This nursing role provides medical oversight of people we support through direct observation and consultation. With the goal of assisting persons supported to achieve their highest level of health and independence. The nursing role focuses primarily on interventions that maximize the psychosocial, physical, affective, cognitive, and developmental strengths of the persons, families, staff, significant others, and those who deliver services. The Registered Nurse provides nursing leadership and serves as an advocate to ensure individuals receive appropriate healthcare treatment, health education, and other health related services which honor the rights of the persons supported. Essential Duties: total equals 100% -- less than 10% not documented Person Centered Care/Consultation/Assessment- 40% · Consult on all clinical needs for the people we serve including difficult or complex medical/behavioral cases. · Provide in-person care as determined by the individual's needs and program/state requirements. · Obtains an initial health history and performs initial physical examination on admission per state requirements. · The ability to effectively triage medical concerns via phone and instruct non-licensed staff to provide appropriate care. · Provides 24-hour on-call support in rotation with nursing team to residential staff to address individual medical needs and emergencies. · Identifies health care needs for the person based on the data collected through the nursing process and develops a plan with goals and interventions specific to the needs of the person. · Implements the plan to promote, maintain, or restore wellness, prevent illness, and/or provide appropriate end-of-life care. · Evaluates response of the person to the interventions/supports provided and revises the plan accordingly. · Provides nursing care according to the physician's orders, in compliance with recognized nursing standards and State and Federal Regulations. Documentation- 20% · Review compliance of physician orders including, but not limited to treatments, procedures, labs, appointments, specialized diets, medications, outcomes, and adaptive equipment for each person we serve. · Collects health and functional status data for persons with intellectual and/or developmental disabilities systematically and continuously. The data is documented, communicated, and made readily accessible. · Participates in quality assurance data collection and reporting as indicated by program requirements. · The ability to prioritize and manage tasks successfully to meet the needs of the program per state regulations. Collaboration- 15% · Maintains open and consistent communication with individuals, families/guardians, other health-care providers, and regulatory oversight agencies. · Participates and prepares for on-site survey and licensing audits. · Collaborates as part of the interdisciplinary team in developing, implementing, and evaluating the individual's plan of care. · Participation and review of general event reporting and interdisciplinary team follow-up. · Oversee the implementation of health care policies and procedures and recommend revisions as needed in collaboration with Regional Director of Nursing and National Director of Nursing. Job Summary: Provides direct nursing care to our patients, assists medical providers and integrated pod team members to optimize the patient care experience, works with and on the care coordination team, and is responsible for triage across clinic sites. Functions as a nurse leader when assigned to a pod team. Essential Functions: • Utilize professional nursing clinical skills and assessment to provide primary care nursing. • Participate as member of health care team planning for and providing optimal patient care including establishing nursing care plans when needed. • Manage and lead daily pod patient flow, including nurse visits, medical assistant visit, co-visits and walk-ins. • Assist and facilitate patients in obtaining medical services, including scheduling patients as appropriate, triaging patients, and assessing patients who walk-in to the clinic seeking services. • Provide patient triage and assessment and obtain health information and relevant data. • Consistently follows telephone triage guidelines, including documentation and notifying provider in a timely manner. • Transition care for patients discharged from the hospital within 24-48 hours to prevent readmission and related complications. • Evaluation of appropriate follow-up care for patients seen in the emergency department. • According to departmental protocol, provide medications and conduct medication reconciliation. • Provide a safe, comfortable, therapeutic environment for patients and families. • Identify changes in clinical signs and symptoms in life threatening situations and initiate appropriate measures. • Evaluate outcome of patient care, consult with other professionals as required, and adjust nursing care processes as necessary to ensure optimal patient care. • Assist with OSHA (Occupational Safety and Health Administration), CLIA, VFC (Vaccines for Children) and the Cancer Screening Programs and other organization or site quality processes. • Provide co-visits in collaboration with providers and medical assistants.
