CCBHC is an integrated community behavioral health model of care intended to provide integrated, evidence-based, trauma-informed, recovery-oriented and person-and-family-centered care. The CCBHC offers the full array of CCBHC-required mental health, substance use disorder (SUD) and primary care screening services. The Care Coordinator will work in partnership with others as part of a structured inter-disciplinary team, within in a Certified Community Behavioral Health Center (CCBHC) model to coordinate care across the spectrum of health services, including access to high-quality physical (both acute and chronic) and behavioral health care as well as social services, housing, educational systems, and employment opportunities as necessary to facilitate wellness and recovery of the whole person. Care is provided in a culturally sensitive way and is delivered to an adult population who carry a serious mental illness, substance abuse, co-occurring and/or medically co-morbid diagnosis.
Bridge Teams provide intensive, community-based services to individuals with severe mental illness. Clients served by Bridge Teams are transitioning from a recent psychiatric hospital discharge to outpatient services. Clients often have complex mental health needs, including acute psychosis, histories of trauma, and co-occurring substance use disorders. The primary function of Bridge Teams is to help reduce unnecessary hospitalizations and incarcerations, by providing individuals the resources essential to thrive and maintain independence in their natural environment.
CORE DUTIES & RESPONSIBILITIES
• Attend and participate in daily organizational meetings and treatment team meetings with multi-disciplinary team to utilize a team based care approach when working with clients.
• Document client progress to maintain a permanent record of client activity according to established methods and procedures.
• Demonstrate understanding of various best-practice treatment models to work with individuals with Severe Mental Illness, Co-occurring disorders, and trauma.
• Understand the wrap-around focus of CCBHC services, assisting clients in multiple life areas through multi-disciplinary teamwork and community referral.
• Obtain and maintain professional licenses and certifications required for the position through attending required trainings, tracking CEU's, and submitting renewals in a timely manner.
• Be familiar with CCBHC standards, understand the history, philosophy, structure and purpose of CCBHC and demonstrate fidelity to the model.
• Effectively complete and submit all clinical documentation with established agency timelines.
• Coordinate care across the spectrum of health service, including access to high-quality physical (both acute and chronic) and behavioral health care.
• Include the Consumer and family members in comprehensive treatment planning. With the adult consumer's consent, encourage participation of family members and other designated persons in treatment planning and care.
• Schedule, coordinate, facilitate daily/weekly treatment team.
• Participate in daily/weekly treatment teams. (daily huddles)
• Streamline plans of care across the Care Team.
• Coordinate care with external health care providers such as pharmacies, PCPs, FQHCs, and DCOs, as well as with behavioral health referrals. Assist consumers and their families with obtaining and keeping referral appointments to outside providers.
• Participate in initial treatment plans, strengths and culture preferences, and intakes as needed. Update and modify wrap plans as needed.
• Provide individual, family, and/or group services that include screening, coordinating, planning, advocacy, linkage, referral, education, crisis intervention, resource acquisition, and monitoring in a culturally sensitive way.
• Track consumers' admissions and discharges from psychiatric & medical hospitalizations.
• Participate in hospital and emergency department discharge processes to transition consumers to a safe community setting according to CCBHC protocols. Ensure timely transfer of medical records, prescriptions, and active follow-up.
• Provide high intensity case management and care coordination services to clients following a post psychiatric hospitalization discharge.
• Complete data surveys on clients.
• Monitor individual health status and service use to determine adherence to or variance from treatment guidelines.
• Develop and maintain housing resources for clients experiencing homelessness, including application processes and requirements, document submission, disability application process and submission. May also include outreach to clients in the community.
QUALIFICATIONS: Requires Bachelor's degree in social work or equivalent education. ODMHSAS CMII certification preferred.
SKILLS AND KNOWLEDGE: Must obtain Case Management II Certification and Behavioral Health Wellness Coach certification upon hire.
TRAVEL AND SPECIAL REQUIREMENTS: Must possess Driver License and use personal automobile for local travel. Must transport clients in personal automobile.

Collect and enter criminal justice data on WJT clients including assessment date, referral source, judge, criminal history, charges, children, etc.
Collaborate with WJT supervisory staff and program consultants around data collection, reporting, benchmarks and program outcomes.
Collaborate with WJT staff on system improvements to address data collection and data input.
Maintain data system expertise to maximize tracking and reporting benefits.
Develop and oversee tracking systems for special projects and new programs.
Management and Expansion of Grants, Contracts and Internal Benchmarks
Manage the SNAP grant for employment and education services as needed.
Develop and maintain employment outcome data collection and reporting.
Create efficient billing and invoicing procedures for SNAP.
Maintain monthly Participant Activity Sheets.
Submit and track reverse referrals for clients.
Track client SNAP eligibility and add-on support receipts.
Coordinate with leadership and staff to ensure compliance in documentation for all SNAP eligible clients.
Coordinate with finance department and state liaison on invoicing and monthly reports.
Administrative and Training Duties
Assist in establishment of administrative procedures with expansion of WJP programming and clients.
Provide ongoing staff training on established documentation and data collection procedures and proper utilization of tracking systems.
Establish procedures and protocols on proper documentation and utilization of data system.
Participate in staff meetings, in-service training, and workshops.
Perform other duties as assigned.

Under immediate to general supervision, the Peer Recovery Support Specialist (PRSS) provides peer support services in a culturally sensitive way; serves as an advocate; provides information and peer support for consumers in emergency, outpatient or inpatient settings. The PRSS performs a wide range of tasks to assist consumers in regaining control of their lives and recovery processes. The PRSS will possess the skills to maintain a high level of professionalism, ethics, and interactions with all ODMHSAS stakeholders. Demonstrates knowledge of co-occurring disorders and appropriate interventions. Services are predominantly delivered to an adult population who carry a serious mental illness, substance abuse, co-occurring and/or medically co-morbid diagnosis.

Under limited supervision, the Facility Maintenance Technician is responsible for the physical condition of agency owned properties and grounds to ensure a safe and reliable work environment for staff and visitors primarily at the Legacy Tower West, but will be going to all F&CS locations (owned or leased) as required.
DUTIES & RESPONSIBILITIES:
Facilities
Maintain data base record of all major equipment and systems repairs and maintenance
Maintains all equipment and schedules for equipment service calls when applicable;
Perform daily walkthroughs of facilities to monitor safety and repair needs;
Maintain building manuals, warranties and other documentation in organized and accessible manner
Oversees conference room schedules and usage which includes: furniture and equipment arrangements and act as primary contact with room organizers.
Electrical/Plumbing/HVAC
Monitor internal lighting systems for replacement of lights, safety issues, and completes maintenance and repair as required;
Monitor staff usage of personal lighting/heating equipment, ensuring that all meets UL and Agency standards;
Completes routine plumbing and HVAC maintenance/repairs and resolves emergencies and coordinates repairs to equipment as applicable;
Monitors and adjust as required heating, ventilation and air conditioning systems for staff comfort;
Oversees a regularly scheduled filter replacement program for HVAC equipment;
Performs routine maintenance.

Ensures compliance with all agency Safety policies
Complies with building and safety code requirements, including annual inspection of elevator and fire suppression equipment and extinguishers
Identifies and communicates any safety concerns with appropriate Department Supervisors

The Adult Education Teacher is responsible for the High School Equivalency (HSE) instruction of clients impacted by the criminal/legal system. Classes will primarily be held in county jails for men and women but may also take place in classroom or community settings post-release. Work one-on-one with staff and supervisors to assist with data entry, tracking system(s), documentation, program referral, and CCBHC quality measure reporting.
Analyzing trends and working with CCBHC leadership to use data to drive performance improvement efforts, at both an individual and population health level.
Report outcomes and participating in the CCBHC Advisory Committee, as needed.
Conduct end-to-end analysis that includes data gathering and requirements specification, processing, analysis, ongoing deliverables, and presentations.
Will review report requests and help prioritize and develop work plans to efficiently meet work requests utilizing available resources.
Perform other duties as assigned.

Assists pharmacist on duty as primary responsibility. Performs all duties requested by the pharmacist.
Performs minor calculations.
Enters data accurately and efficiently from new prescriptions sent via electronic prescribing, fax, and handwritten or transferred prescriptions.
Communicates effectively with nursing and clinical staff
Answers phone calls prioritizing customer service and assisting customers appropriately
Maintains patient profiles with most up to date information
Dispenses completed prescriptions under the pharmacist's supervision ensuring correct drug is filled and appropriate quantity is dispensed
Participates in a rotating workflow where you are able to do something different every day
Restocks the pharmacy shelves with supplies, drugs, etc., as needed.
Processes orders in computer for ordering of medications and supplies when necessary.
Processes returns, outdated items according to established procedures.
Develops a current knowledge of pharmacy policies and procedures and supports them.
Completes assigned training willingly and in a timely manner
Performs file maintenance of RX hard copies and invoices
Engages clients at pharmacy intake in a welcoming, friendly manner
Maintains good rapport with co-workers, patient care personnel, clients and others to ensure a smooth running pharmacy.
