Key Responsibilities
Client Support Executives obtain referrals for services as well as promote, educate, and market all company services. Serving as a liaison between Optimal Care and referral sources you will coordinate care for referred patients from home health, non-medical home care, hospitals, and other medical community partners. This position aligns closely with industry-standard roles including titles such as Hospice Sales Specialist, Hospice Care Liaison, Hospice Business Development Coordinator, or Account Executive, Hospice Services. The Patient Navigator plays a critical role in supporting Health Center patients throughout their care journey by coordinating diagnostic testing and ensuring seamless follow-up with outside entities. This position focuses on obtaining diagnostic results and closing the loop on patient referrals by proactively managing the external work queue, identifying referrals requiring attention, and facilitating communication between patients, providers, and external healthcare facilities. The Patient Navigator acts as a liaison to support both patients and the health center team, promoting timely access to services and adherence to recommended follow-up care
Key responsibilities include outreaching patients to schedule necessary appointments, assisting in overcoming barriers to care such as transportation or financial challenges, and retrieving diagnostic and referral results to ensure comprehensive documentation and continuity of care. Monitor, update, and follow up on patient referrals using external referral management systems to ensure patients are scheduled with outside specialists, services, or community partners. Address patient navigation needs flagged within the internal electronic work queues, providing a high level of support and guidance to patients throughout the care continuum. Identify and Reduce Barriers to Care. Provide support directly or connect them to community-based resources
Position Requirements:
* High School Diploma or GED Equivalent
* Associate's or Bachelor's Degree (preferred). Graduate of an accredited school of Practical Nursing preferred
Experience:
* Experience in healthcare, health center or clinical case management (preferred)
Licensure/Certifications: • MA, LPN, RN (preferred) • BLS certification in the first 6 months (employer provided)

Working in a Lake county school, the Counselor works independently to develop relationships with clients, families, school officials and other interested parties with meeting the students' mental and behavioral health needs. This role will provide assessments, individual, group and family therapy as needed while also acting as a strong advocate for clients and consultant with our school partners. An interdisciplinary approach offers the opportunity to collaborate with a diverse group of colleagues. The Counselor offers support, assessment, intervention, and consultation within the school setting across contracted districts.
Supervision towards licensure advancement is provided by the organization.
Primary Responsibilities:
* Provide mental and behavioral health services to children and adolescents in Lake County school systems.

Key Responsibilities
Physician Assistants provide primary and/or consultative professional care in a variety of care settings. The Physician Assistant will function in collaboration with physicians according to a Collaboration Arrangement. The Physician Assistant practice will focus on caring for geriatric and seriously ill populations to assist patients and families with comfort and quality of life.
In this role you will be responsible for:

Drives between locations, navigating to each location using GPS device or other tools if required.
Assess health status by obtaining relevant health and medical history. Performs a physical exam based upon the age and patient history as per the Collaborative Agreement with a collaborating physician in the patient's home environment. Identifies significant health and medical risk factors.
Performs Annual Wellness Visits and other quality driven assessments to provide patient with preventative services and understanding of disease progression.
Orders, interprets and evaluates laboratory tests and other diagnostic studies to assess patient's clinical problems and health care needs. Develops a diagnosis by analyzing and synthesizing collected data.
Performs diagnostic or therapeutic interventions consistent with certification, law and state medical privileges. May prescribe and monitor medications as allowed by prescriptive authority through the appropriate State Board Agency
Develops a treatment plan in collaboration with the patient and significant others. Participates in medical team rounds; collaborates with nursing, medical and other healthcare team members to ensure quality patient care.
Refers patients to physicians as appropriate for consultation or for specialized health resources and treatment. Ensures continuity of care by serving as a liaison between patient and other members of the multidisciplinary care team or with other specialty areas as necessary.
Provides patients and families an understanding of the diagnosis and disease progression while decreasing unnecessary emergency room and hospital visits.
Required Qualifications
Master's or doctoral degree as a Board-Certified Physician Assistant
Current State License as a Physician Assistant
