Responsibilities:
Clinical & Nursing Care
* Conduct basic assessment/data collection, such as obtaining participant histories, vital signs, and other routine observations, within the LVN license scope. Observing and documenting participant responses to interventions, treatments, and medications, and reporting significant changes to RN or provider.
* Following and contributing to the interdisciplinary plan of care.
* Providing patient interventions per care plan under RN or provider delegation - e.g. dressing changes, skin tests (e.g. TB), phlebotomy (if certified), non-medicated treatments.
* Assist with wound care: cleaning, dressing application, monitoring for signs of infection, but not performing advanced wound care / sharp debridement.
* Provides administrative support to the RN Case Manager, including securing outside office notes, prescription documentation, follow-up appointment information, and all necessary pre-procedure and pre-operative instructions.
* Providing patient education as directed by RN or a higher licensure.
* Keeps clinic workflow moving, focusing on productivity and efficiency, including assisting during lunch times and breaks, ensuring exam rooms are set, ordering supplies and vaccines to ensure correct supply is on hand, supporting provider and RN case manager productivity, and calling in refills to pharmacy.

* Document medication administration accurately in electronic health record, including patient consent, patient response, and escalating any adverse reactions or errors in a timely manner and per policy.
* Request for pharmacy refills of medication.
Compliance, Safety & Quality
* Practice in accordance with California vocational nursing regulations, the California Business & Professions Cole, and organizational policies.
* Consistently follows policies and procedures to ensure positive participant outcomes.
* Follow infection control, safety protocols, and PACE emergency procedures.
* Report incidents, adverse events, or scope-of-practice concerns to the nurse manager.
* Maintain required certifications (e.g. BLS) and license in good standing.
Requirements:

* Traveling between sites and other locations is required as an essential function of the job. Must have a car, a valid California driver's license, and proof of minimum levels of car insurance as required under California law, although limits of $100,000 are recommended. An acceptable driving record is also required. California law requires all drivers to obtain a valid California driver's license within ten days of establishing residency. Implements patient education techniques such as: health coaching and teach back to support self-management. Identifies patient/family learning needs and acts to meet them.
Documents pertinent patient information, nursing procedures and patient responses in EHR, following established SYH guidelines and maintaining patient confidentiality.
Prepares treatment rooms and patients for examination.
Applies principles of aseptic technique and infection control as directed by the Infection Control policy and procedures.
Performs point of care testing, including but not limited to urine tests, pregnancy tests, A1C, and others as directed.
Assists the physician with therapeutic and diagnostic patient care functions which may require sterile technique, wound care, urine catheter insertion/removal, wound care, inhalation treatments, removal or application of dressings, EKG, visual testing, and audiometric testing.
May hold clamps or retractors, sponge vascular areas, and pass instruments as specifically directed by the physician.
Handles clinic use medication inventories and orders and replenishes medical supplies, medications, and materials throughout the day as needed. Maintains clinic inventory logs.
Prepares and administers medications, excluding IV medications, as directed by physician order and in accordance with SYH policy and procedure.
Participates and responds, when assigned to clinical emergency codes.
Autoclave: oversight of process to ensure the cleaning, processing, sterilizing, and checking of instruments complies to meet audit and infection control requirements
Immunization coordination: oversight of clinic's daily routine workflow, excursion reporting process and emergency vaccine management plan.
Utilizes effective communication methods and skills, following lines of authority as appropriate.
Demonstrates knowledge of and applies safety principles as identified by San Ysidro Health.
Demonstrates knowledge of legal issues, including patient confidentiality and risk management in all aspects of patient care and unit functioning.
Ensures the clinical area is audit prepared by completing monthly Quality Assurance packet and addressing deficiencies.
Participates in unit and department Performance and Quality Improvement activities.
Provides assistance with nursing schedule and demonstrates flexibility in the resolution of staffing issues; demonstrates flexibility when floating to other units upon requests.

RNs in the 47 bed Emergency Services at Scripps Memorial Hospital La Jolla will get hands-on experience working with comprehensive stroke, LVAD, Level I trauma and ECMO program. RNs have an array of educational opportunities and are actively involved in process improvements. The Emergency Services RNs at La Jolla are trained as MICN (Mobile Intensive Care Nurses) and have the opportunity to work in a designated Base Station providing direction and support to medics. Additional expectations include but not limited to:
* Provides direct patient care within the scope of his/her practice.
