3.0 NETWORKING:
Provides needed information to other staff, professionals, and team members in a professional confidential manner. Serves as advocate and/or liaison on behalf of individual served, the Developmental Services Division and UCS. Participates as a problem-solver in “team,” “care plan,” and other meetings.

MAJOR RESPONSIBILITIES:
1.0 DIRECT SERVICE:

2.0 RECORD KEEPING:
Completes all Developmental Services documentation in a timely and accurate manner including, but not limited to, billing appointments, progress notes, monthly summaries, and sequence sheets. Is familiar with each client's goal and writes quality, informative notes regarding progress and daily observations. Informs Coordinator and case manager if goal needs to be updated or changed.
3.0 NETWORKING:
Provides needed information to other staff, professionals, and family, as needed. Serves as advocate and/or liaison on behalf of individual served, the Developmental Services Division and UCS. Participates as a member of the individuals, Developmental Services, and UCS teams. Encourages and advocates for positive relationships with other community entities. Participates in treatment plan meetings and assists with establishing quality treatment plan goals. Encourages and ensures ongoing communication between team members.
