**Hours** : 56 hours bi-weekly **Schedule** : day/evening/night, every third weekend This position is responsible for providing medication management, clinical support and consultative services in a complex pharmacy operation. This position partners with key stakeholders to drive patient and family education, medication safety, quality assurance, customer service, process improvement and regulatory compliance. This position may also participate in education of pharmacy students and residents.
**Responsibilities**
+ Provide clinical, formulary and pharmacotherapy expertise and act as a resource to students, staff and interdisciplinary members of the care team.
+ Provide clinical pharmacy services to all patients in assigned area using Standard Operating Procedures (SOP). This includes dispensing of medication, completing review for all patients, monitoring, adjusting patient therapies and documenting changes according to department policies and pharmacy regulations.
+ Act as resource for pharmaceutical care to patients, partnering with interdisciplinary care team members to ensure appropriateness of medication order and therapy, drug selection, formulary correctness and compliance any other formulary or departmental initiatives as appropriate.
+ Accountable for the distribution and/or preparation of medications, sterile products, compound IV mixtures and other extemporaneous orders as appropriate.
+ Maintain proficiency in the skills and theory of the practice of pharmacy by reviewing literature, attending in-services, seminars and conferences and actively participating in department clinical meetings and activities.
+ Participate in regulatory, quality assurance and process improvement activities as required.
+ Meets all regulatory requirements.
+ Uses available information technology and other resources to support day-to-day work operations
**Required Qualifications**
+ Minnesota Pharmacist License
**Preferred Qualifications**
+ Doctorate of Pharmacy
+ 1 year pharmacist experience in complex healthcare environment
+ Completion of an ASHP-accredited PGY1 residency program

Responsibilities
* Provide appropriate clinical care including patient counseling and prospective drug use evaluation to all patients receiving their medication therapy from the Pharmacy.
* Perform initial patient evaluation as it pertains to the prescribed therapies. Collect, analyze, interpret, and evaluate appropriate data relative to medication therapy.
* Evaluate the significance of signs and symptoms relative to disease processes and makes recommendations for the modification of drug therapy.
* Evaluate the significance and clinical importance of adverse reactions and complications associated with drug therapy.
* Monitor patient records for potential drug interactions as well as patient allergies.
* Provide counseling and answer questions from patients, family and health care professionals concerning drug therapy.
* Provide prescription drug therapy to patients/customers ensuring accurate labeling and dispensing of medications in a timely fashion.
* Identify and resolve patient specific medication related problems and issues in the patient's best interest.
* Ensure dispensing of prescriptions in accordance with all legal requirements, regulatory agencies, third party contracts, and Fairview Pharmacy Services Policy and Procedures.
* Responsible for error management activities.
* Responsible for accurate and complete billing to payers.
* Utilize all technologies to ensure accuracy and efficiency in the dispensing process.
Required Qualifications
* B.S./B.A. or PharmD in Pharmacy from an accredited school of Pharmacy
* Working knowledge of third party pay plans

Position Description:
Inpatient surgery pharmacist position at Mayo Clinic Hospital- Saint Marys Campus in Rochester, MN. In this role, the pharmacist provides exceptional care to patients throughout their peri-operative journey; including pre-operative, intra-operative, and post-operative phases of medication management. Decentral clinical services consist of collaborative practice with multidisciplinary teams, identification of drug-related problems, provision and documentation of recommendations through progress notes regarding optimization of drug therapy, admission and discharge medication reconciliation, transition of care planning, participation on discharge rounds, intermittent surgical team rounds, and direct patient care.
