LSS of WI and Upper MI is seeking a Human Service Professional to join our Interactions and skill building team! This is a part-time, benefits eligible role. This is a 32 hour per week role, taking place Thursday - Sunday.
Interactions provides supervised visitation for children in out of home placement in our office, clients home, or out in the community. The position also provides parent education as needed.
The role requires flexibility to include evening hours based on the needs of the families. The role does require working in the Eau Claire Community and the surrounding counties but will have the ability to work remotely for documentation and administrative tasks.

Accurately complete time reporting.

Complete documentation within required timelines and expectations per LSS, regulation, funders, and practice standards.
Maintain client records.
Make recommendations within the client team and scope of practice.
Testify at legal proceedings as required by law and/or role.
Attend client/recovery team meetings.
When supervision is a program requirement, actively engage in the clinical supervision process.
Attend and participate in staff development programs, including in-service training, staff meetings, and professional seminars.
Within your scope of practice, provide consultation to peers.
Work a flexible weekly schedule that includes some evening and/or weekend hours for client appointments. Work directly with Program Supervisor or Manager to set expectations for scheduling clients. Schedule is based on client need.
Understand productivity expectations. Meet or exceed the standards that have been established for the role and the program.
Perform other duties and special projects as required.

About the Role
As a primary health care provider working in collaboration with a licensed physician or physicians, the Advanced Practice Provider (APP) will assess the physical status of patients by means of interview, health history, physical examinations, and diagnostic tests. In accordance with medical protocols agreed upon by the physician, the APP evaluates test findings, makes initial medical diagnosis and initiates appropriate action to facilitate the implementation of the therapeutic plan consistent with the continued health care needs of the client.
What You'll Be Doing
The APP will obtain and document a complete and advanced health and development history from the patient and/or family members, and perform and document an advanced physical examination of the patient, delineate problems and present data.
In collaboration with a licensed physician or physicians, orders and/or performs special screening and diagnostic tests and procedures; analyzes all data collected; takes appropriate course of action as necessary.
Explains the necessity, preparation, nature and anticipated effects of scheduled diagnostic and therapeutic procedures to the patient/family. Participates in active patient teaching relevant to patient's concerns/diagnosis including but not limited to preventative health maintenance issues.
Assists in the formulation and implementation of the plan of care, which may involve various treatment modalities, prescribed medications, consultation with other health care providers, and referrals. Seeks consult with supervising physician when patient's progress does not meet anticipated and/or predetermined criteria.
In collaboration with a physician or physicians, identifies and manages acute and chronic illnesses in accordance with established protocols.
Documents data, patient assessment and plan of care as defined by the Associated Medical Professionals standards.
Maintains and improves professional knowledge and skills by participating in seminars, lectures, and continuing education programs.
Maintains and applies a working knowledge of medical documentation and correct billing procedures and healthcare regulatory compliance standards.
Provides formal and/or informal in-services to clinical staff as appropriate. May also be asked to assist in teaching students, residents and interns in complex techniques and procedures, which are unique to the cardiology setting.
Provides emergency treatment and initiates emergency life saving measures as appropriate, including BLS/CPR, and ACLS.

What We Expect from You
Graduate of an accredited NP/PA program.
AANP or ANCC/NCCPA board certification, license or qualification for state licensure is required.
Two years of Nurse Practitioner/Physician Assistant experience is preferred.
Must have current BLS certifications.

Essential Duties and Responsibilities:
Demonstrate commitment to the agency's mission statement and core values at all times. The mission statement is “360 Communities delivers safety and stability that improves lives.”
Provide community resource support at the Dakota County Housing Clinic (DCHC) during initial eviction hearings at Dakota County courts. Supports may also be provided at various sites across Dakota County.
Provide access to housing advocacy, emotional support, ongoing support and follow-up with clients to support housing stability.
Provide financial assistance navigation and application assistance for individuals and families facing eviction (e.g. navigate emergency assistance application process, refer client to other funding sources, or provide funding through 360 Communities financial assistance process).
Work cooperatively, build, and maintain relationships with partner agencies, Dakota County and other organizations to strengthen client experiences with community resources supporting housing stability.
Complete a thorough intake process with each client to determine appropriate services and referrals.
Provide follow up and short-term case management with clients to identify goals related to their financial and housing stability (i.e. budgeting tool, employment resources, debt management, help with accessing public assistance programs, etc.).
Provide appropriate service coordination in response to intake and goals, either through connecting to services within 360 Communities or through a referral basis to outside programs and agencies.
Follow all funding requirements and guidelines to ensure stewardship of funds made available to 360 Communities clients.
Ensure seamless communication with other agency programs (Violence Prevention services, Partners for Success , etc.) regarding families accessing services through 360 Communities Housing Support Services, to ensure holistic services to children, families and individuals.

The Health Unit Coordinator/Nursing Assistant (HUC NA) has a dual focus, a combination of the Health Unit Coordinator (HUC) and Nursing Assistant (NA) roles. They serve as a member of the interdisciplinary patient care team.
The HUC NA supports patients with activities of daily living and performs patient care skills as delegated by the licensed nurse within the Ohio Board of Nursing Scope of Practice.
The HUC NA also collaborates with unit leadership in coordinating the day-to-day activities of the patient care unit. Job Qualifications
An LSW or LPC license in the state of Ohio is required. An LISW or LPCC license is preferred.
Master's degree in Social Work or a social service discipline with a minimum of three years mental health and chemical dependency.
Essential Job Duties & Responsibilities
Provide individual and group therapy using evidence-based practices tailored to individuals with schizophrenia and other serious mental illnesses.
Conduct comprehensive mental health assessments, develop individualized treatment plans in collaboration with the client, and care team.
Deliver services with a recovery-oriented, trauma-informed, and person-centered framework that emphasizes client autonomy and community integration.
Coordinate care across behavioral health, medical, and housing systems, acting as a liaison among providers, case managers, housing staff, and families when appropriate.
Support housing stability to addressing behavioral health needs to impact tenancy including crisis intervention, symptom management, and medication adherence.
Collaborate with housing staff and property management to ensure clients maintain housing and receive appropriate wraparound service
Maintain accurate, timely documentation in compliance with agency, funding, and licensing standards.
Engage in regular interdisciplinary team meetings to ensure continuity of care and shared decision-making.
Provide crisis intervention and risk assessment, including suicide and violence risk, and coordinate higher levels of care as needed.
Participate in training and professional development to maintain licensure and stay current on best practices in behavioral health and supportive housing.

DIRECT CARE WORKER Position Defined:
The DIRECT CARE WORKER (DCW) is a key role and instrumental in providing individual and group support. The DCW spends their time helping clients learn to become independent. This position helps to ensure all programmatic features of client's Individual Treatment Plan is implemented, monitored and executed in the most consistent, nurturing and professional manner.
