Position Pay Scale: $15.30 - 15.75 per hour
Schedule: Sunday - Thursday (7:30AM to 4:00PM)

* Physical condition allowing for normal housekeeping duties including but not limited to; lifting up to 40 pounds, bending, twisting, stooping, reaching, grabbing, pushing, pulling, standing for or walking an entire shift.

Primary Function: Implement Fairhaven housekeeping procedures according to a daily schedule and plan in an effort to maintain, clean, sanitize, organize and enhance the appearance of the facility.
Essential Functions:
* Implements housekeeping procedures to meet residents' needs in compliance with department procedures, Fairhaven policies and federal, state and local regulations
* Stocks supplies on carts and in assigned store rooms throughout the building
* Cleans and organizes resident rooms, public areas and duplexes following proper procedures applicable to area of facility/campus assigned
* Notifies supervisor when supplies are needed, equipment needs repair, and when facilities need further maintenance beyond housekeeping responsibilities
* Follows facility safety procedures and Safety Data Sheets (SDS)
* Works in other areas of Support Services and assists other departments as assigned, including occasionally working different shifts as needed including weekends
* Responds as needed in the event of an emergency
* Reports unusual resident conditions to supervisor
* Participates in creating an atmosphere which allows for the privacy, dignity, cleanliness, safety and well-being of each resident
* Safeguards privacy and confidentiality of all resident health care information, which includes complying with all HIPAA regulations
* Acts as a customer service representative and public relations ambassador for Fairhaven with residents, families, vendors, volunteers and other visitors
* Interacts positively with all residents at all times creating an atmosphere which promotes a homelike environment
* Maintains, cares for and safely uses supplies and equipment effectively and efficiently per Fairhaven policies and procedures
* Maintains sanitary conditions; follows infection control and waste management procedures as they relate to this position
* Reports incidents of abuse, neglect or a violation of the resident's rights immediately
* Protects personal belongings of each resident, reports missing items and assists in attempting to locate them.
* Completes all in-service training requirements in a timely manner
* Completes all duties and tasks as assigned

* Works as directed during or after an emergency situation. May be required to work after normal hours to assist in the event of an emergency

Experience & Education or skills required:
* High school diploma or equivalent preferred, but not required
* Housekeeping experience in a Long Term Care Facility preferred

Fairhaven is seeking a full-time Human Resources Assistant to perform a variety of activities supporting the Human Resources function. This non-exempt position is 40 hours per week, (8:30-5:00pm) with some flexibility required. Applicant must be organized, detail-oriented and creative.
Responsibilities are primarily administrative, but also include orientation/on-boarding, assisting management with first interviews, participating in HR related meetings, and assisting the HR Director with employee-related events. This position will provide support to employees and managers on a variety of Human Resources issues. The HR Assistant requires daily contact with employees and a considerable degree of confidentiality, tact and initiative.
To be successful, the ideal candidate should enjoy "busy work" and exhibit strong administrative and organizational skills. Job Description
Primary Function: Assists the Director of Human Resources in the administrative duties of the Human Resources Dept.
to remain in compliance with federal, state, and local laws and regulations.
Essential Duties:
* Assists department in carrying out various human resources programs and procedures for all company employees. Provides administrative support to the human resources function as needed.
* Prepares new hire paperwork, enters employees in payroll system and establishes personnel files. Verifies I-9 documentation and maintains compliant records. Maintains all personnel files in compliance with applicable legal requirements.

* Prepares government reports related to EEO compliance or other HR functions.
* Completes all in-service training requirements in a timely manner.

Collect all types of laboratory specimens and reports as required by laboratory and clinic procedures and protocols.
Administer oral solutions according to established training (this includes, but not limited to: Glucola for glucose tolerance testing).
Ensures that patient identification information matches test request information consistently.
Process specimens including labeling, centrifugation, aliquoting, freezing, referral and preparing for transport as required by test order, laboratory procedure and laboratory protocol.
Perform data entry of patient information in an accurate and timely manner.
Process billing and insurance information.
Maintain patient and specimen information logs.
Provide superior customer service to all patients and clinic personnel.
Assists with the proper storage of hazardous materials, including monitoring and organizing reagent/supplies inventory.
Perform all non-patient facing duties, including inventory, stock supplies, sanitization, filing, and answering phone calls.
Maintain all appropriate phlebotomy logs in a timely manner, such as maintenance logs and temperature logs.
Maintain personal skill competencies and assures education needs are met to comply with licensing and accrediting standards and regulations.
Read, understand, and comply with company, laboratory and clinic policies, protocols, and procedures.
Performs other duties as assigned.
Travel to additional sites when needed.
Qualifications:
High School diploma or equivalent.
Phlebotomy certification.
1 year of phlebotomy experience.
Medical assistant or paramedic training preferred.
Customer service experience preferred.
Ability to draw blood utilizing sterile needles, vacutainer tubes, and other collection containers.
Good written and verbal communication skills with a strong attention to detail.
Ability to work independently.
Excellent competency with computer navigation and data entry.

* Responsible for food service including all kitchen operations, food preparation and dining areas.
* Develops and plans menus and standardized recipes consistent with government standards and other factors such as variety, seasoning, food availability, serving portions, known preferences, staffing and food costs.
* Develops and administers department goals and annual budget.
* Performs supervisory functions including planning, hiring, coordinating, organizing, and directing staff, department advocacy, performance appraisals, disciplines, staff development, new employee orientation, on going employee training, conducting staff meetings and other activities to foster good communications and strengthen employee morale

* Visits new independent Haven residents to discuss the food service program.
* Interacts positively with all residents at all times creating an atmosphere which promotes a homelike environment.
* Protects personal belongings of each resident and report missing items and assist in attempting to locate them.
