SUPERVISOR'S TITLE: ACTT Team Leader
WORK SCHEDULE: Schedule to be determined by provider, minimum of 16 hours weekly. Occasional weekends and holidays
Ensures that the psychiatric needs of each client are met in appropriate time frames and serves as an additional clinical support to the team.
ESSENTIAL DUTIES:
Typically sees the beneficiaries for the assessment and treatment of the beneficiary's symptoms and response to medication including side effects. Frequency will vary for each beneficiary, with the majority seen within 4 to 6 weeks of last appointment, with many of the contacts being in the community. Less frequent visits should only occur when there are unusual circumstances present (for example, when the beneficiary has been difficult to find);
ACT Team psychiatrist provides clinical supervision and oversight of the psychiatric services delivered by the NP or PA;
Collaborates with the team leader in sharing overall clinical responsibility for monitoring beneficiary treatment and clinical supervision to the team;
Actively collaborates with nurses to develop and implement medication administration policies and procedures as well as oversee the medical care of beneficiaries that include regular screenings for medical conditions and assessment of wellness and health management;
Educates non-medical team members on psychiatric & non-psychiatric medications, their side effects, & health-related conditions; provides diagnostic and medication education to beneficiary, with medication decisions based on shared-decision making;
Regularly participates in daily team meetings and treatment planning meetings; attends daily team meetings in proportion to time allocated on team;
Provides brief therapy (formal or informal); and
Provides psychiatric back-up to the program after-hours and weekends. (Note: may be on a rotating basis as long as other psychiatric care providers who share on-call have access to beneficiary's current status and medical records/current medications).

LEVEL OF EDUCATION/TRAINING/QUALIFICATIONS:
ACT Team Psychiatrists must be board eligible or certified by the American Board of Psychiatry
and Neurology and Licensed to practice in NC and meet the requirements as specified in NCAC 27G .0104.
PHYSICAL DEMANDS: Regularly walk, stand or stoop; occasionally lift, carry, push, pull or otherwise move objects weighing up to 25 pounds; and regularly drives a motor vehicle.
WORKING CONDITIONS:
Work is performed in an office or other environmentally controlled room; Work may expose Intake Coordinator to contagious or infectious diseases; Work may expose Intake Coordinator to dangerous and volatile situations; which could result in bodily injury.

The Personal Care Aide (PCA) is a paraprofessional member of the client care team who works under the scheduling supervisions of the Client Care Coordinator and clinical supervision of a registered nurse, Clinical Manager, by providing selected personal care services and limited related household tasks. These activities will be part of the client's plan of care and must be provided under a physician's order.

Essential Job Responsibilities:
Implements and provides day-to-day Activities of Daily Living (ADL) and Instrumental Activities of Daily Living (IADL) support to clients within their home as ordered by the physician's plan of treatment and client's plan of care.
Appropriately assist clients with activities of daily living and personal care in accordance with the care plan including:
Assisting or performing personal care including: Baths (tub, shower, bed). Mouth care, denture care, skin care, foot care (no toenail cutting). Grooming (shampoo, comb hair, shave, file nails). Dressing and undressing activities.
Elimination (toilet, commode, urinal, bedpan).
Transfers.
Assisting with ambulation with or without mechanical aids.
Assisting the client with therapy exercises under the direction of a Registered Therapist
Appropriately assist with household tasks directly related to the client's health and in accordance with the care plan:
Maintenance of a clean, safe, and healthy environment.
Planning and preparing meals according to specific dietary requirements of the client, and if necessary, feeding the client.
Light housekeeping of client's immediate environment.
Laundry of clients bedding and clothing.
Assistance in completing activities such as shopping/grocery, errands, and appointments outside of the home.
Providing companionship and stimulation for the client including reading, walks, etc.; and accompanying the client to doctors or other appointments.
Utilize equipment in an appropriate and safe manner.
Remind client for self-administration of medication.
Do Not Administer Medications. Medications include eye drops, ear drops, ointments, pills, suppositories, injections, herbal remedies, and over-the-counter medications such as Tylenol, Aspirin, vitamins, and minerals.
Accurately documents shift details and services accurately within established time parameters.
Ensures the safety of clients and self. Follows Universal Precautions and demonstrates safe hand washing techniques.
Observes the client's general physical, emotional, and mental status, and report all changes in the client's conditions, family situation, or incidents during shift immediately to agency coordinator, supervisor, and client's designated representative (as instructed by employer).
Maintain and improve skills through attendance with staff development programs and inservice education requirements.
Maintain Confidentiality in all professional areas and consider the needs of clients and their families, other employees, physicians, visitors, and all members of the community in an informative, courteous, and compassionate manner.
Ensure all HIPAA regulations are adhered to.
Follow all agency policies and procedures.
All other duties as assigned.
ADA Requirements:
Routinely required to sit, stand, and walk in variable surfaces, including occasional climbing of ramps or stairs or the use of foot controls.
Routinely required to use hands for various tasks, including hand controls, gripping, and handling of objects.
At times, will be required to reach beyond mid-line of body, reach overhead or at chest level.
There are specific vision abilities required to perform this job, which include close, distance and peripheral vision, depth perception and ability to adjust focus.
All employees will be required to use universal safety precautions when performing their assigned tasks.
Employee must have the ability to lift, push, pull and carry a minimum of 25 pounds.
Employee must have the ability to stoop and bend up to 2.5 hours per 8-hour shift.
Requirements
Qualifications:
Licensure/Certification: Current certification/registration with the North Carolina Board of Nursing
Current PCA certification.
Be in compliance with all existing regulations and company policies.
CPR preferred and may be required for some clients.
Must meet transportation, language, and cultural requirements specific to branch needs.
PCAs are required to possess a sympathetic attitude toward the care of the sick, ability to read, write and carry out directions and maturity to deal effectively with the demands of the job.
Other Hours of Work: Varies on a case-by-case basis, variable hour employee.

As an AFL caregiver you will support individuals with disabilities in a home setting. Level of supports are based on the needs of the individual in your care. Responsibilities include, but are not limited to:
Seeking to understand the individual in the context of their personal history, their social and family networks, and their hopes and dreams for the future.
Supporting the individual in eating nutritious meals and snacks consistent with dietary needs, resting (by providing sleeping accommodations), daily self-care activities, leisure/recreation, education and working. Supports are provided according to the person's abilities and interests while ensuring privacy and dignity.
Preparing, teaching and assisting the person in food preparation, and cleaning of the home.
Honoring the choices, preferences and culture of the person you support.
Assisting in providing opportunities for social, emotional and spiritual needs to be met.
Assisting with transportation and scheduling of all medical, therapy, vocational and recreational activities.
Ensuring that the home environment is safe and in good condition.
Implementing services and supports to address goals identified through the Person Centered Plan. This includes medical, psychiatric, recreational, spiritual, and emotional needs.
Assisting the person supported by attending treatment team meetings and facilitating communication related to individual needs, response to service and supports, and progress toward goals.
Participating in supervision as agreed upon in the RHA supervision contract.
RHA will assist you in the following capacities:
Provide support through a multidisciplinary team for crisis assistance and training as needed as you begin your journey as an AFL caregiver. Training includes RHA's proprietary ProAct training, which focuses on supporting resiliency, emotional stability and problem solving.
Assist in coordinating respite care to support your needs as a caregiver.
Supervision will be provided to assist you in adjusting to your new role. The frequency and amount of supervision will be determined in a collaborative relationship with your RHA supervisor.
RHA's compensation package shall be determined based on the needs of the individual served and you will be paid as a contract provider.

The Direct Support Professional (DSP) is responsible for a variety of duties related to providing support and care for individuals living in Skill Creations, Inc. facilities, who have needs in the area of physical /medical care, behavioral support, cognitive support and independent living. The Direct Support Professional provides care according to the specific needs of the individuals. This includes but is not limited assistance and education with personal care, medication administration and independent living skills. Direct Support Professionals also help to transport/accompany individuals to activities in the community and to local or out of town appointments. Hourly pay for full and part time DSP positions in residential facilities begins at $17.60 per hour (
Back up and PRN Employee Positions are paid $17.00 per hour),

Key Responsibilities
Deliver purposeful, age-appropriate activities that promote independence, engagement, and personal growth
Implement training techniques and interventions to support social skills, personal hygiene, motor coordination, and daily living skills
Provide personal care and hygiene assistance in a respectful, person-centered manner
Supervise and support participants during all program activities, including providing coverage when additional assistance is needed
Encourage individuals to actively or partially participate in daily routines and program activities
Accompany participants on group outings, community activities, and work-based experiences
Transport participants using agency vehicles as required and serve as a backup van driver
Maintain program equipment and supply inventory and notify supervisors when reordering is needed
Complete accurate and timely documentation for assigned individuals by the end of each shift
Identify program challenges and recommend solutions that support quality services and positive outcomes
Attend required trainings and staff meetings to support professional growth and program excellence
Perform additional duties as assigned to support program objectives, participant safety, and team success
