Position Summary
The PT - Physical Therapist provides direct rehab care in physical therapy to patients upon physician referral in accordance with all applicable laws, regulations, and Life Care standards.
Education, Experience, and Licensure Requirements
Graduate of an accredited program in physical therapy, BSPT, MSPT, or DPT (2003 or later must have MSPT or DPT)

One (1) year experience in post acute care or related setting preferred

The Physical Therapist (PT, DPT, RPT) in Home Health is responsible for the assessment and evaluation of patient care needs related to functional status/mobility, endurance/stamina, pain and other medical conditions. Based on this assessment and evaluation, the Physical Therapist works to help determine a treatment plan, performs interventions aimed at improving and enhancing the patient's well-being, and evaluates the patient's progress

Treats patients to relieve pain, develop and restore function, and / or maintain maximum functioning using acceptable standards of practice.
Instructs and informs participating members of the health care team, the patient, and the family/caregivers regarding the plan of care, functional limitations, and progress toward goals.

Responsibilities
Essential Functions
Effectively evaluate the results of patient respiratory care delivered per care plans.
Accurately introduces, administers and charts the effects of routine respiratory therapy and medication administration.
Accurately setup, maintain, and chart the usage of oxygen therapy, aerosol/humidity therapy and ventilator therapy.
Provide ventilator support and airway management to neonatal, pediatric, adult and geriatric populations.
Perform arterial blood gas and capillary blood gas analysis (following Medical Director approval). Participates and performs blood gas quality controls per regulatory standards. Participates in blood gas proficiency studies.
May be trained in endotracheal intubation and arterial line insertion. May take a charge role.
Completes assessments on ventilator patients and all other patients on respiratory care services.
Will accurately chart on the respiratory flow sheets in the EMR and on the care plans. Will also charge accurately in the EMR.
Processes and maintains respiratory equipment.
