GENERAL STATEMENT OF DUTIES:

Under the direction of the Supervisor, and as part of a team, provides case management and/or supports coordination in the home, office, and community for children/adolescents with serious emotional disturbances (SED), developmental disabilities, and/or co-occurring disorders; develops the person/family centered plan with the child, family and natural supports; assesses, links, monitors, coordinates services; assists families in working with the school systems, the court, child welfare and other community resources; works with the family, other professionals and natural support networks to provide support and structure for the child in the home and community; performs related work as required.


MINIMUM REQUIREMENTS:

Bachelor in Social Work with a Michigan Bachelor Level Social Worker (LBSW) credential AND one (1) years experience in the evaluation and treatment of children or adolescents. Will consider a candidate with a Michigan Limited License Bachelor Level Social Worker (LLBSW) credential AND one (1) year experience in the evaluation and treatment of children or adolescents, OR a candidate with a Bachelor’s degree in a human service field (defined as Counseling/Guidance or Psychology), who also must possess a Bachelor Level Social Worker (LBSW) credential AND one (1) year experience in the evaluation and treatment of children or adolescents. The employee with a LLBSW must take and pass the ASWB Examination within four (4) years of hire to retain employment. The employee with a LLBSW must also make application for the LBSW upon completion of 4000 hours in order to retain employment.

Employee must maintain valid licensure to retain employment.


ADDITIONAL REQUIREMENTS:

Must retain DCH qualifications as a Child Mental Health Professional.

Must have a valid Michigan driver’s license, current automobile insurance, and the use of a vehicle during work hours. Ability to physically perform the tasks.

Wellmore Behavioral Health family case manager job description

Eligible families shall be referred from one of the local Department of Children and Families Area offices in the Region as part of the Differential Responses System and will require intensive coordination of multiple services to meet those needs. The age range for these children is from age 0 to 18.

Responsibilities

1. Manage a “caseload” of approximately 12 families.
2. Meet with families at a time and location that is most convenient to them. This may include doing home visits on evenings, weekends and holidays, as needed.
3. Maintain weekly in-person and/or phone contact with families served and document all contacts.
4. Perform family assessments using “Wraparound” principles and standardized forms (“Uniform Client Record” --“UCR”, etc.). This includes the identification of Family Strengths, Vision, Needs, and Strategies in the development of an Individualized Service Plan.
5. Assist families in setting up and convening Child Specific Team (CST) meetings – at least once per month – to develop, implement, and monitor progress on their Individualized Service Plan.
6. Help families to manage crises through the development and implementation of a proactive and reactive safety/crisis plan.
7. Maintain confidentiality in the management of records and communications regarding families – including obtaining appropriate consent to release information.
8. Attend school-related meetings (e.g. “PPTs”) with families, as needed – providing advocacy and support for the family at those meetings.
9. When appropriate, access “Flex-funds” to help meet family needs identified in the CST process and documented on the family’s service plan (within Flexible Funding Guidelines and available funds).
10. Develop knowledge of existing community resources and educate families on resources that will best meet their needs.
11. Provide families with an explanation of Family Advocacy Services. Refer families for family advocacy [e.g. “FAVOR”], when indicated.
12. Inform families about available Family Support Groups and other “natural supports” available to them in the community.
13. Complete case closure forms and ensure a smooth transition out of Community Support for Families. Obtain satisfaction surveys at the time of case closure.
14. Work in collaboration with the DCF Social Worker(s) -- when DCF is involved -- to ensure seamless, collaborative care management.

Requirements

College degree (BA, BS or BSW) or (MA, MS, MSW, MFT) in a human service field (e.g., social work, marriage and family therapy, counseling, or psychology, etc.).

Must have the skills necessary to meet the cultural and linguistic needs of the families to be served by the program. Bi-lingual (Spanish-English) is preferred, but not required.

A valid Connecticut driver’s license, personal transportation and the associated necessary insurance is required.

Salary: $45,000

Schedule: Full time, Monday - Friday, 8:30-5:00 PM, flexible schedule
