As a member of the kidney disease health care team, this position participates in decision-making, teaching, leadership functions, and quality improvement activities that enhance patient care outcomes and program operations. PRINCIPAL DUTIES AND RESPONSIBILITIES: All duties and responsibilities are expected to be performed in accordance with Fresenius Kidney Care, policy, procedures, standards of nursing practice, applicable contractual service agreements, state and federal regulations. Performs all essential functions under the direction of the Supervisor and with guidance from the Educator, Preceptor or in collaboration with another Registered Nurse. What you will do Assists clients in eliminating barriers to timely, quality care through all phases of health care including prevention, detection, diagnosis, treatment, and survivorship with particular focus on disparate populations. Completes needs assessment of all clients and documents within database and electronic medical record as appropriate and effectively provides oncology nurse triage based on AAACN model of the nursing process including assessment, analyzing and planning, implementation and evaluation. Makes referrals to appropriate resources and services and documents within database and electronic medical record as appropriate. Provides ongoing support and follow-up per outlined protocols and documents within database and electronic medical record as appropriate. Provides education and educational resources to those affected by cancer, including assisting in clients making informed decisions, with documentation in database as well as in electronic medical record as appropriate. Appropriately refers mental health and emotional support needs to social work navigator or back to community or health system mental health professional. Assists leader in planning, development, implementation and evaluation of goals and objectives. Assists leader with data collection, analysis and reporting. Achieves and maintains a current knowledge base regarding local, regional and national resources while ensuring electronic database of resources remains up to date including working collaboratively with the American Cancer Society and other organizations that provide support to cancer patients. Works collaboratively with all Avera Cancer Institute staff and providers as well as other staff and providers throughout the Avera system to provide coordinated services which facilitate continuity of care for the patient and their families.
