Summary
As an Associate Supervisor, you will have the opportunity to provide educational and mentoring support to frontline colleagues (direct reports), which allows them to cater to the well-being of children and families within the Child Welfare System. The successful candidate will also connect to these same families and play a role in the team's goals and outcomes. This role is part of the Children Permanency Partnership's (CPP) Foster Care program, which includes a consortium of agencies. The Associate Supervisor helps with increasing stability, safety, and well-being using trauma-informed mental ( delete the word mental) practice. The Associate Supervisor will provide direct supervision over an average of 60 clients at any given time. (Please replace this sentence with "The Associate Supervisor is responsible for ensuring quality case management services are provided to 60-100 clients at any given time.")

Primary Responsibilities:
* Provide direct supervisor to a team of 4 - 6 colleagues with a focus on moving clients towards permanency.
* Interact with youth and families as a direct line of communication as well as act as a liaison for staff with all other professionals in the field.
* Attend all required meetings, court hearings and mandatory trainings and work groups.
* Track performance measures/outcomes as dictated by contract as well as agency.
* Participate in on call rotation with other supervisors providing the opportunity to support peers during afterhours, weekends, holidays, in the form of in person and phone contacts.
* Provide other supports as indicated by contract or agency requests as they pertain to best practice around Foster Care.
* Perform other duties as assigned.
Qualifications:
* Minimum of a BA/BS degree in Social Work or a comparable human service field.
* MA degree in Counseling, Social Work or a relevant helping profession preferred as due to MO State Regulations, this position cannot be promoted to Lead without a MA.
* Must have at least 2 years (4 - 6 years preferred) of providing case management services.

Key Responsibilities
Prepare patients for provider visits by completing the Advanced Standard room process:
Enter chief complaint, take vital signs, review history and allergies.
Complete thorough medication reconciliation.
Address open care gaps (fall risk, depression screening, cancer screenings, diabetic eye exams).
Recheck elevated blood pressures and document preventative visit details in Epic.
Assist with in-office testing (blood draws, mental status exams, EKGs, oxygen studies, point-of-care testing).
Administer vaccines and injections within scope of practice.
Schedule follow-up appointments per office policy.
Collaborate with providers and clinical care team to monitor and coordinate care plans.
Qualifications
Licensed Practical Nurse (LPN)
Experience: 2 years' hospital or clinical experience.
Licensure: Graduate of an accredited practical nursing program. Consideration will be given for comparable education experience and successful licensure. Must obtain and maintain a current LPN license within the hiring state and meet all State Board of Nursing requirements.
Medical Assistant
Education: High School Diploma or GED.
Certification: Must obtain BLS certification within 90 days of hire.
Preferred Experience: 6 months of clinical healthcare experience (physician's office preferred).
Certified Medical Assistant / EMT / Paramedic
Education: High School Diploma or GED.
