What You'll Do
As a Personal Care Resident Assistant (Level 1), you will:
Provide resident care in alignment with each Resident Assessment and Support Plan (RASP) and guidance from supervisory staff.
Assist with activities of daily living such as eating, bathing, dressing, oral hygiene, toileting, and mobility - always honoring safety, dignity, and personal preferences.
Help residents with light room tidying, laundry tasks, stockings, and telephone use.
Support activities programming as needed.
Assist in the dining room by serving drinks, delivering meals, and offering meal-choice support.
Order and charge supplies accurately using the electronic system.
Monitor and report changes in resident condition promptly to the Charge Nurse.
Obtain and document vitals, weights, intake/output, and maintain accurate electronic/paper records.
Uphold infection control, safety, sanitation, and regulatory standards.
Assist with admissions, transfers, discharges, and post-mortem care.
Participate in required meetings and follow all Corporate Compliance and HIPAA guidelines.
Perform additional duties as needed to support resident care and facility needs.

The ideal candidate will have:
At least 1 year of experience practicing as a Physician Assistant or Nurse Practitioner.
In collaboration with an attending physician, the Advanced Practice Provider is responsible for the direct care and follow-up of a population of patients that matches his/her delineation of privileges and skill set. This care is delivered in defined inpatient and hospital-outpatient areas.
The components of the role include clinical practice, consultation, professional development, education, and research. The NPs and PAs in these roles may be required to have acute/critical care education and certification. The appropriate education/certification will be determined by the scope of the role in a particular practice area. The clinical portion of this role will require increased level of accountability including managing a cohort of high acuity patients.
In addition to managing patient care, the APP may be required to utilize invasive interventions and procedures to promote physiologic stability. The role will also include off-shift responsibility. The DSP works directly with individuals who live in the home and must implement and document Individuals' goal and intervention plans. a) Inspect the site regularly to ensure compliance with safety regulations

c) Closely supervise Individuals in the community as per the individuals ISP and annual assessment
e) Report all allegations or suspicions of abuse to the Incident Manager via the IM cell phone

This position may require lifting depending on the needs of the individuals Because of the nature of this Agency, it is essential that the DSP believe in the philosophy of the Agency as well as be able to implement programs that are viable and essential to the individual's need. Critical Elements:

Ensure the Program Site is in Compliance with All Regulations: The Direct Support Professional must be knowledgeable and informed on all applicable regulations, bulletins, and health alerts. The Direct Support Professional must monitor the site with regards to compliance with these regulations. Examples of duties in this area are:
a) Ensure personnel records at the site contain all required information and filed appropriately at the site b) Ensure all medical appointments are completed within regulatory time frames and that all paperwork is filled out accurately and completely c) Complete all paperwork in a timely, accurate and thorough manner d) Ensure medication logs are accurate, match pharmacy labels, and are documented correctly e) Ensure medication questionnaires are completed for each medication and are signed by a physician and pharmacist f) Ensure controlled medications are counted and documented daily, and stored in a double locked area g) Ensure Individuals review their rights and sign the form annually h) Monitor the program site for compliance with all applicable regulations i) Correct and/or report all areas of non-compliance to the supervisory personnel
