We're looking for a bold, compassionate Family Nurse Practitioner to join our Metro Detroit team and help deliver meaningful, patient-first care in a DPC environment. What You'll Do Clinical Care & Patient Relationships Serve as a primary care provider for a dedicated patient panel, building long-term, trusted relationships. Examine patients, obtain comprehensive health histories, and assess physical and behavioral health needs. Diagnose and manage acute and chronic conditions with a focus on prevention and continuity of care. Prescribe medications and therapies in collaboration with supervising physicians, as required. Diagnostics & Procedures Order, perform, and interpret diagnostic testing (labs, EKGs, imaging coordination). Perform in-office procedures including injections, suturing, wound care, and immunizations. Provide proactive follow-up and care coordination to reduce unnecessary ER visits and specialist referrals. Holistic & Preventive Care Counsel patients on lifestyle, nutrition, chronic disease management, growth and development, family planning, and preventive health. Deliver culturally competent care to a diverse patient population, including Spanish-speaking members. Partner with patients to create personalized, goal-driven care plans. Team Collaboration Work closely with physicians, medical assistants, and care team members to ensure seamless care delivery. Support clinic operations by supervising and coordinating support staff when needed.
