Job Summary:

* The QI Specialist reviews data, identifies trends, and communicates all adverse events to the Clinical Quality Improvement Program Manager (CQI PM). Additionally, this role assists in training teams to reduce service quality related problems and increase awareness and understanding of culture of quality and participant safety
* The QI Specialist adheres to expectations of assignment and project ownership, and this position is expected to stay current with trends in quality improvement, patient safety and behavioral health practice transformation and integration
* The QI Specialist is expected to follow and champion principles of just culture, by creating an environment of psychological safety, impartiality, and system focused approach to adverse event reviews and situations

Quality Improvement & Clinical Reviews
* Enhance quality and services through data-driven evaluation and recommendations aligned with performance metrics
* Create and maintain relevant policies and procedures
* Support Quality Committees and facilitate related processes and reporting
* Conduct and manage audits (internal and external) across service lines, including data entry, tool development, and tracking corrective actions
* Participate in interrater reliability assessments and scoring standardization
* Execute internal reviews for documentation and quality compliance with regulatory and contractual standards

Job Summary:
The Hospitality & Engagement Specialist role is part of the Behavioral Health team which supports ETS' mission by providing supporting services to patients. This role makes a critical difference at ETS by assuring patients are provided a safe and therapeutic environment where they can access needed resources. This position will foster relationships and community building by providing patient-centered services from a harm reduction perspective utilizing motivational interviewing, therapeutic verbal de-escalation, and trauma- informed care approaches. The central purpose of the Hospitality & Engagement Specialist is to help create a welcoming environment where patients feel a sense of respect and belonging and, in turn, access resources needed to improve their quality of life.
What you contribute to this role - Responsibilities:

Helps create a safe and therapeutic environment by letting patients in the door and welcoming them to the ETS space
Develops rapport and trusting relationships with patients through respectful, person-centered engagement practices.
Orients new patients to the space(s) and facilitates patient flow in and out of the space while maintaining a hospitable and friendly demeanor.
Meets with Patients within their first 2-days to review the ETS Clinic Services Safety Agreement and complete a Crisis Intervention Plan.
Monitors patients' activities inside the clinic and to the extent of ETS property with the intent to address any potential safety risks or violations of program rules and regulations.
Produces and distributes a daily email, prior to the beginning of dosing, that provides the clinic with needed information about the day's safety team rolls.
Submits incident reports or other relevant documentation when appropriate.
Monitors and reports hazardous materials and conditions.
Works in collaboration with Counselors and Case Managers to support individual service plans for patients with challenging behaviors.
Demonstrates highest level of integrity and maintain exceptional levels of confidentiality related to 42 CFR part II and HIPAA.
Responsible for clinic opening and closing procedures.
Conduct Bathroom Checks.
Assists with receiving and stocking supply shipments for the clinic.
Assists police, fire, EMS, and all other emergency service providers in the performance of their duties while responding to or on ETS property.
Responds to all facility alarm activations including but not limited to security, duress, fire and intrusion alarms.
Helps implement systems to support patients in the milieu space with behavioral issues.
Helps support large patient events and activities.
As needed, operate oversized RV-style vehicle(s).
Attends ETS team meetings and other required meetings.

Develops professional rapport and trusting relationships with patients through respectful, person-centered engagement practices.

The TIM (Treatment in Motion) SUDP role is part of the Counseling and Behavioral Health team that supports ETS' mission through assessment, individual and group counseling, case management, community-based outreach and engagement, and overall patient care for patients enrolled in ETS' Treatment in Motion Program.

Job Summary:
The Medical Provider (ARNP / PA-C) role is part of the Medical Care team that supports ETS' mission through connecting individuals with Evergreen Services Dispensary, clinic-based services and coordinating overall patient care while they are enrolled at Evergreen Treatment Services.

What you contribute to this role - Responsibilities:
Help ETS succeed in carrying out our mission through working together with other staff to transform systems of harm and inequity to create different approaches to improving community health and safety through addressing substance use and homelessness.
Assist with the organization-wide initiative to reimagine our interconnectedness within our community to overcome the aspects of our society and organizational culture shaped by white supremacy and settler colonialism.
Serve as the primary medical care team coordinator for their assigned patients.
Conduct intake medical assessment through physical examinations, establish diagnosis and determine initial medication choice and dose. This can take place in the Mobile Unit, community or in a clinic.
Conduct patient dose evaluations and change medications as indicated. This can take place in the Mobile Unit, community or in a clinic. or via telehealth.
Make appropriate recommendations for follow-up.
Document findings and decision-making about care in electronic health record.
Draw blood for necessary laboratory testing and for pre- and post-test counseling where required.
Carry out HIV and Hepatitis-C testing if appropriate.
Facilitate appropriate communication and referrals with outside medical providers for needed care and follow-up.
Evaluate, as necessary, patients with urgent or emergent medical needs and determine if further medical assistance is necessary (e.g., calling Aid Car).
Write medical orders for all take-home privileges, seeking CSAT and DBHR exceptions as necessary.
With the development of Evergreen Treatment Services Primary Care Provider program, the Medical Provider - MD/DO role will be fully aligned to support and enhance the needs of the program, ensuring comprehensive and coordinated care delivery to patients.
The Medical Provider - MD/DO role provides an elevated level of care to patients as defined by the Medical Director or Associate Medical Director.
Future responsibilities included but may not be limited to:
Program Support: Collaborate with the Primary Care Provider program team to develop and implement strategies that enhance patient care and program efficiency.

Job Summary:
* The Outreach Team focuses on engaging clients living on streets and encampments with low barrier resource navigation, maintaining engagement with case management services, and establishing partnerships with other providers and organizations to coordinate care. The REACH Outreach Team conducts assessments and provides referrals to services such as shelter, case management, behavioral and physical healthcare, substance use treatment, harm reduction resources, and housing, if able. REACH services are person-centered, strengths-based, traumainformed, culturally relevant, and respectfully provided from a harm reduction perspective to promote client autonomy and dignity in how they access resources and services.
* The Behavioral Health Outreach Specialist will also be responsible for forging pathways and partnerships with outpatient mental health and substance use treatment providers, emergency departments, community-based organizations, and neighborhood businesses. With expertise in behavioral health services, this position will pair with REACH staff to engage people who are living outside with unmet mental health and substance use treatment needs. This position will coordinate linkage to the appropriate services according to the clients' needs, including but not limited to coordination with outpatient behavioral health programs (e.g. PACT and HOST) as well as crisis-oriented programs including HealthOne, Health99, King County Crisis & Commitments, the CARE team and Mobile Rapid Response Crisis Team.
* A critical function of this position is to conduct warm handoffs (accompaniment) from streets to behavioral health appointments, and from hospitals and jails to community placements. Additionally, direct services provided by this staff will include risk-reduction and safer use strategies, substance use disorder assessment, intervention, and linkage to medication for opioid use disorder (MOUD) and recovery support services.
* The Behavioral Health Outreach Specialist will serve as a subject matter expert for helping clients access behavioral health and substance use treatment, providing consultation on available services to other REACH staff.
* Behavioral health and substance use services will be provided in a holistic manner across the entire continuum of care, including traditional out-patient treatment settings, crisis care, and outreach-based services.
* The position is a full-time (Monday-Friday, 8-5pm), in person direct-service based position that will conduct outreach to individuals at various sites in Seattle and, at times, to sites throughout King County. * Harm Reduction Education and Counseling:
* Educate clients on safer drug use practices, overdose prevention, and accessing harm reduction resources (e.g., syringe exchange programs, MOUD, co-occurring outpatient programming etc.).
* Utilize Motivational Interviewing to provide brief, ongoing counseling and support to help clients set achievable goals related to their mental health, substance use and overall well-being.
* Short Term Case Management and Referrals:
* Assist clients in navigating social services, healthcare systems, and community resources to address their housing, employment, and healthcare needs. This includes helping clients schedule appointments and accompanying them to the appointments.
* Coordinate with external agencies and service providers to ensure comprehensive care and support for clients, including medical detox and inpatient substance use or psychiatric treatment when indicated.
* Outreach and Community Engagement:
* Conduct street outreach to engage with individuals who may be hesitant to access traditional services.
* Develop and maintain productive, respectful relationships with community stakeholders and other resources including SUD and mental health treatment providers.
* Identify gaps and barriers in available community resources and advocate for systemic changes that will facilitate easier access to care for individuals facing the most barriers.
* Participate in regional and local meetings that pertain to initiatives related to addressing the behavioral health needs of people experiencing homelessness or housing instability in King County.

Requirements
What you bring to this role - Qualifications:
Credential (License, Certification, Provider One, etc.)
Washington State Department of Health Licensed Substance Use Disorder Professional (SUDP) certification, other relevant licensure or educational experience preferred
Academic training in the social services field and in the areas of chemical dependency treatment is preferred
Education and/or Relevant and Lived Experience (if applicable):
Minimum 5 years' experience of working in direct patient care, public health or a related field
Knowledge and Skills:
An understanding of racial justice and social equity and a commitment to helping create an equitable environment for all ETS participants as well as fellow staff.
Ability to establish and maintain effective working relationships with participants, and staff from a wide variety of ethnic, socioeconomic, and cultural backgrounds
Ability to read and interpret general business correspondence, policies, and procedures, referral information, financial documentation, and applicable government regulations
Ability to write business letters, uncomplicated reports, instructions, and procedures
Ability to present information effectively and respond to questions from participants, staff, referral sources, and the general public
Ability to propose practical and compliant solutions and practice alternatives based on data obtained in audits and adverse events reports.
Ability to observe and detect clinical patterns in adverse events and relate them to review (audit) findings.
Ability to interpret a variety of instructions furnished in written, oral, schedule, or diagram format o Ability to solve practical problems and deal with a variety of concrete variables in situations where standardization may be limited. o Strong interpersonal skills and verbal/written communication skills

Additional Essential Information:
Physical Conditions and Requirements:
The employee is expected to be able to come to various ETS sites, support in person audits and program monitoring activities, provides staff in service trainings and participate in other quality improvement and participant safety activities.
The employee may be exposed to illicit drug residues and fumes or other biohazardous materials when carrying out job functions. There is also potential for exposure to bloodborne pathogens. ETS will provide employees with appropriate training to limit the risk of exposure to bloodborne pathogens. Policies and procedures are in place addressing each item specifically.
The employee is regularly required to sit; use hands to finger, handle or feel objects, tools, or controls; reach with hands and arms and talk or hear; frequently required to stand, walk, and kneel; occasionally to climb balance, or stoop; rarely to crouch or crawl.
The employee must occasionally lift and/or move up to 30 pounds. Specific vision abilities required by this job include close, color, and peripheral vision and the ability to adjust focus. The noise level in the work environment is moderate.
