    Under the supervision of the Director of Admissions, the Coordinator of Admissions is responsible for managing the planning, scheduling, and coordination of client admissions and discharges for the facility's Substance Use Disorder (SUD) and Mental Health (MH) treatment programs. The coordinator also serves as an Admissions Counselor, conducting pre-admission screenings, scheduling intake appointments, assessing client appropriateness and eligibility, and completing all required admission documentation. Placement is determined based on Eva's Village admission and exclusion criteria, as well as the level of care guidelines established by the New Jersey Substance Abuse Monitoring System (NJSAMS). Part 2 and HIPAA standards. · Must be able to work independently and complete assignments with minimal supervision. · Good interpersonal and customer service skills required. · Must maintain a professional demeanor and perform duties effectively in stressful/emergency. · Must be able to maintain objectivity and deal diplomatically with all callers to the Admissions Department · Maintains statistical data for the Admissions Department, including information on client referral sources and local, state, and federal funding sources. · Assists as directed in the development and implementation of outreach efforts with public and private programs and social services agencies. · Participates in administrative and clinical staff meetings within the agency. · Maintains effective interdepartmental communications. · Performs other duties, assignments, and responsibilities as deemed necessary and/or identified by their direct supervisor. · Duties as the Admissions Counselor: · Conducts pre-admission telephone screening interviews with all prospective clients and/or identified referral sources seeking admission to Eva's Village Substance Use Disorder (SUD), Mental Health (MH), and/or Co-Occurring treatment programs. · Evaluates prospective clients during pre-admission telephone screenings for psychological, social, and physiological signs and symptoms of substance use disorders (SUD), Mental Health (MH) disorders, and/or Co-Occurring Disorders. · Determine individuals' appropriateness and eligibility for program admission and/or referral. · Identifies any coexisting conditions (e.g., medical, psychiatric, or physical) that may require additional professional assessments and/or supportive services. · Adhere to all applicable laws, regulations, and agency policies & procedures governing substance use disorder, mental health, and/or co-occurring treatment services. · Assess prospective clients using the Addiction Severity Index (ASI). · Complete and compile the following screening/assessment tools for the clients Electronic Health Record, as per the New Jersey Substance Abuse Monitoring System (NJSAM) o Level of Care Index (LOCI) o Diagnostic and Statistical Manual of Mental Health Disorders-Text Revision (DSM-5-TR) o Daise o Admission Module o Clinical Authorization. · Provide orientation to the prospective client on the agency's policies, business hours, fee schedules, service provisions, client rights, criteria for admission, treatment, and discharge. · Maintains up-to-date knowledge of current and available community resources. · Manages time effectively and prioritizes work demands appropriately. · Answer incoming phone calls and respond to emails in a timely and professional manner. · Handles client and customer inquiries via phone and email. · Conducts follow-up calls with clients and referral sources as needed. Qualifications & Experience · Bachelor's degree in counseling, social work, or other human services required. · Master's degree in counseling, social work, or other human services field preferred. · CADC required, LCADC preferred · 1-5 years' experience in substance use disorder and/or mental health treatment field · Knowledge of substance use disorder and mental health disorder treatment. · Knowledge of prevention, treatment, and recovery supports services and resources. · Knowledge of the bio-psychosocial model, DSM-5-TR Mental Health Disorders and Diagnosis · Knowledge of the American Society of Addiction Medicine (ASAM) criteria · Understanding confidentiality rules and regulations related to 42 C.F.R.     Under the supervision of the Senior Director of Integrated Services and/or Clinical Administrator, the Manager of Peer Recovery Support Services provides guidance, supervision, and operational oversight to Eva's Village Recovery Community Center (RCC) and Opioid Overdose Recovery Program (OORP). The Manager of Peer Recovery Support Services also functions as the Patient Navigator / Trainer within the OORP program. Job Duties 1. Embodies Eva's Village's core values, voice, and tone to provide an exceptional and effective experience for our clients, funders, employees, and volunteers. 2. Oversee, supervise, and monitor the daily operations of the RCC and OORP program, services, and staff. 3. Collaborates with the Senior and Associate Peer Services Coordinators to develop and coordinate activities and services within the Recovery Community Center, ensuring alignment with related programs, departments, and staff to maximize efficiency and maintain compliance with relevant policies, procedures, and standards. 4. Shall attend any training and/or meetings in person or via conference call that are scheduled by DMHAS to support the delivery, sustainability, and effectiveness of OORP services. 5. Maintains effective intra-office and inter-office communication within departments. 6. Shall ensure cooperation and collaboration with the DMHAS statewide evaluator for the OORP, which may include focus group participation; key informant interviews; and individual data reporting (i.e., select social indicators; substance use, overdose and mental health history; patient outreach; services provided, and recovery and treatment follow-up). 7. In collaboration with the Sr. Director of Integrated Services shall engage in communication regularly with the DMHAS Project Director as well as the State Opioid Treatment Authority pertaining to any identified issues and/or barriers in linking individuals to treatment. 8. Shall engage in the data collection, which is to be reviewed with the Sr. Director of Integrated Services before its submission to the DMHAS Project Director monthly (by the 1st of each month) in a format to be provided by DMHAS. Data shall consist of a minimum of the following: · Number of reported opioid overdose reversals by Emergency Department. · Basic demographics on the individual reversed from an opioid overdose, including a minimum of the following: Age, Race, Hispanic origin, Gender · Number of visits per month by the Recovery Specialists, Number of successful referrals to treatment by Level of Care (LOC) · Number and types of referrals to recovery supports other than substance abuse treatment (i.e. mental health services, housing, employment, etc.) · Number of follow-up phone calls by Recovery Specialists per month 9. Shall submit a Narrative Contract Progress Report to the Senior Director of Integrated Services and/or Clinical Administrator for review before submission to the DMHAS Project Director within 5 working days of the end of each quarter. 10. Maintain client information and ensure up-to-date documentation in the OORP Teams Channel. 11. Providing an evidence-based comprehensive case management assessment that includes life domains such as housing, finances, transportation, legal services, vocational, employment, health care, and family strengths/needs. 12. Communicating with the DMHAS Interim Managing Entity (IME) for substance use disorder assessment authorization and subsequent treatment, as needed. 13. Assist participants in connecting with vital community resources, including but not limited to self-help groups, vocational training, educational opportunities, and social support systems that align with their interests and recovery. 14. Empower participants to advocate for their own needs, ensuring their voices are heard and their goals drive the direction of their recovery services. 15. Providing a warm hand-off to appropriate treatment and recovery services, to include referrals to DHS licensed programs that offer Medication Assisted Treatment (MAT). Developing an integrated Recovery Management Plan (RPM) that is consumer-centered and includes strategies for recovery. The plan shall identify priorities, desired outcomes and the strategies and resources to be used in obtaining outcomes based on the case management assessment 16. Developing linkages and linking individuals to resources that address specialized needs, such as services related to HIV/AIDS, mental health disorders, chronic and acute health problems, pregnant and parenting women, veterans and problems stemming from involvement with the criminal justice system. 17. Advocating on behalf of the patient. 18. Training Hospital Emergency Department staff (when applicable) on the Opioid Overdose Recovery Program. 19. Shall assist the Recovery Specialists when necessary. 20. As the Patient Navigator shall also engage in, but not limited to, the following: a. Will be on-call at all times for the OORP program after the scheduled employment shift. b. Document the integrated case plan in the individual's case file, as well as document how the warm hand-off was conducted. 21. Performs other relevant duties as assigned by the Director of Integrated Services and/or Clinical Administrator. Qualifications 1. Must possess a master's degree in health, psychology, counseling, social work, education or other behavioral health profession. 2. Must possess a Certified Peer Recovery Specialist Certification. 3. The Telehealth Clinician will work closely with Eva's Village Integrated Service Team, including Clinical, Medical, and Residential staff as well as other relevant personnel to ensure comprehensive care coordination and appropriate service linkages for clients. JOB DUTIES Completes comprehensive screenings and assessments of clients if indicated and ensures treatment of clients with co-occurring disorders is provided by appropriately trained and qualified clinical staff (N.J.A.C.-10:161B-1.8(d)5., N.J.A.C.-13:34-10 2, and N.J.A.C.-13:34-10.3) Completes Clinical Assessments, consultations, and psychotherapeutic counseling services under the supervision of a State Licensed Clinical Social Worker (N.J.A.C. 13:44G-8.1(2) or other clinical supervisors deemed as appropriate by the Committee under N.J.A.C. 13:34C-6.2 and 6.4 or the supervision of a qualified supervisor as defined in N.J.A.C. 13:34-10.2) Completes the professional application of Social Work methods, Professional Counselor values in the assessment and psychotherapy counseling of individuals, families, or psychotherapy groups (N.J.A.C. 13:44G-1.1, N.J.A.C. 13:34-11.2, N.J.A.C. 13:34C-2.1).     Job Description Under the primary supervision of the Clinical Supervisor, and/or Clinical Director of Integrated Services, the Primary Counselor functions as part of the treatment team and provides comprehensive treatment services including substance use disorder counseling services, client assessment, screenings, case management, care coordination, individual counseling services, group counseling, psychoeducation classes, treatment planning, continuum of care referrals, and crisis intervention, etc., to clients struggling with substance use disorder. The Primary Counselor also participates in peer review, quality assurance activities, and other relevant duties as assigned. The Primary Counselor performs all procedures accurately, completely, and efficiently for clients, following applicable program policies and procedures. JOB DUTIES 1. Completes detailed bio-psychosocial assessments and/or evaluation of the client's history of psychological and/or psychiatric treatment (i.e., which shall include, but not limited to, previous admissions to psychiatric facilities, history of suicidal/homicidal ideation and attempts, outpatient psychiatric treatment, psychotropic medications, etc.) N.J.A.C.- 10:161b-9.1 (b)4., N.J.A.C 8:111) 7. Complete within three visits of admission a comprehensive biopsychosocial assessment of all clients using an assessment instrument which assesses, but not limited to (i.e. medical status, vocational/employment support, alcohol, tobacco and other drugs, legal status, family status, psychiatric status, and behavior risk factors for HIV and hepatitis. (N.J.A.C.- 10:161B-9.1 (a)). 8. If applicable, engages in full compliance with the Scope of Practice of an LSW, LAC, and LCADC but not limited to: Social Work Services, Clinical Social Work Services, Professional Counselor/ Associate Counselor, and Alcohol and Drug Counselor under the supervision of an LCSW, LPC or LCADC (N.J.A.C. 13:44G-3.2, N.J.A.C. 13:34-10.1 and N.J.AC. 13:34C 1.2-1.2). 9. If applicable, will complete DSM-5- TR diagnoses. 10. Initiate and complete the development of a measurable client treatment plan upon the client's admission and enter the client's treatment plan into the client record within 72 hours following the client's admission (N.J.A.C 8:111-9.2(a)1). 11. Revision and/or updates of client treatment plan(s) as assessments are completed, and/or new client information is obtained or on an as-needed basis (N.J.A.C. 8:11119.2(a)1i 12. Identify and document goals for discharge, which shall be incorporated in the client's treatment plan upon admission to the facility and shall address problems identified at admission and during treatment (N.J.A.C 8:111-18.1(a)1, 10:61B-17.1(a)1). 13. Initiate the development of the client's treatment plan upon admission and shall be entered into the client's record at least after three visits following the admission not to exceed 30 days (N.J.A.C.- 10:161B-9.2(a)1). 14. Initiate, implement, review and revise client's treatment plan to include DSM-5TR diagnosis, ASAM Level of Care Assessment, measurable goals, objectives and treatment outcomes (N.J.A.C.-10:161B-6.2 (a)11). 15. Shall document evidence of client participation in the development and implementation of the treatment plan, including, but not limited to, dated signatures of client as well as signatures of participating multidisciplinary team members (N.J.A.C.-10:161B-9.2(a)2vii). 16. Facilitates psycho-educational and/or process therapy group sessions on substance use and recovery-related topics in the Data, Assessment, Plan (DAP) documentation note taking format. 17. Provide detailed daily progress notes for all individual, group, and/or family counseling services in clients' electronic health record (EHR) at the close of the business day. 18. Provide case management duties (i.e., make referrals and coordinate scheduling of clients for evaluations and follow-up appointments with Eva's Village's psychiatrists and/or APN or other community psychiatrists/behavioral health providers). 19. Performs Level of Care (LOCI) extension requests utilizing the New Jersey Substance Monitoring System (NJSAMS) and American Society of Addiction Medicine Criteria (ASAM) 4th Edition. 20. Participates in a case conference with multidisciplinary treatment team to determine the best course of treatment for clients and to address all the client's needs. Shall review the treatment plan and client's progress at least every 30 days, with such review and revisions, if any documented in the client's electronic health record (N.J.A.C. 8:111-9.2d 21. Incorporates results of random drug and alcohol screening into client interventions and treatment planning process upon review with the client (N.J.A.C. 8:111-9.2(e), 10:161B-9.2(e)). 22. Shall document in the client's clinical record, the client and the client's family (if indicated and considered appropriate), participation in the development of the client treatment plan and continuum of care plan (N.J.A.C 8:111-9.2 (c) 1. 23. Maintains effective intra-office and inter-office communication within departments. 24. Participate in clinical staff meetings within the agency. 25. Maintains client Electronic Health Record (EHR) and assures up-to-date client documentation for mental health services provided. 26. Perform crisis intervention as clinically necessary. 27. Collaborates with Parole, Probation, Recovery Court, Eva's Village Recovery Court Liaison, etc., as clinically necessary. 28. Qualifications EDUCATION: High School Diploma or GED equivalent Bachelor's or Associates degree preferred Valid driver's license If in recovery, a minimum of two years clean EXPERIENCE: Thorough knowledge of 12-Step program, AA, NA, and the biopsychosocial model of substance abuse/dependence Ability to speak, read, and write English Must be able to work independently and complete assignments with minimal supervision Good interpersonal skills are required Must maintain a professional demeanor and perform duties effectively in stressful/emergency situations Must be able to maintain objectivity and deal diplomatically with clients and family members.
