REACH Mission and Values
The REACH Program of Evergreen Treatment Services works with individuals experiencing homelessness and behavioral health conditions to help them achieve stability and improved quality of life. The REACH mission is to foster community health and safety through outreach, relationship, healing interventions and systems advocacy for people who use drugs. REACH provides outreach-based-care coordination, multidisciplinary clinical services, and supports to access and maintain housing. All services are based in principles of harm reduction that offer respect and dignity to individuals moving through stages of change in their lives.

Job Summary:
The Case Manager - LEAD role is part of the REACH team that supports ETS' mission through fostering cohesion by providing engagement and intensive case management services to individuals suspected of low-level drug offenses and/or prostitution.
The Case Manager will provide direct services to a case load of approximately 25-30 individuals. Case managers provide outreach, long-term engagement and supportive services for participants through intensive case management activities and collaboration with LEAD partners, service providers, housing providers and other community organizations.
The position is full-time onsite, and the days and hours are Monday-Friday 8:30am-5pm.
Responsibilities:
Help ETS succeed in carrying out our mission through working together with other staff to transform systems of harm and inequity to create different approaches to improving community health and safety through addressing substance use and homelessness.
Assist with the organization-wide initiative to reimagine our interconnectedness within our community to overcome the aspects of our society and organizational culture shaped by white supremacy and settler colonialism.
Provide Outreach and Intensive Case Management services for assigned participants:
Engage participants on the street and at social service provider facilities to establish a working relationship and offer services.
Assess participants for severity of chemical dependency and housing status and determine needs for other services, e.g., medical, mental health.
Assist participants in gaining access to a variety of funding programs (e.g., SSI, ABD, VA).
Assist participants in finding housing and maintaining occupancy.
Develop and implement with the participant's input an individualized Service Plan which addresses the needs of the participant for food, clothing, shelter, and health care and substance use disorder treatment or reduction/elimination of drug/alcohol use through self-change methods. Update this Plan periodically to reflect movement toward or attainment of articulated goals and the emergence of new participant needs and to help the participant move toward the achievement of autonomy.
Develop and maintain a working relationship with Sobering Center staff, DSHS workers, chemical dependency treatment providers, mental health providers, health care providers, shelter providers, landlords, detox centers, Assessment Center staff, protective or representative payees, and other community programs which may support participants.
Provide structured Intensive Case Management services consistent with program policies.
Develop and maintain collaborative relationships with LEAD partners including Seattle Police Department, King County Sheriff, King County and City of Seattle Prosecutor's office.
Provide advocacy and support for participants within the criminal justice system including court appearances and written communication.
Attend regularly scheduled Operational Work Group Meetings and the staffing of participants with partners.
Accompany participants to appointments as needed.
Assist participants in developing a spending plan and in shopping.
Advocate for the participant with a wide variety of other service providers:
Assist participants in gaining entry into service programs.
Develop relationships with housing resources and assist the participant in gaining access to appropriate housing.
Identify gaps and barriers in available community resources and advocate for systemic changes.
Attend REACH and LEAD team meetings and other required meetings.
Develop and maintain participant files for assigned caseload according to program, contract and state requirements.

Skills necessary to provide advocacy and support for participants within the criminal justice system including court appearances and written communication. Job Summary:

Job Summary:
The Hospitality & Engagement Specialist role is part of the Behavioral Health team which supports ETS' mission by providing supporting services to patients. This role makes a critical difference at ETS by assuring patients are provided a safe and therapeutic environment where they can access needed resources.
This position will foster relationships and community building by providing patient-centered services from a harm reduction perspective utilizing motivational interviewing, therapeutic verbal de-escalation, and trauma- informed care approaches. The central purpose of the Hospitality & Engagement Specialist is to help create a welcoming environment where patients feel a sense of respect and belonging and, in turn, access resources needed to improve their quality of life.
What you contribute to this role - Responsibilities:
Help ETS succeed in carrying out our mission through working together with other staff to transform systems of harm and inequity to create different approaches to improving community health and safety through addressing substance use and homelessness.
Assist with the organization-wide initiative to reimagine our interconnectedness within our community to overcome the aspects of our society and organizational culture shaped by white supremacy and settler colonialism.
Helps create a safe and therapeutic environment by letting patients in the door and welcoming them to the ETS space
Develops rapport and trusting relationships with patients through respectful, person-centered engagement practices.
Orients new patients to the space(s) and facilitates patient flow in and out of the space while maintaining a hospitable and friendly demeanor.
Meets with Patients within their first 2-days to review the ETS Clinic Services Safety Agreement and complete a Crisis Intervention Plan.
Monitors patients' activities inside the clinic and to the extent of ETS property with the intent to address any potential safety risks or violations of program rules and regulations.
Produces and distributes a daily email, prior to the beginning of dosing, that provides the clinic with needed information about the day's safety team rolls.
Submits incident reports or other relevant documentation when appropriate.
Monitors and reports hazardous materials and conditions.
Works in collaboration with Counselors and Case Managers to support individual service plans for patients with challenging behaviors.
Demonstrates highest level of integrity and maintain exceptional levels of confidentiality related to 42 CFR part II and HIPAA.
Responsible for clinic opening and closing procedures.
Conduct Bathroom Checks.
Assists with receiving and stocking supply shipments for the clinic.
Assists police, fire, EMS, and all other emergency service providers in the performance of their duties while responding to or on ETS property.
Responds to all facility alarm activations including but not limited to security, duress, fire and intrusion alarms.
Helps implement systems to support patients in the milieu space with behavioral issues.
Helps support large patient events and activities.
As needed, operate oversized RV-style vehicle(s).
Attends ETS team meetings and other required meetings.

* The LEAD Program Supervisor role is part of the Community Justice Law Enforcement Assisted Diversion (LEAD) team that supports ETS' mission through helping law enforcement divert individuals engaged in low-level drug, property, and sex work related offenses from the criminal justice system to case management services. These clients will be provided social services that are hoped to help cut down on their legal recidivism. This role is stewarding our program expansion in your designated area.
* This role makes a critical difference at ETS by taking responsibility for day-to-day supervision of LEAD case managers and Screening and Outreach Coordinators through regularly scheduled individual meetings and clinical supervision.
* The position is full-time onsite, and the days and hours are M-F 8:30 a.m. to 4:30 p.m.
* Supervisors have the option (not mandatory) of signing up to be the supervisor on call one week or weekend per month as part of their role.
Responsibilities:

Job Summary:
The Outreach Specialist role is part of the Outreach team that supports ETS' mission through engaging clients living on streets and encampments with low barrier resource navigation, maintaining engagement with case management services, and establishing partnerships with other providers and organizations to coordinate care.
This role makes a critical difference at ETS by conducting assessments and providing referrals to services such as shelter, case management, behavioral and physical healthcare, substance use harm reduction resources and education, and housing, if able. The North Outreach Specialist will be based in Seattle City Council District 5 region of Seattle working with the North Outreach Team. This position will work closely with the City of Seattle UCT to coordinate shelter referrals at eligible sites and partners in neighborhoods such as Lake City and Aurora Ave.
The position is full-time onsite, and the days and hours are M-F 8a- 4:30p

Job Summary:
The Screening & Outreach Coordinator role is part of the REACH team that supports ETS' mission through acting as the contact point for all referrals to LEAD case management.
As such, this position must ensure effective and efficient communication and collaboration between all the partners involved in referring and receiving referrals for LEAD. This position will provide screening, outreach, and engagement to individuals referred to LEAD. They will provide consistent response to the local police department and manage referrals from law enforcement and city officials.
The Screening & Outreach Coordinator will conduct street outreach as needed to engage referred individuals who have yet to engage with their assigned case manager.
The Screening & Outreach Coordinator must develop and maintain positive, collaborative relationships with all LEAD partners and other service providers in order to best serve LEAD participants.
The position is full-time in the field and will utilize our office space, and the days and hours are M-F; 8:30-5:00.
This position requires holding an on-call referral phone after-hours during the work week one week per month, staff in this role must be available to respond to diversion calls in during on-call shifts as designated.

* Meets with Patients within their first 2-days to review the ETS Clinic Services Safety Agreement and complete a Crisis Intervention Plan.
* Monitors patients' activities inside the clinic and to the extent of ETS property with the intent to address any potential safety risks or violations of program rules and regulations.
* Produces and distributes a daily email, prior to the beginning of dosing, that provides the clinic with needed information about the day's safety team rolls.
* Submits incident reports or other relevant documentation when appropriate.
* Monitors and reports hazardous materials and conditions.
* Works in collaboration with Counselors and Case Managers to support individual service plans for patients with challenging behaviors.
* Demonstrates highest level of integrity and maintain exceptional levels of confidentiality related to 42 CFR part II and HIPAA.
* Responsible for clinic opening and closing procedures.
* Conduct Bathroom Checks.
* Assists with receiving and stocking supply shipments for the clinic.
* Assists police, fire, EMS, and all other emergency service providers in the performance of their duties while responding to or on ETS property.
* Responds to all facility alarm activations including but not limited to security, duress, fire and intrusion alarms.
* Helps implement systems to support patients in the milieu space with behavioral issues.
* Helps support large patient events and activities.
* As needed, operate oversized RV-style vehicle(s).
* Attends ETS team meetings and other required meetings.
* Always follow all ETS policies and procedures.

What you bring to this role - Qualifications:
Education and/or Relevant and Lived Experience:
* High school diploma or equivalent required
* Undergraduate degree highly desirable
* Academic training in the social service or SUDP/SUDPT field highly desirable
Experience
* A minimum of two years of experience in work related to social work or outreach programs required
* Experience providing services to addicted individuals from a harm reduction perspective preferred
* Experience with case management, homelessness and co-occurring disorders highly desirable
Knowledge and Skills

* Sets up and breaks down a mobile treatment facility in an urban environment.

* Orients new patients to the remote TIM locations and patient flow in and out of the space while maintaining a hospitable and friendly demeanor.

* Conducts twice-daily safety inspection of mobile treatment unit both in the check-out and check-in of the vehicle to its secured location through the completion of a verified checklist; also monitors security cameras and control systems.
* Routinely maintains critical functions to the operation of the mobile treatment vehicle such as; filland-dump of gray water tank, tire pressure checks, fluid level checks, filling of the gas tank.
* Assists with receiving and stocking supplies for the mobile treatment unit.
* Assists police, fire, EMS, and all other emergency service providers in the performance of their duties while responding to the remote treatment location.
