At Wood River Health, APP's provide the full range of primary care, urgent care, and acute care according to their training scope and credentialing. This includes responsibility for examination, diagnosis, and treatment of patients with acute, episodic, and chronic care, independently and as part of a healthcare team. APP's will collaborate with the Chief Medical Officer, or designee, in the management of care and participate in administrative and clinical meetings as scheduled.
APP's essential duties include, but are not limited to:
Prescribe or administer treatment, therapy, medication, vaccination, and other specialized medical care to treat or prevent illness, disease, or injury.
Order, perform, and interpret tests and analyze records, reports, and examination information to diagnose patients' condition.
Collect, record, and maintain patient information, such as medical history, reports, and examination results.
Monitor patients' conditions and progress and reevaluate treatments as necessary.
Explain procedures and discuss test results or prescribed treatments with patients.
Advise patients and community members concerning diet, activity, hygiene, and disease prevention.
Refer patients to medical specialists or other practitioners when necessary.
Coordinate work with fellow Advanced Practice Providers, Staff physicians, Chief Medical Officer, Registered Nurses, Social Workers, Rehabilitation Therapists, Pharmacists, and other health care providers.
Counsel patients about drug regimens and possible side effects or interactions with other substances such as food supplements, over-the-counter medications, and herbal remedies
Recommend interventions to modify behavior associated with health risks.
Detect and respond to adverse drug reactions, with special attention to vulnerable populations such as infants, children, pregnant and lactating women.
Educate patients about self-management of acute or chronic illnesses, tailoring instructions to patients' individual circumstances.
Plan, implement, or administer health programs or standards in hospitals, businesses, or communities for prevention or treatment of injury or illness.
Peer review and quality improvement activities.
Collaborates cooperatively with all personnel of the agency.
Maintain strict confidentiality in all matters
Operate within the scope of the Health Information Portability and Accountability Act to safeguard the privacy of protected patient health information

Education & Experience:
Graduate from an accredited NP or PA training program is required
A minimum of 2 years of primary care experience in either a family medicine or primary care outpatient practice is preferred
Ongoing CME to update skills/therapies, as per Rhode Island Department of Health regulations.
Ability to effectively use MS Office Suite, Internet, and email
Cultural sensitivity necessary to work with a diverse patient and staff population
Licensure & Certification:

Provide support in community settings and connect participants to community resources and community members to facilitate community integration. Examples of these supports may include but are not limited to: teaching a participant how to use public transportation, transporting participant to work, assist a participant with navigating their new workplace, assisting a participant with opening a bank account, identifying recreational activities in the community which meet an individual's interests and encourages/fosters connecting with friends, developing new relationships, volunteering, and community memberships.
Teach socialization skills and activities of daily living including but not limited to self-awareness and human development, menu planning, money management skills/budgeting; transportation and community orientation; individual skills; and utilization of community resources. Assist individuals with personal hygiene as needed to ensure that they present themselves in a culturally valued, image-enhancing way.
Administer and document individual medications as prescribed by a physician in accordance with agency and program procedures.
Record and appropriately utilize necessary data, daily progress notes, and complete all other ISP and program required documentation before the end of shift.
Transport individuals to and from community activities, job development activities, job-related activities, and other activities as required.
Provide opportunities for leisure time activities and foster independence in the choice and participation of those activities.
Provide a supportive environment that includes opportunities for participants to determine personal preferences for daily schedules and routines.
Ensure that all information regarding participants supported is maintained with the highest level of confidentiality, based on policies and procedures, within a manner based on dignity and respect to participants supported and regulations regarding participants' protected health information (HIPAA).
Maintain knowledge of ongoing program issues and developments as well as informing the supervisor of program issues and concerns.
Execute duties to reflect reasonable safety standards. Universal/standard precautions must be utilized, and training obtained in areas that constitute a risk.
Participate in core training and maintain certification of required training as identified by ESRI including but not limited to CPR, First Aid, MAP, and Crisis Prevention Intervention.
Provide crisis intervention and access emergency services as needed.
Attends and actively participates in ISP meetings, supervision, and staff meetings and assure/maintains a person-centered approach.
Ensure that participants we support are treated with dignity and respect in accordance with ESRI's Human Rights Policy.

Position Overview
As a Petco Certified Dog Trainer, you will play a vital role in furthering Petco's mission as a Pet Health and Wellness company by enhancing the bond between pet parents and their dog companions. Your expertise in modern dog training techniques and positive reinforcement methods will help pet parents develop well-behaved, happy, and healthy dogs. You will provide one-on-one training sessions, group classes, and offer advice on basic training, environmental management, and customized training plans to address specific training goals. In addition to dog training, you'll also be actively engaging with guests to provide 360-degree personalized solutions for the unique needs of pets and pet parents. You'll positively impact the health and wellness of pets by providing recommendations on healthy nutrition, world class services and essential supplies.

The Dog Trainer role consists of 2 core responsibilities:
Provide World Class Dog Training Services
* Attend and successfully pass Petco's 6-week Dog Trainer Instructor Program led by a Petco Certified Dog Trainer Mentor/Academy Instructor.
* Complete all assigned Workday Learning modules and DTIP Student Workbook materials per the DTIP roadmap.
* Recommend and sell appropriate dog training classes, training, and supplies to ensure pet parents and their dogs have the necessary resources for a successful dog training experience.
* Keep accurate records of training sessions, progress, and pet parents' feedback.
* Maintain confidentiality and professionalism at all times.
* Work cross functionally with the grooming and veterinary teams to provide holistic whole health solutions for all pet parents.
* Complete and apply ongoing training programs to maintain a high level of expertise in the role.
Provide 360-Degree Solutions for all Petco Guests
* Provide an outstanding guest experience by actively engaging with guests in the recommendation and selection of merchandise and services in accordance with their identified needs.
* Generate future business through a deep understanding of the guest and their pet(s) and connection to our digital platforms.
* Demonstrate a high level of awareness and knowledge for the health, welfare, and proper handling of all animals.
* Perform related duties to support the Pet Care Center in achieving its performance goals.
* Ensure merchandise is stocked, priced, and displayed to enhance the shopping experience for pet parents.
* Complete cash register transactions and support guests with their OMNI shopping experience as needed.
* Adhere to established operational guidelines, policies, and procedures.
* Promote a positive culture of teamwork, inclusion, and collaboration.
* Complete other duties and special projects as assigned.
* Evaluate guest inquiries and refer to the Leader on Duty as needed.

Certification Requirements
* Stage 1: All candidates are required to complete the 'Dog Training Instructor Program (DTIP)' to become Petco Certified Dog Trainer consisting of the following:
* Dog Training Apprentice: 6-weeks weeks of instructor led learning and development.
* Stage 2: Dog Trainer: Successfully run and grow the dog training business in one of Petco's Pet Care Centers with local and national marketing support, with access to internal partner resources, networking, and client acquisition through our state-of-the-art online booking and scheduling systems.
* Stage 3: Complete the Health & Wellness Certification Program for Dog.
* Note: Approved candidates with prior experience that can pass both skill and knowledge assessments may be eligible for accelerated training.
Basic Qualifications
* Passion for animal care, training, and welfare and a genuine interest for improving the lives of pets and their pet parents.
* Excellent communication and interpersonal skills to effectively interact with pet parents and provide clear instructions, demonstrations and applicable selling techniques.
* Demonstrated ability to handle and work with dogs of all sizes, ages, and breeds in a safe and humane manner.
* Strong organizational skills with the ability to manage multiple tasks and prioritize responsibilities effectively.
* Flexibility to work weekends, evenings, and holidays as needed to accommodate the training schedule and store operations.
* Physical stamina to stand, walk, bend, and lift heavy objects (up to 50 pounds) associated with dog training equipment and supplies.
* Familiarity with computer systems and software to manage training records, schedules, and communication with pet parents.

Collaborate closely with the Behavioral Healthcare, Developmental Disabilities, and Hospitals (BHDDH), the Office of Rehabilitation Services (ORS), and the Department of Developmental Services (DDS) to ensure alignment with regulatory guidelines and to access additional resources for individuals being supported. This involves regular communication, coordination of services, and staying updated with policy changes that may impact service delivery.
Act as a trainer and teach classes to further develop the skills and knowledge of staff to provide employment support to individuals.
Teach classes to individuals to ensure they are well-prepared for employment opportunities.
Perform any other duties as assigned by the Supervisor or Director, tailored to the specific needs of the program.
Qualifications:
· High school diploma or equivalent required; associate or bachelor's degree in human services or related field/one year experience working with people with disabilities and their families preferred
· ACRE certification preferred
· At least 18 years of age
· Possession of a valid driver's license, reliable transportation, and proof of valid insurance and car registration
· Ability to successfully pass criminal background checks
