**Description**
The Manager, Care Management (CM) is responsible for direct oversight of the daily operations of the Care Management department as it relates to Discharge Planning. Ensures departmental day to day operations are successful by creating schedules, arranging coverage, and dividing work as necessary in order to ensure work is completed in a timely manner which will ensure compliance with regulatory requirements. Primary duties and responsibilities: Leadership and Management: 1. Responsible for maintaining and managing processes as it relates to day-to-day workflow. 2. Responsible for coaching, developing, training, and the performance management process for employees. 3. Ensures that department, division, and system strategic goals are connected to the daily work of the department. 4. Understands metrics such as LOS, avoidable days, readmissions, saved days and will be able to implement measures in order to achieve targeted goals Collaboration and Communication: 1. Thorough ability to effectively communicate and organize workflow, must quickly and effectively break down barriers to discharge. 2. Works with Care Management Leadership and Physician Advisors across the system and will collaborate with the interdisciplinary teams as well as the revenue cycle teams as needed. 3. Displays a broad understanding of complex discharge planning and strength in problem solving and critical thinking. Is able to successfully complete all roles within the department at any given time. Compliance and Program Development: 1. Collaborates with Care Management Leadership in order to successfully implement new policies and procedures. 2. Oversees departmental compliance by ensuring that the team adheres to established policies and procedures. 3. Ensures that the Care Management systems and tools are being utilized as designed including staying up to date with system updates and upgrades. 4. Monitors quality and productivity standards in order to manage individual performance as well as the team's overall performance. Travel: Less than 10% of the time may be required. Work Type: Hybrid employee - splits time between working remotely and working in the office.
Minimum Qualifications: Education - If a RN, bachelor's degree required. Master's degree preferred. Must have a valid, active unencumbered Nursing license or temporary permit approved by the Georgia Licensing Board. Experience - 7 years minimum experience with at least 5 years in a management position Licensure - LMSW/LCSW will be accepted in lieu of RN requirements. ACM or CCM certification preferred within a year of hire or eligibility Certification - 3 years of experience in Case Management (CM) required. 3 years healthcare experience required. Knowledge, skills, and ability requirements: - Leadership experience preferred. Proven ability to coach, mentor, and lead. - Demonstrates leadership abilities.

What You Will Do
Provides clinical supervision to staff including assigning, monitoring and evaluating cases for clinical team(s). Conducts regularly scheduled team meetings. Provides back-up coverage for program leadership as required.
Manages triage and case assignment procedures, new referrals, liaison activities, and staff scheduling to insure adequate coverage at all times.
Collaborates with other team members and Behavioral Health Services (BHS) leadership in formulating clinical and administrative policies and procedures, preparing policy and procedure manuals, implementing and maintaining established policies and procedures, and proposing modifications and revisions of policies and procedures, as indicated.
Collects, tracks, and monitors progress and outcomes for all staff assigned to the team(s); produces and maintains detailed reports for all data pertinent to the program. Reports relevant data to funders and central administration as needed.
• Oversees the maintenance of updated case records for team(s) through EMR and coordinates effective electronic communication throughout all provider databases, as needed. Maintains case records in accordance with program policies/procedures, as well as VNS Health, city, and state standards and regulatory requirements.
Monitors the program budget and is knowledgeable of all financial aspects of the program, including, but not limited to, reimbursement and purchasing.
Ensures volume and productivity meet program standards and operations.
Oversees compliance of quality and performance indicators, and supervises staff to achieve goals. Performs internal audits to ensure compliance with policies and procedures and takes corrective action, as necessary to address deficiencies.
Provides clinical subject matter expertise and serves as a resource to supervisors, clinicians and staff. • Provides assessment, direct services to program participants and families in the community; advises and consults in case conferences, staff meetings, and discharge planning as needed.
Promotes positive relationships within VNS Health and other community service organizations. Serves as program liaison to other community agencies, negotiating formal liaison and organizing consultation and education for referral sources.
Participates in 24/7 on-call coverage schedule and performs on-call duties, as required.
Investigates complaints registered by program participants, completes Incident Reports and other safety and quality reports within required time frames.
Collaborates with program leadership and other staff in the development and implementation of in-service education programs.
Performs all duties inherent in a supervisory role. Ensures effective staff training, interviews candidates for employment, evaluates staff performance and recommends hiring, promotions, salary actions, and terminations, as appropriate.
Oversees the development of systems and records for billing each MCO.
Qualifications
Licenses and Certifications:
Valid New York State driver's license may be based on program needs required License and current registration to practice as a Nurse, Social Worker, Psychologist, Marriage and Family Therapist, Mental Health Counselor or other related license in the State of New York required
For IMT: LCSW required or LMHC
Education:
Master's Degree degree in Social Work, Psychology, Marriage and Family Therapy, Mental Health Counseling, Nursing or other related field required
Work Experience:
Minimum of five years of supervisory and administrative experience with demonstrated competency in program management, budget management, and community relations required
Strong interpersonal and leadership skills required.
Knowledge of Microsoft applications required
For Adult Services: Prior experience working in a community behavioral health care setting required Experience with EMR systems preferred Knowledge of city and state agency and/or managed care functioning preferred

About the Role:
IPSY is looking for a Creative Project Manager to help our Marketing team bring ideas to life. In this role, you'll be the organizational heartbeat behind email campaigns, social media content, print materials, and web deliverables-keeping timelines on track and workflows running smoothly. You'll work closely with creatives and cross-functional partners to ensure every project moves efficiently from concept to completion, without losing momentum or creativity along the way.
This position reports to the Senior Manager of Brand Operations and can be fully remote from any of the 16 U.S. states where IPSY has an established business presence (see application for a list).
What You'll Be Doing:
Lead creative project workflow from concept to launch, ensuring deliverables are on track, on time, and aligned with the brief.
Own creative project timelines in Asana (project management tool) and update them as projects evolve for accuracy through completion.
Assess and optimize designer, copywriter, and photographer team bandwidth.
Schedule and develop agenda for project-related meetings.
Take notes during creative meetings and task out to cross-functional teams accordingly.
Coordinate monthly photoshoot timeline management from kickoff, shoot date, retouching dates, approvals, and asset management.
Be the point person for all conversations, meetings, brainstorms and schedules for the projects you manage.
What We Are Looking For:
5+ years of experience in project management at an agency or within corporate creative/marketing.

+ Must have 8 years of Information Technology Project Leadership experience, with at least 5 years in a PMO like role OR an equivalent combination of education, training, and experience.
+ Must be able to manage large scale programs and projects across the full project lifecycle.
+ Previous experience with business transformations and transitions.
+ Proven experience mentoring Project Managers.
+ Experience working in a large healthcare organization and ability to work across diverse organizations and build effective relationships.
+ Preferred certification in Prince, MSP, or PMP
