Clinical Supervisor- Hospice
Supervises the program to ensure that a high quality, cost-effective service is delivered by the department and that is in keeping with Hartford HealthCare at Home's strategic plan as well as the Program's goals and objectives. Provides direct patient care that is specific to each patient's needs, including those needs that are age specific.
In general, most of the time will be spent in the following activities:
Provides staff guidance in making clinical and case management decisions focused on clinical excellence for end of life care and symptom management. Directs staff in the Medicare Hospice Benefit guidelines
Assures the development of personnel qualifications and policies
Identifies and facilitates professional development needs and competency of staff through home visits with staff to supervise and evaluate clinical performance; identifies and follows through with development plan
Maintains compliance with regulatory agency policies and procedures by providing staff guidance in making clinical and case management decisions that are cost effective and focused on clinical outcomes
Accountable for team performance in achieving desired clinical and operational performance measures
Utilizes clinical software to identify opportunities for education, most appropriate utilization of services, oversee clinical progression of patients and trend team outcomes and performance.
Assures that clinical and functional outcomes are achieved at the highest level possible
Resolves patient care problems collaboratively with internal/external customers - collaborates with peers across the regions
Oversees the daily patient assignments for clinical staff based on client needs and staff competency to maintain team productivity standards; LEAN Daily Management process (Visual Boards)
Coordinates referrals and ensures timeliness of SOCs and timeliness of adjunct service delivery
Ensures that patient needs are continually assessed
Ensures that an individualized plan of care is developed, implemented and updated as needed for all patients.
Ensures coordination of patient care, including communication with all physicians involved in the plan of care
Determines staffing needs and coordinates hiring to meet service demands for assigned geographic areas
Hires, trains, coaches and evaluates staff. This role is also responsible for completion of personnel evaluations according to agency timelines.
Reviews and analyzes team and patient metrics to consistent with agency initiatives.
Participates in preceptor programs
Anticipates customer needs and responds quickly, accurately and pleasantly
Participates in preceptor programs and student education regarding hospice and palliative care end of life issues
Qualifications
Licensure: A registered nurse with a license to practice in the State of Connecticut

Experience: According to current state licensure regulations. Hospice experience with OASIS and case management certification preferred.

Small free-standing Psychiatric Hospital with unique, tight knit units that work collaboratively to provide care to Eastern Connecticut communities.
Leading provider for children, adolescents and adults with mental illness and substance use addictions.
Two units: Pediatric/Adolescents (26 beds) and Adults (33 beds)
Three shifts available - first, second, third, that are 8 hour shifts.
Set schedules: 8 hour to 32 hours per week as well as flexible per diem positions.
The Registered Nurse utilizes psychiatric/chemical dependency nursing skills as a direct client care provider and is an integral member of the multidisciplinary team. The RN works under the direction of the Nurse Manager and/or the RN Designee and is responsible for the delivery of direct client care to clients individually, in groups and in the milieu. The RN is a role model for other staff members and is a leader in teaching other staff, in program development, in guiding group decision making, in developing and implementing treatment plans and improving the quality of care.
Qualifications
Psychiatric experience preferred.
Graduate of an accredited School of Nursing, Bachelor's degree preferred.
Current Connecticut RN License
Excellent communication and critical thinking skills.
Effective leadership and delegation skills.
Experience with leading behavioral interventions or managing violent behaviors.

The Maintenance Technician 1 versees the day to day engineering operations in the lodge.This includes but is not limited to project management, general life safety, ordering, working with vendors. Responsible for completing preventative maintenance and repairs through-out the hotel guestrooms, public spaces, equipment, buildings & grounds.
Responsibilities:
Perform preventive maintenance on tools and kitchen and mechanical room equipment, including cleaning and lubrication.
Identify, locate, and operate all shut-off valves for equipment and all utility shut-offs for buildings.
Test, troubleshoot and perform basic repair on all types of equipment, plumbing (e.g., plunge toilets and unclog drains), electrical components including lamps, cosmetic items, extension cords, vacuum cleaners, internet devices, replace electrical switches and outlets, and other guestroom items.
Assure each day's activities and problems that occur are communicated to the other shifts using approved communication programs and standards.
Ensures that all work performed is conducted in a safe and timely manner adhering to standards of safety and effective equipment utilization.
Maintains working knowledge of departmental safety procedures. Attends safety-training programs and in-service as required for continued development.
Maintains the necessary service records on mechanical and structural systems.
Ensure all standard operating procedures are followed for park operation, employee relations, timekeeping and safety. Including ensuring all daily, weekly and monthly documentation is being completed.

Estimated Salary Range:
