The Clinical Care Coordinator is a master's Level professional who is responsible for providing clinical direction and therapy for intensive home-based stabilization services to families and foster families in their homes. The Clinical Care Coordinator develops treatment plans and coordinates timeline of implementation with insurance providers. Directs and coordinates treatment of the Clinician I, in providing therapeutic treatment to families and children. Coordinator collaborates with community service providers to provide overall clinical treatment.

+ **Exceptional Client Care** : Reviews and assists family in completing intake materials. Develops solution-oriented goals and assessments for treatment plans, from admission to discharge, with family and service providers.

+ **Strong Educator and Leader:** Coaches Clinician I counselors and interns to promote efficient and effective therapeutic performance. Communicates daily with Clinician I on therapeutic planning and schedules weekly supervision sessions, including co-family therapy meetings to assist the clinicians in developing family system dynamics.

+ **Excellent Communications:** Communicates effectively with families, colleagues and providers to promote increased collaboration

+ **Technical Familiarity:** Understands and is capable of understanding and using multiple technologies, in order to accurately and, in real-time, document necessary information

("The HARRIS CENTER") is looking for a Care Coordinator to join our team. This person is responsible for

What you will do:
  • Performs clinical assessments to determine service needs and progress with consumer treatment goals.
  • Participates in formulation of the consumer recovery plans and assures recovery plan is written and authorized with an LPHA signature for consumers on caseload.
  • Ensures all services provided meet eligibility criteria and are authorized in accordance with agency and regulatory guidelines.
  • Provides skills training and psychosocial rehabilitation services that address the severe and persistent mental illness and symptom related problems that interfere with the individual's functioning.
  • Provides interventions in response to crisis to reduce symptoms and avoid admission to a more restrictive environment.
  • Performs activities to assist consumers in gaining and coordinating access to necessary care and services based on individual needs.
  • Provides training/education on the nature of mental illness
