Travel required- most in person patient and provider visits will be in Virginia Beach. RN Clinician responsible for case management services within the scope of licensure; develops, monitors, evaluates, and revises the member's care plan to meet the member's needs, with the goal of optimizing member health care across the care continuum. Performs telephonic or face-to-face clinical assessments for the identification, evaluation, coordination and management of member's needs, including physical and behavioral health, social services and long-term services. Identifies members for high-risk complications and coordinates care in conjunction with the member and health care team. Manages chronic illnesses, co-morbidities, and/or disabilities ensuring cost effective and efficient utilization of health benefits; conducts gap in care management for quality programs. Assists with the implementation of member care plans by facilitating authorizations/referrals within benefits structure or extra-contractual arrangements, as permissible. Interfaces with Medical Directors, Physician Advisors and/or Inter-Disciplinary Teams on care management treatment plans. Presents cases at case conferences for multidisciplinary focus. Ensures compliance with regulatory, accrediting and company policies and procedures. May assist in problem solving with provider, claims or service issues.
Demonstrates the minimum knowledge, skills and abilities to care for the individualized needs of the patient to include physical, psychological, socio-cultural, spiritual and cognitive needs as well as functional abilities including the need for diversified use of such practices. The annual salary range for this role is $145,000 - $155,000/year, with a variable bonus, based on performance. Cardiovascular Invasive Specialist Responsibilities
Provides safe and therapeutic care in a holistic and caring way while incorporating differences into the provision of care. Integrates knowledge, skill and experiences to meet the patient needs.

Leads continuous quality improvement for clinical programming.
Provides clinical supervision to associate level and licensed therapists in individual and group settings, offering guidance and feedback to motivate and positively develop the clinical team.

What We Are Seeking:
Education and Experience
Position requires a Master's degree in Psychology, Social Work or closely related field and a minimum of 5+ years of experience within a behavioral healthcare setting. Position requires a LISW, LPCC, LPC-MHSP, LCSW, or LMFT. Must be licensed to practice and provide clinical supervision in the State of Virginia. Experience in treating Mental Health/SUD is strongly preferred.

Shift: This role is 5 days per week - 8-hour daytime workdays. It does include some weekends and holidays that rotate amongst the team. You cannot work remotely for this position. Currently, this position has no on-call requirements (i.e., needing to be on-call for certain shifts). However, during a scheduled weekend, Sundays are on-call.

Provides Medical Nutrition Therapy to individuals with specialized health conditions and or in specialized populations.
Uses the Nutrition Care Process to facilitate the provision of more complex nutrition care with moderate autonomy and supportive guidance from advanced practice dietitians.

Assess nutritional status, develop, and document care plans, monitor results and interventions according to AND Nutrition Care Process using the EMR.
Responsible for assessing nutritional needs including management of oral nutrition supplements, enteral nutrition, and parenteral nutrition.
Evaluate and monitor patient's estimated nutrition needs for various medical conditions.
Participates in interdisciplinary rounds with the care team, acts as the nutrition expert making evidence-based recommendations.
