The Patient Navigator/Case Manager provides a unique role in patient care. This role focuses on addressing health disparities and improving health outcomes for individuals transitioning from the justice system.
The Navigator works directly with jail inmates, spending time within the jail facility to build relationships and provide case management services. The role also includes maintaining an external office space for follow-up care coordination and patient support. This dual presence ensures seamless care transitions, resource connection, and empowerment for patients as they reintegrate into the community.
By reducing barriers to care and fostering patient independence, the Navigator ensures that patients are connected to necessary resources, supported through transitions, and empowered to navigate healthcare systems successfully.
Essential Functions
Assist patients transitioning from incarceration in accessing necessary services to establish a medical home and/or follow-up care.
Provide case management and navigation services within the jail facility, working directly with inmates to identify health and social needs.
Maintain an external office space to continue care coordination and support after patients are released from the justice system.
Work with multi-disciplinary teams and external partners to identify patients who can benefit from intensive case management services.
Provide tailored care coordination to address barriers and ensure timely treatment, focusing on populations targeted by the QTI-TJI grant.
Build relationships with patients, healthcare providers, and community organizations to foster trust and collaboration.
Assist patients in understanding and completing next steps in their treatment plans.
Facilitate patient enrollment in programs such as the sliding fee scale (SFS) program or Prescription Assistance Programs.
Support patients with dental referrals and follow-up processes as applicable.
Collaborate with healthcare teams to ensure care plans address social determinants of health (SDoH) and patient-specific needs.
Maintain comprehensive patient records, including profiles, referrals, appointments, and outcomes, in the electronic health records (EHR) system.
Provide health education and coaching to empower patients to advocate for their own care and navigate systems independently.
Serve as a reliable resource for patients by staying informed about program updates, community resources, and healthcare changes.
Identify and address patient barriers creatively, seeking innovative solutions aligned with grant goals.
Coordinate care transitions and communicate with patients and providers to reduce gaps in care.
Promote patient health and comfort by addressing both physical and emotional needs.
Collaborate with healthcare teams to identify opportunities to improve care processes and grant-funded program effectiveness.

Qualifications
Education & Experience:
Bachelor's or Master's degree in Social Work or Health and Human Services
Previous experience providing outreach, referrals, and/or social services to diverse or high-risk populations
Experience working in a Federally Qualified Health Center (FQHC)
Comfortable and skilled with electronic medical records (EHR) systems
Skills & Competencies:

About this Position:
Our Registered Respiratory Therapists administer all therapeutic, diagnostic, and mechanical modalities under the purview of Respiratory Care Services, for which the Therapist has demonstrated appropriate proficiency, and has been documented as competent to perform these procedures. In order to qualify for a Registered Respiratory Therapist Critical Care position, you must have at least 2 years of critical care experience in addition to an Adult Critical Care Specialist (ACCS) or Neonatal/Pediatric Specialist (NPS) certification as appropriate to population served/specialty.

Our program structure:
Our externships are flexible-they can range from two to eight weeks during the year, according to your college curriculum - but are typically two or three weeks in length. Your schedule will include days, evenings and weekends.
Our externship rotations are designed to allow students, under veterinary supervision, to practice the problem-solving, technical and communication skills that they have acquired during their clinical preparation. Externs are given the opportunity to work alongside our veterinary team and are encouraged to participate in all aspects of patient care. A stipend of $250 per week will be provided.
What you'll be doing:
Externs experience the typical caseload of a small animal practice. Under the supervision of your mentor doctor you will gather histories, perform physical examinations, develop diagnostic and therapeutic plans, scrub in to surgery, assist in the management of medical cases, provide wellness care, develop your client communication skills, polish your technical skills and produce appropriate medical records (SOAPs) within the time limits of daily practice.
Who we're looking for:
Extern students are expected to be enthusiastic and effective participants in our patient care activities, upholding the professional and ethical standards of our practice. Students participating in the externship program are suggested to review the Veterinary Practice Act in regard to the scope of duties that may be performed, and to be familiar with the type and degree of veterinary supervision that is required. These should be followed carefully in order to protect both yourself and our practice.
As a veterinary student extern, we expect you to dress and behave in a professional manner and to meet the expectations that we have of all team members as outlined in our employee handbook.

Qualifications
Minimum Requirements:

Valid Florida Driver's License
Preferred Experience:
Experience working in a Federally Qualified Health Center (FQHC)
Familiarity with Electronic Medical Records (EMR)

We are actively seeking a dedicated, flexible, and experienced Medical Assistant (Float) to join our compassionate care team. This unique float role supports multiple clinic locations and is ideal for a skilled MA who thrives in a fast-paced, patient-first environment.
As a Medical Assistant, you will be responsible for direct patient care and clinical support, working closely with providers and the nursing team to deliver high-quality, coordinated services that improve outcomes in our community.
What You'll Do:
Work directly with providers, managing same-day appointments and supporting clinical care
Perform visit prep, patient assessments, and point-of-care testing (e.g., EKGs, Spirometry, Finger Sticks)
Manage lab specimen collection and requisitions
Assist with patient education, referrals, and discharge instructions
Maintain and update EMR records, allergies, prescriptions, and screenings
Collaborate across departments to deliver seamless, integrated care
Travel to multiple clinic locations based on staffing needs
Support weekend, evening, and urgent care shifts as assigned
Participate in clinic training, CPR codes, and safety initiatives
Minimum Requirements:
Graduate of an accredited Medical Assistant Program
Current Florida Medical Assistant certification
Current BLS Certification
Preferred Experience:
At least 1 year of prior MA experience
Proven success in customer service or patient-facing roles
Strong references from former/current supervisors
Volunteer experience or additional healthcare certifications a plus
