This individual is a clinically competent Cardio-Vascular Advance Practice Clinician (APC) will directly report to the Director of APCs with a matrix reporting structure to the Executive Director of the Cardiac and Vascular Service Line. S/he must be able to effectively deliver care through demonstrated expertise to the Cardio-Vascular patient. The principle duties and responsibilities include but are not limited to: performing pre-procedure assessments, Histories and Physicals, education and counseling of patients on cardiac and vascular diseases and treatments, responsible for tentative diagnosis, order and provide preliminary interpretation of lab studies, post procedure management of patient, discharge patient.
Responsibilities:
Responsibilities
Provides timely and thorough pre procedural work up on all Cardio-Vascular patients to include orders, History and Physical, review of consent and lab tests. Provides post procedural orders, discharge planning and education on all Cardio-Vascular patients. Provides direct patient care to Cardio-Vascular patients and evaluates patient outcomes. Participates with evaluation and eminence of closure devices, sheath pulls and arteriotomy sites. Performs orders and interprets results of diagnostic and lab tests on all pre and post cardio-vascular patients. Provides training and education to the CCL Staff, ED and inpatient units, staff and community as needed. Actively participates in mock codes and cardiac arrests on cardiac units. Actively participates in review of PI data and all PI activities associated with the cardio-vascular patient
Assessment & Management
Assesses for risks associated with the care of the acute and complex chronically ill patient including: medication side effects, immobility, impaired nutrition, fluid and electrolyte imbalance, immunocompetence, invasive interventions and diagnostic procedures.
Orders and collects data using appropriate evidence-based assessment techniques, relevant supporting diagnostic information and diagnostic procedures where indicated.
Assesses for risks associated with the care of the acute and complex chronically ill patient including: medication side effects, immobility, impaired nutrition, fluid and electrolyte imbalance, immunocompetence, invasive interventions and diagnostic procedures.
Diagnosis
Collaborates with interdisciplinary health care team in making diagnoses of acute and complex chronic conditions.
Manages diagnostic tests through ordering, interpretation, performance, and supervision.
Formulates differential diagnoses by priority.
Diagnoses complications and orders appropriate interventions
Formulates Plan of Care
Identifies expected outcomes from diagnosis, and formulates and documents a plan of care to address complex acute and chronic health care needs.
Utilizes evidenced based practice guidelines and protocols in an individualized, dynamic plan of care that can be applied across the continuum.
Implements and modifies plan of care.
Prescribes diagnostic strategies and therapeutic interventions.
May perform advanced procedures consistent with privileges and competency validation.
Communication and Collaboration
Maintains ongoing communication and collaboration with interdisciplinary health care team.
Facilitates and communicates with patient, family and staff to promote continuity of care across the inpatient and outpatient continuum of care.
Documentation
Documents/dictates key components of patient's progress via daily progress note, transfer, and discharge summary, and or clinic note where applicable. (h&p, daily progress notes, plan of care, problem lists, procedure notes, acute event note, discharge summaries, in medical record per specific patient unit or service, death summary)
Documentation is timely, meets acute care compliance standards and captures patient acuity.
Professional Practice
Demonstrates Professional Practice behaviors including: preceptor/mentoring, education and instruction of graduate students.
Seeks opportunities for active engagement in research and the analysis of evidenced based practice.
Actively participates in Advance Practice Grand Rounds and specialty specific meetings.
Maintains CEUs, and membership in a professional organization.
Qualifications:
Minimum 3 years experience in clinical nursing. Minimum 2 years experience functioning as an APC in the critical care cardio-vascular setting, preferred settings include but are not limited to the Cardiac Catheterization Lab, Non-invasive stress testing, and Vascular procedural area. Must have strong clinical background with strong communication and motivational skills. Westside Family Healthcare's Rural Health Outreach Program serves Delaware's farmworker population-communities that often experience significant barriers to care due to language, transportation, and economic challenges. As a Federally Qualified Health Center (FQHC), Westside operates a mobile health unit that travels to farms, camps, and community sites across Kent and Western Sussex Counties. Through this program, the RHO team provides comprehensive, culturally competent healthcare services directly in the communities where patients live and work. Services include primary medical care, behavioral health, infectious disease screening and treatment, immunizations, health education, and care coordination. By delivering care on-site, the program ensures that farmworkers and their families receive the support they need-regardless of their ability to pay. The FNP will work on the mobile health unit, with clinical hours primarily in the evenings and weekends to align with the availability of the farmworker population. Some shifts may extend late into the evening depending on location and patient volume.
