going training.
