Job Description
The Care Coordinator manages and arranges care activities on behalf of Enrollees to facilitate the appropriate delivery of health care services and improve health outcomes, including but not limited to organizing care and facilitating communication across medical, behavioral health, social, and pharmacy providers, agencies and supports
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following:
Works closely with treatment team to collaborate aftercare.
Acts as liaison between the agency, enrollee and outpatient providers, community resources and supports.
Travels for home-based visits of enrollees.
Assists clients in assessing appropriate resources, programs and supports including completion of necessary paperwork.
Facilitates education sessions, including health and wellness coaching for individuals and groups.
Documents in an electronic health record member's information, activities, care plans and services.
Completes a Comprehensive Assessment and Care Plan for every engaged member. Work with Primary Care Providers on Care Plan approval.
Facilitates follow up calls.
Participates with total staff to implement program objectives and philosophy.
Attends meetings and trainings as required.
QUALIFICATIONS
High School diploma or equivalent required with at least three years' relevant professional experience; or
Associate degree and at least one year professional experience in the field; or
Bachelor's degree in social work, human services, nursing, psychology, sociology or related field.
Driver's license required.
Personal vehicle required (work mileage reimbursement provided).
Experience with treatment of enrollees with chemical dependence and/or psychiatric impairment.
Computer skills

Ability to function effectively as a treatment team member.
Evidence of good health, grooming and professional conduct.
Possess awareness of own strengths and limitations.

About The Role

In collaboration with the CEO, the Medical Director oversees and directs all medical teams in utilization management, population health and wellness, clinician engagement, safety and quality, and patient engagement. Duties & Responsibilities
Serves as the clinical spokesperson for the organization expressing the collective vision and principles endorsed by leadership and the board of directors.

Provides the leadership, management and vision necessary to ensure that the organization has the proper clinical controls, administrative and reporting procedures in place to effectively develop and manage goals and objectives.
Performs all duties as assigned by privileges issued and as an active member of the medical team.
Education & Experience
Current licensure as an MD/DO to practice in the State of MA by the State Board of Medical Examiners.
Current DEA, State Controlled Substance certification.
Three (3) or more years of supervisory skills.
Preference will be given to work experience in a public health or community clinic setting, previous works as a Medical Director, completion of an accredited primary care residency program, or Board Certification in Family Medicine.
Valid MA Driver's License.
Must be able to meet and maintain current credentialing and privileging requirements.
Experience with electronic medical records a plus.

The Controller is responsible for the direction of the facility's financial services including general accounting, payroll, budgeting and all other aspects of the facility's accounting department.
ESSENTIAL FUNCTIONS:
* Develop and implement financial systems, processes, controls and procedures for accounting functions.
* Coordinate and supervise activities of the accounting department.
* Prepare employees for greater responsibility by involving them in different areas of the department.
* Provide financial information to operating management in a routine and timely manner.
* Serve as a backup for the Payroll Accountant when needed.
* Develop and submit requested reports in a timely fashion that assists in the analysis of the business.
* Prepare and distribute daily revenue, cash and census reports.
* Assume responsibility for developing and achieving budget targets for the department and the facility.
* Monitor the expenditures of the facility to maintain compliance with the budget.
* Operate within the confines of the annual budget as adjusted for volume.
* Monitor daily deposits and reconcile with G/L posting to assure accuracy of daily reporting and facilitates month end processing and reconciliation.
* Organize and coordinate activities of the accounting departments as they pertain to the preparation of monthly financial statements and month end close.
