Job Description
Responsibilities:
Completes all required documentation accurately, in a timely manner, and thoroughly in accordance with department standards. Assists in preparing reports as required.-
Conducts initial and on-going assessment of client's health and/or support service needs. Sets level of client need.
Creates formalized case plans and goals with clients; clients are high-need and require more intense services. Develops a written care-plan with the client, identifying problems and needs. Plan includes areas assessed, intended interventions, and expected results in measurable terms, with short and long term goals. Updates plan as client's needs change.
Performs other duties as assigned.
Provides therapeutic interventions to clients. Provides intensive individual support to high need clients. Support may include providing therapeutic interventions, providing internal and community services referrals, and more intensive support may include accompanying clients to housing services, appointments, social services, etc. Focus on medical adherence, support, and appointments designed to maintaining necessary care.
Requirements:
3 years of work experience providing human services to high risk, medically underserved, disabled populations, and/or relevant community health populations required.
Bachelor's degree in Social Science field, Public Health, Healthcare Administration, or closely related field required. Master's degree in relevant field preferred.
Or equivalent combination of education and experience that provides the skills, knowledge and ability to perform the essential job duties, and which meets any required state or federal certification requirements.
For the AOD Services Department (159) Only: Current American Heart Association healthcare provider CPR (BLS), or Advanced Cardiac Life Support, Advanced Trauma Life Support, or Advance Resuscitation Training required.
Traveling between sites and other locations is occasionally required. Must have a valid California driver's license, an automobile, and proof of minimum levels of car insurance as required under California law, although limits of $100,000 are recommended. An acceptable driving record is also required. California law requires all drivers to obtain a valid California driver's license within ten days of establishing residency. Ability to work well in both a team-based environment and independently.

Under the supervision of the Program Manager, the Behavioral Health CaseManager is responsible for providing direct services and coordinating care for behavioral health patients with particular focus on those with complex needs. Specifically, the CaseManager works to ensure patients have access to and obtains the care and referral services needed to achieve and maintain optimal functioning. The CaseManager is responsible for providing casemanagement, rehabilitative services, and outreach to patients of SYHealth. In collaborating with Behavioral Health providers, the CaseManager is responsible for developing an evidenced- based, patient-centered treatment plan that supports the overall needs of patients. All Behavioral Health CaseManagers will participate in team huddle meetings, treatment team meetings, conduct presentations, and abide by SYHealth policies and program funding contract requirements at all times.
Essential Functions of the Job:
Provides direct services to patients, including casemanagement, rehabilitative services, warm handoffs, and/or psycho-educational groups.
Conducts comprehensive assessment of the patients' health and psychosocial needs, including health literacy status and deficits, and develops casemanagement plan collaboration with the patient and family or caregiver in conjunction with Behavioral Health Clinicians and/or Primary Care Providers.
Works collaboratively with other SYHealth Departments (e.g. Referrals, Social Services, Health Education, etc.) to facilitate and monitor referral requests and processing to ensure timely receipt of services.
Works closely with Behavioral Health Clinicians to identify patients who are eligible for services and in need of casemanagement and rehabilitative services.
Conducts phone follow ups to capture any potential patients previously referred by Primary Care Providers but that have not yet being schedule with Behavioral Health. Ensuring patients have access using in person and telehealth appointments.
Represents San Ysidro Health and Behavioral Health Department to other organizations in order to create and maintain partnerships with other community programs to improve referral outcomes and quality of care towards patients.
Conducts GPRA assessments as part of CCBHC program.
Maintains timely, complete, and accurate records with detailed assessment results, action plan, and progress notes. Clearly documents plans discussed with the patient, family, Behavioral Health Clinicians, PCPs, etc. in the electronic health record system to ensure that communication is available to all team members.
Assists in the preparation of monthly narrative and statistical reports covering progress toward meeting outcome and process objectives established in the Behavioral Health grant proposal and/or as required by various federal, state, and local programs.
Provides outreach and collaborates with local schools and/or other community agencies to identify patients who are eligible for behavioral health services and facilitates referral process.
Be the liaison and coordinate scheduling for Chronic Pain Management in Female Community Health Center Patients Grant. Funded by Office of Women's Health (OWH).
Maintains productivity of patient visits in accordance with clinic/program guidelines.
Complies with established departmental and health center policies and procedures, objectives, quality assurance program, safety, environmental and infection control standards.

Maintains close contact with patients throughout their care. Identifies the reason for missed appointments, and assists patients in maintaining continuity of care with appropriate interventions.
Educates patients, their families or caregivers, and members of the health care delivery team about treatment options, community resources, insurance benefits, psychosocial concerns, casemanagement, etc. so that timely and informed decisions can be made.
Maintains established departmental policies and procedures, objectives, quality assurance program, safety, environmental and infection control standards.
Attends meetings, trainings and community events as required and participates on committees as directed.

Ensure clients are enrolled in medical care and receive appropriate referrals to needed services as established in case plan. Services include, but are not limited to: medication assistance, housing, mental health counseling, substance abuse counseling, legal services, etc.
Advocate for clients and assist them in the procurement of benefits and services. Provide necessary information, referrals for established care plans and advocate for client needs

Experience Required:

Education Required:

Under the direction of the Chief Behavioral Health Officer, the Licensed Professional Clinical Counselor is responsible for performing psychological assessments and providing brief evidenced-based treatment using the primary care behavioral health model to adults, older adults, children and their families in a primary care setting. This licensed clinician works closely with primary care providers to develop and implement sound treatment plans. Responds to and provides crisis intervention services when needed; Ensures behavioral health services are delivered in a culturally appropriate fashion and is consistent with State of California laws and regulations, professional standards of care, agency guidelines, and professional ethical guidelines.
Essential Functions of the Job:
Evaluates, triages and provides evidence-based, brief psychological/behavioral health interventions to patients with physical health, mental health, and/or substance abuse diagnoses.
Provides consultation to clinical staff and primary care providers (PCPs) as needed regarding the patient's care, treatment recommendations and progress via documentation in the client's record, face to face or telephonic consultation.
Performs brief assessments of patient needs and, with patient and PCP, develops a negotiated care plan. Evaluates and/or revises care plan as indicated with appropriate documentation in progress notes and communicates this to relevant multi-disciplinary team members.
Maintains accurate and timely documentation in the electronic medical record of all patients seen, completes forms and documents as required by the department, company, state, and federal agencies, and produces reports of evaluation and/or treatment, as required.
Performs crisis interventions as needed including but not limited to conducting suicide assessments and makes recommendations to medical providers and patients determines if crisis response services are appropriate for patient safety.
Works in coordination with the Chief Behavioral Health Officer and Director of Behavioral Health Operations to provide information to healthcare providers about mental illness, behavioral health services, and other topics as requested.
Participates in meetings and committees, including but not limited to case conferences, treatment team meetings, peer review, multidisciplinary team meetings, etc. Participates in committees and work groups as directed.
Actively participates in quality assurance activities of the department, including regular peer review and development and revision of policies and procedures.
Maintains productivity expectations in accordance with clinic and program guidelines.
Works closely with all providers to establish full integration of services and team-based approach to care.
Establishes and maintains contact with community resources to ensure continuity of care (e.g. casemanagers, rehabilitation and detoxification centers, psychiatric units.)
Establishes and maintains effective working relationships with clients, family members, other professional staff and the public.
Performs any other job-related duties as needed, assigned, or requested.
Additional Duties and Responsibilities
Complies with established departmental and health center wide policies and procedures, objectives, quality assurance program, safety, environmental and infection control standards.
Demonstrates effective verbal and written communication skills in collaboration with teams to achieve quality, excellence, harmony, and problem-solving to obtain stated goals and objectives.
Enhances professional growth and development through participation in educational program, current literature, in-service meetings and workshops.
Assists in providing supervision of interns and/or unlicensed staff under specification of license as requested.
Coordinates with other behavioral health providers for on-site planned and unplanned time off coverage as needed to ensure continuity of patient care.
Promotes and fosters respect for patient privacy by maintaining confidentiality on all patient EHR data and abides by both the HIPAA Privacy and Security rules.
Job Requirements
Education Required:
Successful completion of terminal graduate training program in a psychotherapy-related field: Master's in Counseling or an equivalent program at an accredited University or College.
Experience Required:
Combination of education and years of experience as specified above. Sensitivity to multi-cultural concerns and the psychotherapeutic process required. Experience with community mental health preferred.
Certifications Required:
LPCC issued by the state of California.
Verbal and Written Skills Required to Perform the Job:
Bilingual in English/Spanish preferred
Excellent oral and written communication skills.
Communicate clearly and concisely, both orally and in writing.
Technical Knowledge and Skills Required to Perform the Job:
Knowledge of integrated treatment and care for individuals with co-occurring disorders.
Demonstrate ability to be culturally sensitive and appropriate in working with minority and underserved communities.
Demonstrate respect for diversity.
Knowledge of behavioral medicine, health psychology, substance use interventions including IMPACT and/or SBIRT model strongly recommended.
Exercise tact, objectivity, sensitivity, strategy and judgement in dealing with a variety of people with mental illness.
Organize, set priorities and exercise sound independent judgement within areas of responsibility.

Working Conditions and Physical Requirements:
Prolonged periods of sitting.
Provide leadership coverage at assigned SYHealth clinics as needed.
May be required to work evenings and weekends and attend meetings.
